Короткий огляд (реферат): Поряд із ураженням легень, хронічне обструктивне захворювання легень (ХОЗЛ) призводить до значних позалегеневих системних ефектів, в тому числі і в центральній нервовій системі. Метою дослідження було встановити закономірності формування когнітивних, емоційних розладів, порушень сну та змін в неврологічному статусі у хворих з ХОЗЛ І та ІІ ст. В дослідженні включено 31 пацієнт з діагнозом ХОЗЛ І ст. та 27 пацієнтів з діагнозом ХОЗЛ ІІ ст., які не мали в анамнезі (згідно медичної документації) гострої або хронічної церебральної патології. Контрольна група включала 19 умовно здорових осіб. Неврологічні ускладнення при ХОЗЛ мають у своєму розвитку певну еволюцію, маніфестуючи суб’єктивними ознаками з подальшим формуванням ознак органічного ураження головного мозку та розвитком когнітивних та субдепресивних розладів; Наряду с поражением легких, хроническое обструктивное заболевание легких (ХОЗЛ) приводит к значительным внелегочным системным эффектам, в том числе и в центральной нервной системе. Целью исследования было установить закономерности формирования когнитивных, эмоциональных расстройств, нарушений сна и изменений в неврологическом статусе у больных с ХОЗЛ I и II ст. В исследовании включен 31 пациент с диагнозом ХОЗЛ I ст. и 27 пациентов с диагнозом ХОЗЛ II ст., которые не имели в анамнезе (согласно медицинской документации) острой или хронической церебральной патологии. Контрольная группа включала 19 условно здоровых лиц. Неврологические осложнения при ХОЗЛ имеют в своем развитии определенную эволюцию, манифестируя субъективными признаками с последующим формированием симптомов органического поражения головного мозга и развитием когнитивных и субдепрессивных расстройств; Chronic obstructive pulmonary disease (COPD) leads to significant extrapulmonary systemic effects, which are formed in form of multiple complications (including the central nervous system). The aim of our study was to establish regularities of cognitive impairments, emotional disorders, sleep disturbances and changes in neurological status in patients with COPD I and II stages. The study included 31 patients with COPD I stags and 27 patients with COPD II stage, who hadn’t history of acute or chronic cerebral pathology (according to medical records). The control group consisted of 19 conditionally healthy persons. In patients with COPD I stage there was a trend to more frequent occurrence of major neurological complaints compared to the control group patients. However, these complaints were generally not stable and disappeared after the rest. Of all complaints headache was markedly predominated – the relative risk of its occurrence in COPD I stage was 5.31 (95 % CI, 1,36-17,41; p = 0.017) compared with patients of control group. In patients with COPD II stage relative risk of headache was increased in 13.13 times (95 % CI, 2,96-45,44; p = 0. 0002), dizziness – in 6.9 times (95 % CI, 1,28-28,02; p = 0.016), unsteadiness when walking – in 7. 89 times (95 % CI, 1,43-29,96; p = 0. 01), tinnitus – in 6. 9 times (95 % CI, 1,28- 28,02; p = 0. 016), impaired memory – in 12.32 times (95 % CI, 2,84-42,53; p = 0.002), decreased efficiency – in 8. 91 times (95 % CI, 2,13-30,02; p = 0.002), sleep disturbance – in 12.32 times (95 % CI, 2,84-42,53; p = 0.003), emotional instability – in 7.5 times (95 %CI, 1,83-25,10; p = 0. 003) compared with patients of the control group. In addition, in patient with COPD II stage was presented tendency to increasing the rates of all complaints compared to COPD I stage, and relative risk of memory impairment was increased in 5. 34 times (95 % CI, 1,16-16,1; p = 0.007). Focal neurological symptoms in patients with COPD were presented in form of oculomotor disorders, central impairments of facial and hypoglossal nerves, reflexes of oral automatism, pyramidal insufficiency. In patients with COPD I stage the rates of all objective symptoms were similar with those of the control patients, whereas in patients with COPD II stage the rates of all objective signs were dramatically increased. In patients with COPD I stage it had been observed only a tendency to increasing of cognitive impairment rates, whereas in COPD II stage, relative risk of these disorders was increased in 12. 38 times (95 %CI, 1,39-53,72; p = 0.008) compared with patients of the control group. In addition, there was a trend towards greater prevalence of predepresive conditions generally in patients with COPD and, in particular, in patients with COPD II stage. A similar pattern was observed due to daytime sleepiness – relative risk of moderate sleepiness in patients with COPD II stage was increased in 6. 67 times (95 %CI, 1,52-23,76; p = 0.01) compared with patients of the control group. Conclusions. Neurological complications of COPD have in their development certain evolution, in form of primary subjective symptoms, followed by the formation of signs of organic brain impairments, cognitive impairments and predepresive disorders.
