    Topicalaspects of combined combat thermomechanical injuries

    При важких механічних ушкодженнях і великих та глибоких опіках розвивається опіковий і травматичний шок, який протікають важче, ніж при таких ізольованих механічних ушкодженнях та опіках. Формування ключового у патогенезі шоку синдрому гемоциркуляторних порушень відбувається внаслідок крововтрати та плазмовтрати, в тому числі з опікових ран. На шляхах медичної евакуації поранених з тяжкими та вкрай тяжкими комбінованими травмами спостерігається тенденція до прискорення транспортування поранених на наступний етап, як можна раніше, іноді не враховуючи тяжкість, що призводить до погіршення стану поранених. Проведено порівняльне комплексне клінічне обстеження 183 постраждалого, які були поділенні на 3 групи. Група 1 становила 71 поранених з бойовими травмами кінцівок, група 2 становила 61 постраждалих з не тяжкою, група 3 становила 57 постраждалих з тяжкою та вкрай тяжкою. Severe mechanical injuries and large and deep burns lead to burn and wound shock, which is more severe than in isolated mechanical wounds and burns. The key syndrome in the pathogenesis of shock named hemocirculation disorders syndrome is formed as a result of blood and plasma loss, including losses caused by burn wounds. In medical evacuation of the wounded persons with severe and extremely severe combined injuries, there is a tendency to accelerate the transportation of the wounded persons to the next stage as fast as possible, and sometimes the severity of injuries is not taken into account, which leads to deterioration in the condition of the wounded persons. There was a comparative integrated clinical examination of 183 injured persons, who were divided into 3 groups. Group 1 consisted of 71 wounded persons with combat extremity injuries, Group 2 consisted of 61 persons with minor injuries, and Group 3 consisted of 57 persons with severe and extremely severe injuries.

    Improvement of the algorithm for intraoperative diagnostics of breast cancer metastasis using sentinel lymph node staining

    Лімфогенне метастазування є однією з найважливіших причин виникнення місцевого рецидиву раку молочної залози та несприятливим фактором прогнозу. З початком застосуванням в хірургічній практиці методики фарбування сторожових лімфовузлів з використанням сучасних барвників (Patent Blue, ICG), кардинально змінися підхід до визначення обсягу оперативного втручання. Виконання фарбування сторожових лімфовузлів при пухлинах молочної залози дозволяє максимально зменшити травматичність хірургічного втручання (за рахунок відмови від аксилярної лімфодиссекції), внаслідок чого мінімізується кількість післяопераційних ускладнень. Метою даної роботи було удосконалення алгоритму інтраопераційного виявлення «сторожових» лімфовузлів у хворих на рак молочної залози. У період з 2016 по 2021 р. на базі Одеської обласної клінічної лікарні було прооперовано 200 пацієнток з раком молочної залози Т1-Т3N0М0, з використанням двох барвників: Patent Blue та ICG. Запропанований алгоритм діагностики та лікування раку молочної залози дозволяє в переважній більшості випадків відмовитися від травматичних операцій на користь органозберігаючих операцій з біопсією «сторожових» лімфовузлів.

    Medical and environmental aspects evolution of diseases of the endocrine system in children of Ukraine

    We have found that in 2017 9.569 cases of endocrine system diseases were detected in children affected by the Chornobyl accident (or 24,67 per 1.000 children), which is by 1.7 times higher than the mean incidence of this pathology in children in Ukraine. Мethods of statistical evaluation and epidemiological analysis of state medical statistics from 1993 to 2020 were used.Throughout 25 years of observation we have discovered that from the period of 1993 to 2017 was a decrease in the incidence and prevalence of the endocrine system diseases in children by 36.9 %, and, more significantly, by 65.5 % – in children who suffered as a result of the Chernobyl accident. At the same time, disability increased by 5.3 times. The prevalence and incidence of endocrine diseases in children affected by the Chernobyl accident are significantly higher than the national mean, especially in the detection of thyroid disease.

    Morphological changes of corticosterocytes and chromaffin cells after 1 month of chemical castration under conditions of correction with quercetin

    It is well known that endocrine pathology is a leading disease in the morbidity structure of the Ukrainian population. It is important to study the metabolic effects of hormones of the structural components of the hypothalamic-pituitary-gonadal axis on other peripheral endocrine glands and body organs. Modern research on testosterone proves its effect on the biochemical processes of nervous tissue, liver, organs of the male and female reproductive systems, and others. The paper describes the main morphological changes of corticosterocytes in the adrenal cortex area and endocrinocytes of the adrenal medulla of white rats during the correction of central deprivation of testosterone synthesis during 1 month of the experiment. It was found that the inhibition of testosterone synthesis caused reactive changes in both - corticosterocytes and chromaffin cells, manifested at the microscopic level by the variability of morphometric parameters.

    Correlation analysis of circadian rhythm with severity of psoriasis and obesity

    The recent increase in cases of comorbidity of psoriasis and obesity leads to severe, atypical, disabling, and therapyresistant forms of dermatosis, significantly worsen the quality of life of patients, reduces work capacity and social activity of patients, which determines not only the medical but also the social significance of the problem. There is a theoretical rationale for the possible role of circadian rhythm disturbances, manifested by a change in eating behavior, a decrease in sleep duration, and the individual chronotype of patients in developing obesity and some autoimmune diseases. However, this issue remains insufficiently studied and proven. The purpose of our study was to identify the correlation between the circadian rhythm and indicators of changes in the chronotype of a person's working capacity and indicators of the severity of the course of psoriatic disease, the degree of obesity, and the degree of the negative impact of the disease on the quality of life of patients. The results of the correlation analysis showed a very high negative correlation between the chronotype of patients' working capacity and BMI indicators and a high negative correlation between the indicators of the PASI index, ACTIVITY, and the chronotype of patients' working capacity.

    Research on the relationship between the severity of the course of psoriasis and metabolic syndrome and the level of indicators of systemic inflammation

    In patients with moderate psoriasis with concomitant metabolic syndrome, the indicators of general clinical and biochemical blood tests and carbohydrate and lipid metabolism were determined, and indicators of systemic inflammation were assessed by the level of susceptible C-reactive protein, ceruloplasmin, interleukin-6 and tumor necrosis factor-α. A laboratory study found that in patients with moderate psoriasis with concomitant metabolic syndrome, an important role is played by the systemic inflammatory process, which is accompanied by the development of insulin resistance, impaired lipid and nitrogen metabolism, and regulation of vascular tone. Furthermore, an increase in the intensity of systemic inflammation is accompanied by an increase in skin lesions, an increase in systolic blood pressure, and a violation of lipid and carbohydrate metabolism.

    Electro-photonic emission analysis in patients with noncommunicable diseases: essential hypertension

    78 polymorbid patients with essential hypertension and 85 functionally healthy respondents were obtained by the method of electro-photonic emission analysis. Patients with essential hypertension had significant differences in all physical and mathematical parameters of electro-photonic emission analysis compared to functionally healthy young respondents, which testifies to a different course of metabolic processes in them. The electro-photonic emission analysis method is a modern, hightech,accessible, valid instrumental method for assessing ultra-weak photon emission and indirect research of mitochondrial function, which can provide an opportunity to objectively assess the course of energy processes at the tissue level of metabolism, determine the level of stress, the parameters of the balance of the body's functioning. The electro-photonic emission analysis method can be recommended as promising for use in scientific research and in clinical practice during the objective examination of patients.

    Characteristics of disorders of the quality of life of young patients depending on the variant of the course of gastroesophageal reflux disease

    The article represents the results of our research on determining the influence of esophageal symptoms of erosive and non-erosive forms of gastroesophageal reflux disease on indicators of the physical and mental components of quality of life according to the Medical Outcomes Study Questionnaire Short-Form 36. The presence and degree of severity of symptoms of gastroesophageal reflux disease were analyzed using the Gastroesophageal Reflux scale Disease Questionnaire. We examined 18 patients with an erosive form and 18 patients with a non-erosive form of gastroesophageal reflux disease. It is shown that against the background of the erosive form of gastroesophageal reflux disease, daily manifestations of the symptoms of the disease are more often observed, namely heartburn (RR=6.00 (0.80-44.95); p>0.05), regurgitation and pain in the epigastric region of the abdomen (RR=2.50 (0.56-11.25); p>0.05), which leads to violations of mainly physical components of quality of life. At the same time, the esophageal symptoms of the non-erosive form of gastroesophageal reflux disease change the quality of life of patients in categories that characterize mental health.

    Morphofunctional characteristics of the structural components of the pancreas exocrine part in chemical castration

    Throughout a person's life, the pancreas, which is the second largest gland (after the liver), is significantly affected by various adverse factors. It is a unique organ that combines external and internal secretory activity, playing an important role in the carbohydrate metabolism of the human body. Today, diseases of the pancreas are one of the most important problems in medicine. Established dystrophic changes of the gland, which were expressed by the formation of fibrous fields, the thickness of the capsule and interlobular connective tissue between the lobes visually increased, a decrease in the area of acini and exocrinocytes was observed, acini lost clear contours and shape due to swelling of the connective tissue. Exocrinocytes of acini lost apical eosinophilia and basal basophilia of the cytoplasm, there was a decrease in the area of the nuclei, which were changed in shape and location. The introduction of triptorelin causes destructive-degenerative changes in the exocrine apparatus of the pancreas, which are characterized by compensatory mechanisms in the early periods of observation, and in the later periods – dystrophic changes in the structural components of the gland are determined, which corresponds to the stage of partial decompensation, a decrease in the activity and functionality of the cellular component of the exocrine apparatus of the pancreas.

    Quantitative and qualitative changes in red bone marrow monocyte dipherone and microenvironmental cells during long-term triptorelin acetate administration in the experiment

    States. Huggins and Hodges demonstrated the therapeutic effect of testosterone deficiency in the gonads in the 1940s and thus confirmed the concept that prostate cancer is an androgen-dependent disease. Androgen deprivation therapy (ADT) is the main palliative treatment for men with locally advanced and metastatic prostate cancer, with the goal of reducing testosterone levels to the level obtained by surgical castration. The long-term chemical castration with triptorelin acetate leads to quantitative and qualitative changes in monocytic sprout cells at all levels of differentiation and proliferation with a complementary reaction of microenvironmental cells. The maximum quantitative changes in monocyte cells were observed at the 3rd month of the study, followed by a gradual recovery to the control group. Changes in the microenvironment cells: macrophages and reticular cells, were characterized by a stable reaction in the early stages of the study in the form of a decrease in the number and ratio of NSCs, with subsequent recovery of quantitative and qualitative changes to the level of the control group of animals. The adipocyte reacts at all stages of the study with a steady increase in quantitative and qualitative characteristics.

    Metrical indices of the cecum wall in areas of gut-associated lymphoid tissuelocalisation under the in influence of food additive complex

    There are insufficient data in the current literature on the effect of food additives on the occurrence of pathological processes in the intestine. A total of 84 sexually mature male non-linear rats were used in the experiment. Rats of the control group consumed drinking water and received oral physiological solution. Animals in the experimental group received food additives at the following doses: sodium nitrite (E250) 0.6 mg/kg, sodium glutamate (E621) 20 mg/kg, ponceau 4R (E124) 5 mg/kg. Sampling for histological examination was carried out at 1, 4, 8, 12 and 16 weeks. A significant reduction in the thickness of the cecum wall of the animals in the areas of localisation of intestinal-associated lymphoid tissue was found at the early stages of observation. A marked lymphoid tissue reaction in the form of intense cellular infiltration of the submucosa and mucosa was recorded. Disturbance of crypt structure and desquamation of epithelium occurred. At the late terms of observation there was a significant increase of the cecum wall thickness in the areas of localization of the lymphoid tissue associated with the intestine due to edema of submucosa,partial restoration of mucosa structures.

    Restructuring of the rat pulmonary vascular bed induced by the conplex of food additives

    The paper presents the results of a morphometric study of the vessels’ lumen diameter of the large circulatory circle, as well as the vessels of the small circulatory circle of the rats’ lungs of under the complex effect of food additives. It was established that the use of a complex of food additives leads to a change in morphometric indicators, and in the early stages it was accompanied by spastic reactions of small circulatory circle pulmonary vessels and vessels of the microcirculatory of the large blood circulation, which was confirmed by a decrease in morphometric indicators of the lumen diameter. During the following periods of study, the vessels of the hemomicrocirculatory channel of the large circulatory circle responded by increasing the average values of morphometric indicators, and of all its links, as a result of the long-term action, first of all, of sodium nitrite on the smooth myocytes of the vessels of the resistive link and the action of compensatory and restorative mechanisms in response to an increase in pulmonary pressure vessels of the small blood circulation to the vessels of the exchange and capacitive links of the large blood circulation. At the end of the experiment, the morphometric indicators did not fully recover, the vessels of the large circulatory circle remained in a decompensated state, which was expressed by an increase in the average values of the morphometric indicators of the diameter of the lumen of the vessels.
