Сімейна гетерозиготна гіперхолестеринемія: клінічний випадок

Abstract

Резюме. У статті наведено клінічний випадок сімейної гетерозиготної гіперхолестеринемії з розвитком у пацієнтки гострого інфаркту міокарда з подальшим проведенням аортокоронарного шунтування. Особливістю цього випадку є також те, що мати пацієнтки у молодому віці померла від інфаркту міокарда, а сестра у 15-річному віці – від серцевої патології. У двох дочок пацієнтки виявлено сімейну гіперхолестеринемію, з приводу якої призначено гіполіпідемічну терапію. Проведено аналіз літератури щодо діагностики і лікування сімейної гетерозиготної гіперхолестеринемії.Summary. The article describes clinical case of familial heterozygous hypercholesterolemia that ruled to the onset of acute myocardial infarction followed by coronary artery bypass surgery. The present signs of familial heterozygous hypercholesterolemia (such as elevated concentration of cholesterol in blood, xantomes, tendinea xantomes, tuberous xantomes of skin) were not proper evaluated by primary care physicians but diagnosis was established only after the onset of acute myocardial infarction and estimated atherosclerotic stenosis of coronary arteries. The peculiarity of this case is the fact that mother of this patient have died at a young age because of myocardial infarction. Her sister died at age of 15 by heart disease. In two daughters familial hypercholesterolemia was revealed, which managed by lipid-lowering therapy. The analysis of the literature dedicated to the diagnosis and management of family heterozygous hypercholesterolemia was performed.
