Документ Effects of l-arginine and l-ornithine supplementations on the treatment of chronic periodontitis: A preliminary randomized short-term clinical trial(Elsevier Ltd., 2021-11-09) Shynkevych, Viktoriia; Kolomiiets, Svitlana; Kaidashev, Igor; Шинкевич, Вікторія Ігорівна; Коломієць, Світлана Веніамінівна; Кайдашев, Ігор ПетровичAbstract The growing interest in the possibilities of modulating macrophages in inflammatory diseases with therapeutic purpose has prompted the development of new approaches for the treatment of periodontitis. This randomized add-on open preliminary clinical study evaluated the short-term effects of L-arginine or L-ornithine as an adjuvant to scaling and root planing (SRP) in patients with chronic periodontitis. Materials and methods Seventy-five periodontitis patients were recruited and monitored clinically and immunologically at baseline (before SRP) and 30 ± 5 days after SRP. All patients were assigned by stratified randomization to SRP (SRP only, n = 25), Arg (SRP + L-arginine, n = 25) or Control (SRP + L-ornithine, n = 25) Group. The medicines were used according to available instructions for 10 and 15 days, respectively. During the study, all patients were on a stable diet, without changing their rations and regiments. As immunological monitoring immunohistochemical study of CD68+ and CD163 + single positive gingival macrophages for 5 patients per group in the same time-point was conducted. The data were statistically analyzed. Results Reduction of periodontal pocket depth (PPD) and bleeding on probing (BoP) was observed in all groups, with significant between-group differences for BoP in the Arg Group (p < 0.0001) at 30 days. The SRP and Arg groups demonstrated nonsignificantly increased density of CD68+ and CD163 + cells. The Orn Group showed an increase in the density of CD68+ and CD163 + macrophages at intragroup (p = 0.0066 and p < 0.0001) and between-group levels (p = 0.001 and p < 0.0001), and these changes corresponded to clinical PPD and BoP reduction. In the Arg and Orn groups at 30 days, CD163 + macrophages significantly predominated over CD68+ (p = 0.013, p < 0.0001). Conclusion The use of L-arginine and L-ornithine as an adjunct to SRP promotes additional limited immunological benefit in the treatment of periodontitis. Infiltration of the composite resin increases the strength of demineralized enamel. The results of clinical application indicate the effectiveness of the Icon infiltration method in combination with microabrasion and professional whitening, as well as an excellent aesthetic effect for 2 years after treatment. The Icon method may be a minimally invasive alternative to various methods of remineralization, especially in pediatric practice. Certain limitations in the application of the Icon method include its relatively high cost.This publication presents favorable treatment outcomes, obtained within a short time, in the therapy of fluorosis of permanent incisors in a 7-year-old girl after the application of the Icon infiltration method. This clinical case may be of practical interest to dentists in the wider use of this technique in pediatric dentistry, especially in the regions of endemic fluorosis. Before disseminating the results of the treatment, we obtained written informed consent from the patient's mother. We used the Icon infiltration method and obtained fast and reliable results, which was a key aspect to choose the treatment approaches in this clinical case. There were no restrictions as to the application of this approach in this clinical case. In the scientific literature, there is an unconfirmed theory that the prevalence of fluorosis and its severity increases over time after tooth eruption. However, recent studies have shown that very mild and moderate fluorosis tends to decrease in severity in adolescence. This trend continues into young adulthood and then slows down at the age of twenty. Given this information, the clinical effect reported by the manufacturers after treatment with Icon might be even longer than the declared period of 2 years after treatment for fluorosis.Документ Long Term Oral Symptoms Systematization in Patients who Underwent COVID-19: Case Series Research(2022-07-17) Skrypnikova, Taisa; Skrypnykov, Petro; Shynkevych, Viktoriia; Скрипнікова, Таїса Петрівна; Скрипников, Петро Миколайович; Шинкевич, Вікторія ІгорівнаA number of common long-term morbidities have been reported following COVID-19, and question arises about oral symptoms. In this case series we included 29 adult patients with oral mucosa symptoms ≥2 months after COVID-19 with the aim of highlighting potential interrelations between demographic, oral clinical symptoms and periods elapsed after COVID-19 and its severity. Analysis of medical histories and demographic data showed the correlation between age and severity of previous COVID-19 (r=.5, P=.01), and the presence at least two chronic diseases such were gastrointestinal diseases, arterial hypertension, diabetes, etc. in all participants. Using clinical criteria of diagnosis, we found oral symptoms combined at least two: dry lips and edematose oral mucosa in all participants. The most common diagnosis was chronic candidal glossitis (Сhi-square test, P=.01), presented in different forms and expansions, and correlated with COVID severity (P=.001, r=.6). The second most common symptom was halitosis which combined mainly with candidal glossitis and negatively correlated with COVID severity (P=.001, r=-.6). And the rarest finding was petechial rashes on the hard and soft palate, which also correlated with COVID severity (P=.004, r=.5). After suffering COVID-19 on the background of chronic systemic diseases, it is expected long term incidence of oral mucosa morbidity during 8 months, with a significant prevalence of chronic candidal glossitis. All detected oral symptoms can be explained by concomitant pathology, in contrast to post-COVID syndrome.
