Локалізація поломок базисів повних знімних протезів верхньої щелепи і кількість лагоджень

Abstract

Наведені результати дослідження щодо локалізації поломок повних знімних пластинкових протезів верхньої щелепи та їх кількість залежно від методу полімеризації базисної пластмаси. Установлено, що майже у 2 рази ламалися частіше протези, які полімеризували на «водяній бані» без застосування армуючого шару, ніж ті протези, в яких застосовували армуючий шар з поліамідної та металевої сітки; така ж тенденція характерна при полімеризації протезів у сухому середовищі під тиском. Приведены результаты исследования локализации поломок полных съемных пластиночных протезов верхней челюсти и их количество в зависимости от метода полимеризации базисной пластмассы. Установлено, что почти в 2 раза ломались чаще протезы, которые полимеризовали на «водя ной бане» без применения армирующего слоя, чем те протезы, в которых применяли армирующий слой из полиамида и металлической сетки; такая же тенденция характерна при полимеризации протезов в сухой среде под давлением. The results of studies on the localization of breakdowns of maxillary full removable laminar dentures and their number depending on the method of polymerization of basic plastics are presented in this article. For the clinical investigation, we were observing 2 groups of patients with a complete lack of teeth in the upper jaw, aged 45-65 years and older; each group was divided into 3 subgroups depending on the method of polymerization of plastics and reinforcing material. Analyzing the areas of localization and frequency of breakdowns in unreinforced and reinforced bases of full removable dentures, in which the polymerization of basic polymer was carried out by two methods, we have found that the use of reinforcing elements affects the number and frequency of breakdowns, as well as the nature of failures and the place of their localization. According to the authors, the mechanism of breakdowns was like that: at the presence of side groups of teeth on the lower jaw during the life in the functioning of dental system, the buccal hillocks of premolars and molars are erased due to the transversal movements of the mandible while fitting to the maxillary full denture. Trying to create maximum occlusal contact, an orthopedic physician grinds in palatal hillocks on the maxillary prosthesis thereby creates a "wedge". When using a full removable dentures, the forces, the effect of which on the basis of the prosthesis can be described as centrifugal forces acting in parallel, appear. It was established that the dentures, which were polymerized with the "water bath" without the use of a reinforcing layer, were getting broken almost 2 times more often than dentures, for which a rein forcing layer of nylon and metal mesh was used. The same trend is typical during the polymerization of prosthesis in a dry environment under the pressure.
