Облігатні стандарти акредитації підрозділів патологоанатомічної служби України та показники її ресурсного забезпечення

Анотація
Результати: високотехнологічні процеси можуть бути тільки в установах (підрозділах) патологоанатомічної служби найвищого рівня. В системі оцінки та акредитації патологоанатомічних бюро не оцінюються функції, декларовані та делеговані бюро: організаційно-методична, наукова робота, виховання нових кадрів. При проведенні акредитації бюро фактично оцінюються вибіркові відділення, як правило, розташовані в місті, мають кращі показники забезпечення та обслуговуючі установи третинного рівня. Тому визначення єдиного рівня для патологоанатомічного бюро не є зваженим і обґрунтованим. Висновки: визначення рівня допомоги, що надається і рівня акредитації для кожного відділення окремо, або оцінка всіх структурних підрозділів бюро і видача єдиного сертифіката бюро з перерахуванням всіх підрозділів і загальної середньої оцінкою (третинного рівня при оцінці більше 50% відділень як теоретичного, вторинного рівня в інших випадках).
Результаты: высокотехнологичные процессы могут быть только в учреждениях (подразделениях) патологоанатомической службы высочайшего уровня. В системе оценки и аккредитации патологоанатомических бюро не оцениваются функции, декларируемые и делегированные бюро: организационно-методическая, научная работа, воспитание новых кадров. При проведении аккредитации бюро фактически оцениваются выборочные отделения, как правило, расположенные в городе, имеют лучшие показатели обеспечения и обслуживающие учреждения третичного уровня. Поэтому определение единого уровня для патологоанатомического бюро не является взвешенным и обоснованным. Выводы: определение уровня оказываемой помощи и уровня аккредитации для каждого отделения отдельно, или оценка всех структурных подразделений бюро и выдача единого сертификата бюро с перечислением всех подразделений и общей средней оценкой (третичного уровня при оценке более 50% отделений как третичного, вторичного уровня в других случаях).
Results: the pathology service is an important subdivision of the medical branch for life-time diagnostics of diseases, especially tumors, and expert-diagnostic evaluation of treatment by autopsy. Domestic standards and protocols, guidelines for the methodology of pathologist research and technology are absent. The methodology of accreditation of the pathoanatomical service in Ukraine is not coherent in view of the imperfection of the regulatory framework, the lack of standards for the provision of the services of the pathoanatomical service and the criteria for their evaluation, the lack of implementation of a quality management system in accordance with the requirements of the standards ISO 9001 / DSTU ISO 9001: 2009. In general, compliance with technological processes is not evaluated, the quality of pathogistological drugs is not assessed, the quality and completeness of the findings of studies, there are no requirements for environmental safety. Certain accreditation requirements in Ukraine (timely completion of the protocol of the pathologoanatomical section, the timing of the manufacture of histological preparations of autopsy material, the accounting of histological preparations of autopsy material, taking into account the implementation of additional histochemical, immunohistochemical methods of study) can not be estimated, as documentary evidence has not been developed and documented. Compliance with the standards of pathoanatomical research is not a resolved issue in the country. In the current requirements for certification at the PASS institution (institutions), there are no issues of control of inappropriate products / services provided by pathologist studies and autopsies. Existing indicators can not reflect the quality of pathoanatomical research. But extremely important indicators of professionalism, quality of provided assistance and safety are the following indicators: the differences between primary and repeated pathologist studies in lifetime diagnosis, the percentage and causes of differences in intraoperative study and the final pathological findings, the share of descriptive findings. High-tech processes can be only in the institutions (units) of the highest-level pathoanatomical service. Experts testify that the quality of pathogistological research is impossible without adequate provision of reagents, consumables, and at high intensity of work and in volumes that exceed a certain limit, without the automation of certain areas of the process. The necessity of scientific comprehension of the next direction of management in the field of medicine, which is the subject of our study, namely, obligatory accreditation standards of the units (institutions) of the pathoanatomical service and indicators of resource support is indicated.The system of evaluation and accreditation of pathoanatomical bureaus does not evaluate the functions declared and delegated by the bureau: organizational and methodological, scientific work, education of new cadres. During the accreditation of the bureau, the sample offices, usually located in the city, are actually assessed, have the best support indicators and tertiary level service institutions. Therefore, the definition of a single level for the pathoanatomical bureau is not weighted and justified. Conclusion: determining the level of assistance provided and the level of accreditation for each department separately, or evaluating all structural divisions of the bureau and issuing a single bureau certificate, listing all units and the overall average score (tertiary level when assessing more than 50% of offices as tertiary, secondary level in other cases). The proposed requirements for the institutions (units) of the pathological service are detailed not only in terms of content, but also with the criteria for evaluation and for different levels of provision of medical care.
