Самооцінка системи управління медичною профілактикою у багатопрофільному закладі охорони здоров’я за функціональною методикою

Abstract

Принцип участі медичного персоналу в забезпеченні якості медичного обслуговування пацієнтів є одним із важливіших принципів концепції «Загального управління якістю». Вивчення ставлення лікарів до різноманітних аспектів медичного обслуговування, дозволяє оцінити рівень задоволеності своєю діяльністю, мотивацію надавати якісні медичні послуги, встановити потребу підвищення рівня поінформованості лікарів про усі спектри діяльності закладу охорони здоров’я, у якому вони працюють. У статті наведено основні результати проведеного автором дослідження: встановлено наявність розбіжностей у оцінці системи медичної профілактики лікарями первинної ланки надання медичної допомоги, зокрема, лікарями-терапевтами дільничними та сімейними лікарями становила 75,8±0,07 (М±m) балів і була значно вищою аніж оцінка лікарів котрі надають вторинну медичну допомогу (лікарів-хірургів, лікарів-ендокринологів, лікарів-дерматологів тощо) – 68,1±0,11 (М±m) балів.

Изучение отношения врачей к различным аспектам медицинского обслуживания, позволяет оценить уровень удовлетворенности своей деятельностью, мотивацию предоставлять качественные медицинские учреждения здравоохранения, в котором они работают. В статье приведены основные результаты проведенного автором исследования: установлено наличие разногласий в оценке системы медицинской профилактики врачами первичного звена оказания медицинской помощи, в частности, врачами-терапевтами участковыми и семейными врачами составляла 75,8 ± 0,07 (М ± m) баллов и была значительно выше чем оценка врачей предоставляющих вторичную медицинскую помощь (врачей-хирургов, врачей-эндокринологов, врачей-дерматологов и т.п.) – 68,1 ± 0,11 (М ± m) баллов.

The principle of participation of medical personnel in ensuring the quality of medical care for patients is one of the most important principles of the concept of “General Quality Management”. The study of the attitude of doctors to various aspects of medical care can assess the level of satisfaction with their activities, motivation to provide quality medical services, and establish the need to raise the level of awareness of physicians about all the spectrum of activities of the health care institution in which they operate. Purpose of the study. To study the attitude of medical personnel (doctors) of a multi-disciplinary health care institution certified in accordance with the standards of DSTU ISO 9001: 2009, to the system of management of medical prophylaxis, from the position of persons directly involved in this process. Object and methods of research. The self-assessment of the management system for medical prophylaxis was conducted in a multi-occupational health care facility – the clinic of the State Institution of Science “Research and Practical Centre of Preventive and Clinical Medicine” State Administration Department. For the self-assessment of the system for managing the prevention of a medical institution, we have developed an authoritative questionnaire “The protocol of evaluation of the management system for medical prophylaxis of the State Institution of Science “Research and Practical Centre of Preventive and Clinical Medicine” State Administration Department”. The basis for the development of this tool is the functional method (management assessment model) of the Management Functional Assessment Model, Maslov D., Watson P., Chilishi N. It has already been tested for the assessment of the management system of medical institutions, in particular for assessing the overall management system of the health care institution. Results of the research and their discussion. The survey involves doctors who are directly involved in the provision of prophylactic medical services at different levels of their provision. 24 doctors of the Center of primary health care and 23 doctors of the clinic consultative and diagnostic center became the respondents, 44 of them had a higher doctoral category, the first – 2, and the second – 1 physician. The overall assessment of the management system for medical prophylaxis in 2018 – 73.0 ± 0.07 (M ± m) points and corresponded to level VI of the management of the institution. Conclusions. In the structure of maturity levels of management of a functional model, this degree corresponds to the following characteristics: “Continuous improvement of management quality is carried out in more areas, it is necessary to maintain the dynamics of improvements and start transforming the remaining problem areas using innovative improvement strategies”.
