Аналіз роботи кардіологічного відділення Полтавського обласного клінічного госпіталю для інвалідів війни

Abstract

Нами проаналізована робота кардіологічного відділення Полтавського обласного клінічного госпіталю для інвалідів війни, де провідне місце в їх структурі займає кардіологічна патологія (99,5%),показано, що захворюваність пацієнтів, як людей похилого і старечого віку, характеризується вираженою поліморбідністю з частими загостреннями, недотримання хворими рекомендацій на амбулаторному рівні призводить до зниження якості лікування, ускладнює і подовжує строки лікування хворих в стаціонарі; Нами проанализирована работа кардиологического отделения Полтавского областного клинического госпиталя для инвалидов войны, где ведущее место в их структуре занимает кардиологическая патология (99,5%),показано, что заболеваемость пациентов, как людей пожилого и старческого возраста, характеризуется выраженной полиморбидностью с частыми обострениями, несоблюдение больными рекомендаций на амбулаторном уровне приводит к снижению качества лечения, усложняет и удлиняет сроки лечения больных в стационаре; The growth of the elderly in developed countries determines the need for the development and implementation of comprehensive measures of medical and social care for people in older age groups, as indicated in the recommendations of the UN General Assembly to develop strategies for the development of geriatric care. The purpose of the study was to analyze the cardiology department of the Poltava Regional Clinical Hospital for disabled veterans. Object and methods. For the purpose of the study, the following methods: social hygiene (prevalence, morbidity), sociological (questionnaires, interviews) statistics. We used the following statistical form Poltava Regional Clinical Hospital for Disabled War: card-patient (Form 003/0) retired from hospital card (form 066 / o), outpatient medical record (Form 025 / o), report data regarding the separation for the 2009-2012 year. Results and discussion. Because of the specificity of the contingent of patients and serviced hospital cardiology department is not a purely cardiologic, along with cardiologic patients are on treatment of patients with disorders of the respiratory, digestive, endocrine system, there are serious consequences associated injuries of war. However, the specialization branch remained stable and amounted in 2012 - 99.5%, in-99 of 2011, 4 in 2010 - 99.2% in 2009. - 98.7%. However, given the specificity of patients, the severity of existing disease-related traumas of war and concomitant severe pathologies age, leads to the fact that the average duration of treatment of our patients in a number of pathological processes significantly higher than the regional average performance in terms of treatment of patients and normal actively influence is extremely difficult, although this figure depends on the quality of examination and treatment of patients in the prehospital phase. The average length of stay of the patient on the bed is high and amounts to 20.3 in 2012, which is slightly higher than normal (19.0), which to some extent can be explained by the severity of hospitalized patients. Owing to absence of the lack of day care in the hospital, bed / day remains the higher (20.3). Unfortunately there are still cases of hospitalization of patients in serious condition because of heavy diseases. This complicates and lengthens the time of treatment in hospital.The Job of the bed is stable for 3 years (345.5. 344.9, 342.3, 342.5 346.6), which is above the average for Ukraine in the hospital, which is 335 days. Bed turnover in 2012, 17,0 , in 2011 - 16 5. Remains high percentage of hospitalized urban residents, especially residents of Poltava (57.9%). It is too bad, because they have the opportunity to receive professional assistance in hospitals in the city. Significantly increased proportion of patients with a heavy failure of blood circulation. Average duration of treatment in this category is often above average in Ukraine among hospitals because of heavy diseases. Conclusions. The incidence of patients are elderly and senile age, characterized by severe combinations of different diseases with frequent exacerbations, default by patients of recommendations at the outpatient level leads to lower quality of care, complicates and lengthens the time of treatment in hospital. Prospects for further examination: improving service levels for outpatient, high-quality clinical supervision.
