Короткий огляд (реферат): Розлади сну негативно впливають на процеси постінсультної реабілітації. Неспецифічним симп- томом різноманітних порушень сну є підвищена денна сонливість (ПДС). Мета: вивчити розповсюдженість ПДС в гострому періоді інсультів, а також фактори, асоційовані з ПДС. Об’єкт і методи дослідження. Нами об- стежено 173 пацієнти в гострому періоді ішемічних та геморагічних інсультів. Рівень денної сонливості визначався за допомогою шкали сонливості Epworth (значення ≥10 відповідали ПДС). Висновки. На момент виписки зі стаціонару частота ПДС у пацієнтів з інсультами становить 25,4%. Ризик феномену ПДС має прямі достовірні асоціації з наявністю у пацієнтів цукрового діабету (відношення шансів (ВШ) 2,28; 95% довірчий інтервал (ДІ), 1,10-4,72; р=0,03), абдомінального ожиріння (ВШ 2,45; 95% ДІ, 1,21-4,98; р=0,01) та розвитком деліріозних порушень під час перебування в стаціонарі (ВШ 2,78; 95% ДІ, 1,29-5,98; р=0,01); Расстройства сна негативно влияют на процессы постинсультной реабилитации. Цель: изучить распространенность ПДС в остром периоде инсультов, а также факторы, ассоциированные с ПДС. Объект и методы исследования. Нами обследовано 173 пациента в остром периоде ишемических и гемор- рагических инсультов. Уровень дневной сонливости определялся с помощью шкалы дневной сонливости Epworth (значения ≥10 отвечали ПДС). Выводы. Частота возникновения ПДС у пациентов с инсультами на момент выписки из стационара составляет 25,4%. Риск феномена ПДС имеет прямые достоверные ассоци- ации с наличием у пациентов сахарного диабета (отношение шансов (ОШ) 2,28; 95% доверительный интер- вал (ДИ), 1,10-4,72; р=0,03), абдоминального ожирения (ОШ 2,45; 95% ДИ, 1,21-4,98; р=0,01) и развитием делириозных нарушений во время пребывания в стационаре (ОШ 2,78; 95% ДИ, 1,29-5,98; р=0,01); Sleep has been shown to enhance learning motor skills and memory consolidation in individuals with chronic stroke. Sleep disturbance occurs, or worsens, after a stroke and occurs in various forms such as sleep apnea, insomnia, sleep-related movements disorders so on. Sleep disturbance has been known to lead to short-term and long-term negative patient outcomes and disturb the life of rehabilitation stroke patients. Excessive daytime sleepiness (EDS) is unspecific sign of different sleep disturbances. Aim: to study post-stroke EDS rate and to clarify the possible interconnections between the EDS and patients’ characteristics at hospital discharges. Methods. We enrolled in the study 146 patients with ischemic stroke and 27 patients with hemorrhagic strokes. Daytime sleepiness was measured using Epworth sleepiness scale (scores ≥10 indicate excessive daytime sleepiness). It was investigated patients’ age, gender, marital status, formal education level, pre-stroke employment status, tobacco smoking status, level of alcohol consumption. For anthropometric characteristics were used waist circumference. The co-morbidities included arterial hypertension, ischemic heart disease, atrial fibrillation and diabetes mellitus. Acute stroke complications during hospital stay included pneumonia, urinary tract infections and pyrexia. The severity of neurological deficit was evaluated with National Institute of Health Stroke Scale at day of admission and at day of hospital discharge. The patients’ functional disability was determined by using modified Rankin scale at day of hospital discharge. Neuropsychological testing had been performed at time of hospital discharge: signs of anxiety were assessed by Hospital Anxiety and Depression Scale, apathy symptoms were assessed by the Starkstein apathy scale, cognitive impairments were evaluated by the Montreal cognitive assessment. Continuous variables were represented as mean±standard deviation and categorical data were represented by number and percentage. Univariate logistic regression analysis were performed to analyze the odds ratio (OR) with 95% confidence intervals (CI) of significant factors associated with post-stroke EDS. P-value less 0.05 was taken to indicate statistical significance. Results. At time of hospital discharge the rate of EDS was 24,5%. The majority of investigated factors didn’t have any significant relations with risk of post-stroke EDS. However, diabetes mellitus, abdominal obesity and delirium were directly associated with risk of post-stroke EDS. Univariate logistic regression analysis showed that patients with post-stroke EDS at time of hospital discharge had more common diabetes mellitus (OR, 2,28; 95% CI, 1,10-4,72; p=0,03), as well as more common abdominal obesity (OR 2,45; 95% CI, 1,21-4,98; p=0,01) and delirium during hospital stay (OR 2,78; 95% CI, 1,29-5,98; p=0,01). Increased risk of post-stroke EDS in patients with diabetes mellitus and abdominal obesity could be explained perhaps by the fact that diabetes mellitus as well as abdominal obesity are frequently followed with increased rate of breathing sleep disorders. According to literature data sleep disturbance and delirium quite often coexist and could aggravate each other.
