Короткий огляд (реферат): Дослідили особливості варіабельності серцевого ритму (ВСР) у дітей народжених передчасно. Визначили, що для дітей народжених передчасно притаманне симпатичне переважання та пригнічення парасимпатичної активності ВНС з перенапруженням адаптаційних можливостей протягом доби, про що свідчить зниження показників SDNNі та rMSSD та збільшення стрес-індексу. Також спостерігалось збільшення вагосимпатичного співвідношення протягом доби (LF / HF 3,9±0,2 в денні години та 4,1±1,3 в нічні години) та зниження ТР в 2 рази в основній групі порівняно з контрольною групою, що також відображає зниження парасимпатичного тонусу (р<0,05). У більшості дітей народжених передчасно у вихідному вегетативному тонусі протягом доби переважала помірна симпатікотонія, а у близько 20% дітей народжених передчасно – виражена симпатікотонія (р<0,05). В динаміці при катамнестичному спостереженні зберігалось превалювання симпатичної ланки ВНС.
Исследовали особенности вариабельности сердечного ритма (ВСР) у детей рожденных преждев- ременно. Определили, что для детей рожденных преждевременно, характерно симпатическое преобладание и угнетение парасимпатической активности ВНС с перенапряжением адаптационных возможностей в течение суток, о чем свидетельствует снижение показателей SDNNі и rMSSD и увеличения стресс-индекса. Также наблюдалось увеличение вагосимпатического соотношения в течение суток (LF / HF 3,9 ± 0,2 в дневное время и 4,1 ± 1,3 в ночное время) и снижение ТР в 2 раза в основной группе по сравнению с контрольной группой, что также отражает снижение парасимпатического тонуса (р<0,05). У большинства детей рожденныхь преждевременно в исходном вегетативном тонусе в течение суток преобладала умеренная симпатикотония, а у 20% – выраженная симпатикотония (р<0,05). В динамике при катамнестическом наблюдении сохраняется превалирование симпатического звена ВНС.
The purpose of the work is to study the peculiarities of cardiac rhythm variability (HRV) in prematurely born children. Object and methods of research. 95 children were examined in the Neonatal Pathology Department of the Zaporizhzhya City Children’s Multidisciplinary Hospital №5: the main group – 75 infants, who were born prematurely, their average gestational age was 33 [31; 35] weeks; control group – 17 term infants, average gestational age was 39 [39; 40] weeks. In the structure of the diseases in the main group perinatal defeats of the central nervous system were prevailing. We analyzed HRV using time methods (SDNNi, RMSSD, pNN50), frequency methods (LF, HF, LF \ HF, TP), and the method of variational pulsometry (Mo, aMo, ΔX). Catamentistic observation included the vegetative status evaluation of 20 prematurely born children at a reconstructed age of 3-4 months. Statistical analysis of the results was conducted using “Statistica 6.0”. Results. During the Holter monitoring conducting of the studied children, violations of rhythm and conductivity were not recorded. The circadian index in the whole observation group was within the range of 0.98-1.1 (1.07-1.12 in recalculation, taking into account the periods of sleep and wakefulness). The autonomic nervous system (ANS) status of the prematurely born children was characterized by the increasing of the sympathetic tone with a decrease in parasympathetic modulation, as evidenced by the SDNN and rMSSD decreasing (40.3% at daytime and 31.4% at night, p <0.05) and increased stress -index (SI) (2.5 times at daytime and 2.2 times at night, p <0.05). It is determined that the smaller the gestational age was, the lower SDNNi and rMSSD were and the higher stress index was (R = 0.6, p <0.05). This indicates a sympathetic predominance and parasympathetic activity of the CNS inhibition with overstrain adaptive capacity throughout the day in children with lower gestational age. There was also the vagosympathic ratio increasing during the day (LF / HF 3.9 ± 0.2 at the daytime and 4.1 ± 1.3 at night), and the decreasing of TP in two times in the main group compared with the control group, which also reflects the parasympathetic tone reduction (p <0,05). Moderate sympathicotonia in the initial vegetative tonus was found in 76% of premature infants in the daytime and 64% of those at night, and expressed sympathicotonia was in about 20% (p <0,05). According to the data of CIG in dynamics during catamnestic observation the reliable differences in the initial vegetative tone was not observed. Prevalence of the sympathetic link of the VNS was maintained (45% of children had moderate sympathicotonia, and 35% had the expressed sympathicotonia). Conclusions. Sympathetic predominance and parasympathetic activity inhibition of the ANS with a tension of adaptation possibilities during the day is peculiar for the premature infants. The changes we found are most likely related to the immaturity of adaptive mechanisms and mechanisms of autonomic regulation in premature infants, and may be transient. On the other hand, premature birth can cause a damage of the central parasympathetic segment. It is advisable to conduct further research and observation to address the need for and the degree of such states correction.
