dc.description.abstractУ статті представлений огляд літературних джерел про формування, особливості клінічних проявів та психосоціальної реабілітації аутоагресивної поведінки при першому психотичному епізоді, впливу біологічних ритмів на розвиток психічної патології. Питання першого психотичного епізоду, а також аутоагресиної поведінки при ньому є об’єктом пильної уваги в сучасній психіатрії, проте недостатньо вивченими залишаються аспекти впливу біологічних ритмів на умови їх виникнення та перебіг захворювання. Подальше вивчення біоритмологічного аспекту формування аутоагресивної поведінки при першому психотичному епізоді дозволить якісно покращити діагностичні, лікувальні та профілактичні заходи.uk_UA
dc.description.abstractВ статье представлен обзор литературных источников о формировании, особенностях клинических проявлений и психосоциальной реабилитации аутоагрессивного поведения при первом психотическом эпизоде, влиянии биологических ритмов на развитие психической патологии. Вопрос первого психотического эпизода, а также аутоагресинои поведения при нем является объектом пристального внимания в современной психиатрии, однако недостаточно изученными остаются аспекты влияния биологических ритмов на условия их возникновения и течение заболевания. Дальнейшее изучение биоритмологичного аспекта формирования аутоагрессивного поведения при первом психотическом эпизоде позволит качественно улучшить диагностические, лечебные и профилактические мероприятия.uk_UA
dc.description.abstractThe first psychotic episode is a serious disorder that manifests at a young age. In today’s presentation it can be divided into three stages: primary, actually psychosis and replacement. The first stage is a stage of high risk and includes psychopathological and characterological change: impaired thinking, violation of effector-volitional, emotional abuse, disorders of perception. The second stage - the stage of actual mental disorder, which often appears paranoid, hallucinatory-paranoid, depressive, manic or paranoid paranoid syndrome. The third stage is reduced social function¬ing, ongoing biopsychosocial help prevent relapse, repeated suicide attempts, disability. The second stage is formed in many patients autoaggressive behavior.Modern research has shown that autoaggressive behavior occurs most often when the manifest acute polymorphic psychotic disorder and paranoid schizophrenia. In some patients experiencing suicidal tendencies arising out produc¬tive psychotic symptoms. Such patients follow their behavior delusional plot or carry out orders peremptory “votes.” Women are more likely to commit suicide by self-poisoning (12.5%), while men dominated traumatic ways of suicide: stab wound of the chest and abdominal cavities (11.6%), hanging (7.7%), samoporizy (7.7 %) and combined methods. In the treatment process can also be divided into three stages. In some patients marked aggression against the back¬ground of hallucinatory-paranoid syndrome, for example, to release the persecution of any outside influence, correct “disease or defect.”In conducting drug therapy in patients with aggressive behavior in primary psychotic episode distinguish 3 stages. The first stage - the relief of acute psychotic symptoms and normalization of the patient’s behavior - begins immediately after diagnosis and ends with the establishment of clinical remission. This stage usually lasts 6 to 8 weeks. The second stage - stabilization - is to continue effective antipsychotic therapy until the complete remission or significant reduction of productive symptoms and recovery, if possible, a former social adaptation of the patient. The stage can last up to 6 months of acute phase of the disease. Each subsequent psychotic episode, in addition to significant expenses in case of hospital treatment, leads to disease progression, significant deterioration of cognitive functions of the patient, and as a result - to early disability. Almost 75% of patients are disabled for 3 years after the first psychotic episode and 83% of patients require full social support for 5 years after the first psychotic episode.At the same time, there are many studies on the impact of biological rhythms on the formation of mental disorders: a link between the seasons and exacerbations, treatment regimens are designed with individual biorhythmological status of patients. But autoaggressive behavior during the first psychotic episode in terms of biological rhythms studied not enough. Therefore, the authors believe that further study of this issue will significantly improve the diagnostic and therapeutic and preventive measures in the first psychotic episode.uk_UA
