Негативний та позитивний «комплаєнс» у хворих із запальними захворюваннями щелепно-лицевої локалізації

Abstract

У статті наведені основі причини формування негативного та позитивного лікувального комплаєнсу в пацієнтів із гнійно-запальними процесами щелепно-лицевої локалізації. Формування у пацієнтів негативного комплаєнсу призводить почастішання і збільшення тяжкості перебігу запальних захворювань, що в свою чергу сприяє значному зростанню тимчасової непрацездатності, а в ряді випадків інвалідизації аналізованої категорії хворих. Отже, дана проблема має не тільки медичне, але й важливе соціальне значення. Встановлено, що формування позитивного комплаєнсу сприяє покращенню ефективності лікування пацієнтів із гнійно-запальними ураженнями щелепно-лицевої ділянки, що супроводжується меншим терміном перебування пацієнтів на стаціонарному лікуванні, зменшенню частоти ускладнень та рецидивів захворювань; В статье приведены основные причины формирования отрицательного и положительного лечебного комплаенста у пациентов с гнойно-воспалительными процессами челюстно-лицевой локализации. Формирование у пациентов отрицательного комплаенса приводит к учащению и увеличению тяжести течения воспалительных заболеваний, что в свою очередь способствует значительному увеличению временной нетрудоспособности, а в некоторых случаях инвалидизации анализированной категории пациентов. Таким образом, данная проблема имеет не только медицинское, но и важное социальное значение. Установлено, что формирование положительного комплаенса способствует улучшению эффективности лечения пациентов с гнойно-воспалительными поражениями челюстно-лицевой области, что сопровождается меньшим термином нахождения пациентов на стационарном лечении, уменьшению частоты осложнений и рецидивов заболеваний; Inflammatory processes of maxillofacial area are one of the urgent problems of modern surgical stomatology. In recent years, the number of these patients is increase, often observed severe, progressive disease, which is complicated by the proliferation of inflammatory process in the adjacent areas topography-anatomical, acute respiratory failure, mediastenit, sepsis.Increased and increasing of frequency of severity of inflammatory diseases lead to a significant increase of temporary disability, and in some cases to invalidization of analyzed patients. Thus, the problem has not only medical but also social importance. There is the notion “compliance” in the medical literature - is a voluntary adherence of patient to treatment, appointed him by the doctor. Compliance means the accuracy of the drugs and medical procedures follows to doctor’s recommendations for the implementation of medical appointments, limitations of bad habits. Based on an analysis of the data of cases histories of patients of maxillofacial department of Poltava Regional Clinical Hospital and survey data identified the main causes of unsatisfactory results of medical and surgical treatment. Depending on the patient implementation of these measures produce negative and positive therapeutic compliance. Negative therapeutic compliance results to more severe course, relapse of disease, secondary hospitalization, invalidization, and in some cases to fatal result. The main reasons for the formation of negative compliance in patients with purulent-inflammatory diseases of maxillofacial localization are indicated. The mentality of the patient and uncritical attitude to their health. Patients often quite a long time engaged by self-treatment. It prolongs the period of treatment and rehabilitation of such patients. The low general cultural level of the population, patients do not inform the doctor about their health, believing that general somatic disease not related to local inflammatory processes. Accessibility of medicines in pharmacies, the widespread introduction of advertising leads to self-selection of drugs, and terms of treatment without consulting with doctor. Such medical tactic often leads to the formation of resistant strains, even more difficult following treatment. Insufficient knowledge of doctors of diagnostic features and treatment of inflammatory diseases of maxillo-facial area often leads to wrong differential diagnosis as a result - to an inadequate treatment. Uncontrolled purpose of thermal procedure for patients with inflammatory diseases of maxillofacial localization leads to the proliferation of inflammatory processes. To form a positive compliance should consider the following factors. It necessary to explain to the patient the treatment regimen intended use, choice of drugs with a small reception frequency rate, for patients who are hospitalized, better to prescribe injectable forms of drugs. It is necessary to take into account the value of “quality-price” . Compliance with the principles of medical ethics and deontology leads to the establishment of close links “doctor-patient” , which positively affects the outcome of treatment. Following such rules helps to improve the effectiveness of treatment, reduce the frequency of relapses diseases and its complications.
