Когнітивний профіль пацієнтів із розсіяним склерозом

Abstract

Одной из самых актуальных проблем современной клинической неврологии есть проблема изучения рассеянного склероза (PC). Согласно литературных источников, отмечается рост заболеваемости на PC, болеют люди молодого, работоспособного возраста, социально активные, что обусловливает актуальность данной проблемы. Известно, что очаговые неврологические изменения существенно влияют на работоспособность, социальную адаптацию, качество жизни пациентов, и не меньшее внимание заслуживает состояние высших корковых функций. Кроме того, определение нейропсихологического дефицита особенно необходимо не только в социальном аспекте для адаптации пациентов, но и для проведения оптимального патогенетического лечения с использованием превентивной иммунномодулирующей терапии, которая нуждается в соблюдении соответствующего режима введения. С целью изучения когнитивных функций было обследовано 75 пациентов с разным типом течения рассеянного склероза и в разные стадии заболевания. Полученные данные указывали на наличие деменции легкой, или умеренной степени у всех пациентов, но когнитивные нарушения преобладали у пациентов с вторично-прогрессирующим течением. В стадии обострения обнаружили более выраженные изменения у пациентов обоих полов в сравнении с ремиссией, при этом расстройства высших мозговых функций пре- обладали у женщин. Обнаруженные расстройства высших мозговых функций свидетельствуют о необходимости не только неврологического, но и комплексного, нейропсихологического обследования пациентов с целью назначения адекватной патогенетической терапии с учетом его когнитивного статуса; One of the most actual problems of modern clinical neurology is a problem of study multiple sclerosis (MS). In order literature, today, growth of morbidity is marked on MS, ill the of young people, which can work, socially active, that stipulates actuality of this problem. It is known, that hearth neurological changes substantially influence on a capacity, social adaptation, quality of life of patients, and not less attention is deserved by the state of higher crust functions. In addition, determination of neuropsychological insufficiency is especially needed not only in a social aspect for adaptation of patients but also for the leadthrough of optimum nosotropic treatment with the use of preventive immunnomodulative therapy which needs observance of the proper mode of introduction. A research purpose was become by the study of cognitive type of the patients with multiple sclerosis depending on the type of flow and floor. With a purpose the study of cognitive functions was inspected 75 patients with the different type of flow of the dissipated sclerosis and in the different stages of disease. Findings specified in the presence of demenciya of easy, or moderate degree for all patients, but cognitive violations prevailed for patients from the second time - by a making progress flow. In the stage of intensifying found out more expressed changes for the patients of both chaffs by comparison to remission, here disorders of higher cerebral functions prevailed for women. The found out disorders of higher cerebral functions testify to the necessity of not only neurological but also complex, neuropsychological inspection of the patients with the purpose of setting of adequate nosotropic therapy taking into account his cognitive status.
