Проведення хіміотерапії хворим на гострі і хронічні гемобластози продовжує залишатись складною проблемою, що зумовлено високим ризиком розвитку вторинних уражень з боку органів і систем організму, у тому числі і печінки. У пацієнтів із онкогематологічними захворюваннями гепатотоксичні реакції можуть розвиватись внаслідок інфільтрації пухлинними клітинами тканин печінки, пухлинної інтоксикації, впливу препаратів цитостатичного ряду, вторинних інфекцій. Надмірна вага і ожиріння належать до факторів ризику формування цитостатик-індукованих гепатотоксичних реакцій, що можуть бути спричиненими наявністю у даної категорії пацієнтів неалкогольної жирової хвороби печінки. З цієї точки зору особливого значення має вивчення факторів ризику, основних клінічних характеристик уражень печінки у хворих на гемобластози з урахуванням впливу надмірної ваги і ожиріння з метою покращення своєчасної діагностики цитостатик-індукованих гепатотоксичних реакцій. Потребують розробки алгоритми профілактики уражень печінки на фоні хіміотерапії у хворих на гострі і хронічні гемобластози з надмірною вагою і ожирінням. Мета дослідження. Обґрунтувати диференційований підхід до профілактики уражень печінки на фоні хіміотерапії у хворих на гемобластози з надмірною вагою і ожирінням шляхом комплексного вивчення патогенетичних механізмів їх формування на підставі експериментальних і клінічних досліджень.uk_UA

The defence will be held in Ukrainian Medical Stomatological Academy, Poltava, 2021. Relevance. Conduction of chemotherapy in patients with acute and chronic hemoblastosis is still a difficult issue caused by the high risk of secondary injury of different organs and body systems, including liver. Hepatotoxic reactions in patients with oncohematological diseases can be developed due to liver tissue infiltration with tumour cells, tumour intoxication, an influence of cytostatic drugs, secondary infections. Overweight and obesity refer to risk factors of cytostatic-induced hepatotoxic reactions, that can be provoked by the presence of nonalcoholic fatty liver disease in this category of patients. From this perspective, there is a considerable significance of risk factors and main clinical features of liver injury studying in patients with hemoblastosis taking into account the influence of overweight and obesity to improve timely cytostatics-induced hepatotoxic reactions diagnostics. Algorithms of liver injury prophylaxis in case of chemotherapy in patients with acute and chronic hemoblastosis with overweight and obesity need to be developed. The aim of the research. To substantiate a differentiated approach to liver injury prophylaxis in case of chemotherapy in patients with hemoblastosis and overweight and obesity through a complex study of its pathogenetic mechanisms based on experimental and clinical research. 11 Aims of the research. To develop an experimental nonalcoholic steatohepatitis model in rats. To study the pathogenesis of doxorubicin-induced liver injury in the developed experimental nonalcoholic steatohepatitis model. To study the effectiveness of experimental therapy of doxorubicin-induced liver injury with S-ademethionine. To study the predictors of tumour liver infiltration in patients with hemoblastosis and its influence on the character of biochemical liver tests changes. In comparative aspect to evaluate risk factors, frequency and character of liver injury in patients with hemoblastoses in case of hemoblastosis progression taking into account the role of overweight and obesity. To establish the character of arginine/citrulline cycle and prooxidant-antioxidant balance changes in case of hemoblastosis progression depending on overweight and obesity. To study the influence of program chemotherapy on the frequency and character of liver injury in patients with hemoblastoses depending on body mass index. To assess the efficacy of S-ademetionine as an adjuvant to chemotherapy in patients with acute leukemia with normal weight aimed to prevent liver injury. To substantiate combined prescription of S-ademetionine and ursodeoxycholic acid in patients with acute leukemias and overweight and obesity in the dynamics of chemotherapy aimed to prevent liver injury onset and further progression. To analyse S-ademetionine efficacy as an addition to chemotherapy in patients with B-cell chronic lymphocytic leukemia, B-cell non-Hodgkin malignant lymphoma, and normal weight aimed to prevent liver injury onset and progression. To study the influence of S-ademetionine in combination with ursodeoxycholic acid in patients with B-cell chronic lymphocytic leukemia, B-cell non-Hodgkin malignant lymphoma with overweight and obesity in dynamics of chemotherapy for hepatotoxicity prophylaxis. To investigate the efficacy of L-ornithine-L-aspartate in patients with multiple myeloma and normal weight aimed to prevent liver injury. To substantiate combined prescription of L-ornithine-L-aspartate with ursodeoxycholic acid in patients with multiple myeloma and overweight or obesity to prevent hepatotoxic reactions.uk_UA
