Аналіз показників гемограм у пацієнтів із запальними і запально-дистрофічними змінами тканин пародонта

Abstract

В статті представлені дані вивчення показників десяти інтегральних гематологічних індексів у хворих генералізованим пародонтитом (ГП) з поєднаним перебігом хронічного холециститу і панкреатиту (ХХ і ХП) і у вагітних жінок із запально-дистрофічними змінами тканин пародонта в різні триместри вагітності. Встановлено, що у хворих ГП з поєднаним перебігом ХХ і ХП спостерігається підвищення індексу спів- відношення лейкоцитів і ШОЕ в 3,4 рази (р < 0,001), показника лімфоцитарно-гранулоцитарного індексу в 1,2 рази (р < 0,05) і загального індексу – в 1,8 рази (р < 0,01), що свідчить про наявність інтоксикації, а також про можливий розвиток аутоімунних процесів. Аналіз лейкограм вагітних показав, що під час вагітності, особливо в поєднанні з запальними змінами ясен і гестозами, спостерігається патологічний процес ураження клітин білої крові. Дані індекси мають ви- значену динаміку; В статье представлены данные изучения показателей десяти интегральных гематологических индексов у больных генерализованным пародонтитом (ГП) с сочетанным течением хронического холеци- стита и панкреатита (ХХ и ХП) и у беременных женщин с воспалительными изменениями тканей пародонта в разных триместрах беременности. Установлено, что у больных ГП с сочетанным течением ХХ и ХП наблюдается повышение индекса соот- ношения лейкоцитов и СОЭ в 3,4 раза (р < 0,001), показателя лимфоцитарно-гранулоцитарного индекса в 1,2 раза (р < 0,05) и общего индекса – в 1,8 раза (р < 0,01), что свидетельствует о наличии интоксикации, а также возможном развитии аутоиммунных процессов. Анализ лейкограмм беременных показал, что во время беременности, особенно в сочетании с воспали- тельными изменениями десен и гестозами, наблюдается патологический процесс поражения клеток белой крови. Данные индексы имеют определенную динамику; The paper presents the study of ten integrated haematological indicators indices in patients with generalized periodontitis (GP) combined with chronic cholecystitis course (CC) and pancreatitis (CP) and in pregnant women with inflammatory changes in periodontal tissues in different trimesters of pregnancy. It has been found that patients with GP with combined course of CC and CP display a significant increase in leukocyte index (LI) by 1.28 times (p < 0.001), reflecting the relationship between the humoral and cellular links of the immune system. The index value of leukocytes and erythrocyte sedimentation rate (IL ESR) confirmed the presence of intoxication. It was increased by 3.4 times (p < 0.001) as compared to IL ESR in healthy individuals. This probably indicates infectious and autoimmune nature of the pathological process in patients with GP against the background of CC and CP which is more significantly manifested in patients with GP of II severity degree. A statistically significant difference was obtained during the determination of hematological and other indices. Thus, the value of lymphocyte granulocyte-index (ILH) was increased by 1.2 times (p < 0.05). The index value of neutrophils and monocytes (ISNM) was increased by 3.5 times (p < 0.01) which reflects the ratio of components of the microphage-macrophage system. The index of the ratio between neutrophils and lymphocytes (ISNL) was slightly reduced 1.2 times (p < 0.05) as compared with healthy individuals, reflecting the ratio of specific and nonspecific cell protection. Analysis of hematological parameters in dynamics of pregnancy showed that leukocyte intoxication index (LII) in the first trimester of patients in the control group was 1.81 ± 0.07 (p < 0.05), which is by 1.8 times more than the donors. Besides, the increase of LII of 6.7% was noted by the third trimester. This dynamics suggests the growth of endogenous intoxication in pregnant women with inflammatory processes in the periodontium. The obtained data of ration between leukocytes and ESR (ILESR) in pregnant women with inflammatory lesions of periodontal tissues confirm the presence of intoxication. Similarly to LII index, ILSHOE increases by 9.94 times by the third trimester of pregnancy. It is likely that such a change may indicate the nature of autoimmune pathological processes during the gestational period. Average parameters of granulocyte-lymphocytic index (ILH) significantly decreased from I to III trimester by 6%, which allows us to differentiate autointoxication and infectious intoxication. Analysis of shift index of leukocytes (SIL) in pregnant women with gingivitis showed a slight increase of 5.6% in the second trimester and data values retention to the time of childbirth that may indicate inflammation activity and violations of immunological reactivity. Index of the ratio of neutrophils and lymphocytes (ISNL) from I to III trimester increased by 6.7%. A statistically significant increase in performance index value of neutrophils and monocytes (ISNM) by 1.2 times from the beginning to the third trimester of pregnancy may indicate activation of nonspecific inflammatory process. It should be noted that other integral indices were statistically and clinically less significant.
