Abstract

Проблема профілактики та лікування хронічних запально-дистрофічних захворювань пародонту, перш за все, генералізованого пародонтиту (ГП), залишається однією з найбільш складних в сучасній стоматології. Це пов'язано з недостатнім розумінням етіології і патогенезу зазначених захворювань. З 60-х років і до наших днів провідною є концепція інфекційного генезу ДП, доповнена в середині 70-х років розумінням ролі імунної відповіді організму. Робота має на меті привернути увагу патофізіологів, імунологів, лікарів-стоматологів до того факту, що пародонт може пошкоджуватися не тільки патогенною мікрофлорою і надмірною реакцією імунної системи на інфекцію по аутоімунному типу, але і без інфекційного фактора, як би «зсередини», внаслідок тих іммуннометаболіческіх порушень , які виникають при розвитку системного запалення; Проблема профилактики и лечения хронических воспалительно-дистрофических болезней пародонта, прежде всего, генерализованого пародонтита (ГП), остается одной из наиболее сложных в современной стоматологии. Это связано с недостаточным пониманием этиологии и патогенеза указанных заболеваний. Настоящая работа имеет целью привлечь внимание патофизиологов, иммунологов, стоматологов к тому факту, что пародонт может повреждаться не только патогенной микрофлорой и чрезмерной реакцией иммунной системы на инфекцию по аутоиммунному типу, но и без инфекционного фактора, как бы «изнутри», вследствие тех иммуннометаболических нарушений, которые возникают при развитии системного воспаления; The problem of prevention and treatment of chronic inflammatory-dystrophic periodontal diseases, first of all, generalized periodontitis(GP) , remains one of the most complex in modern dentistry. This is due to a lack of understanding of the etiology and pathogenesis of these diseases. From the 60s to the present day, the leading concept is the concept of infectious genesis of GP, supplemented in the mid-1970s by an understanding of the role of the immune response of the body. A new understanding of the causes of chronization of internal diseases, which revolutionized the minds of internists, has not yet entered dentistry. This is the role of chronic low-intensity inflammatory inflammation (low grade inflammation, systemic inflammation, SI). This concept, according to the editorial board of the journal Science, is one of the top ten most important discoveries of the 21st century. The present work aims to draw the attention of pathophysiologists, immunologists, dentists to the fact that the periodontium can be damaged not only by pathogenic microflora and the excessive reaction of the immune system to an autoimmune infection, but also without an infectious factor, as it were, "from within," due to those immunometabolic disorders, which occur with the development of systemic inflammation. Below, we will try to outline the essence of this new concept and evidence of the role of SI in the genesis of SOEs, as well as the bilateral nature of the relationship between SI and GP. Dentistry and therapy, as components of medicine, constantly demonstrate the unity of the body. Especially, it concerns periodontology and the relationship between oral infection and the development of systemic internal diseases, focal-caused pathology . This problem has more than a century of history, in which there were also excesses in the direction of a radical sanation of oral infection in the form of removing outwardly healthy teeth, depulled teeth with the purpose of eliminating the dental foci of chroniocepsis and attempts to deny any connection between the pathology of the oral cavity and the internal pathology. In the development of SI, a great role is played by genetic features, both of the immune system, and of physiological systems responsive to external stimuli. The authors of the latest reviews of literature and meta-analyzes, like the researchers themselves, recognize that the mechanisms of participation of oral infection in the pathogenesis of internal diseases have not been adequately studied. There is ample evidence that PD treatment is hampered by the need for correction of SI , and without the elimination of oral infection it is very difficult to combat diseases based on SI. PD can be the first and only sign of immunometabolic disorders. It is necessary to recognize the bilateral nature of the relationship between GP and SI and to use the data accumulated in relation to the etiology and pathogenesis of SI in the prevention and treatment of GP.
