Документ Покращення процесу адаптації до знімних конструкцій зубних протезів у хворих на ішемічну хворобу серця(Вищий державний навчальний заклад України «Українська медична стоматологічна академія», 2016) Рябушко, Наталія Олексіївна; Рябушко, Наталия Алексеевна; Ryabushko, N.; Дворник, Валентин Миколайович; Дворник, Валентин Николаевич; Dvornik, V.; Павліш, Ігор Вікторович; Павлиш, Игорь Викторович; Pavlish, I.; Баля, Геннадій Миколайович; Баля, Геннадий Николаевич; Balya, G.За результатами проведеного анкетування пацієнтів з ішемічною хворобою серця до і після застосування лікувально-профілактичного комплексу ми дійшли висновку, що покращуються загальні відчуття пацієнтів, слабшають відчуття сухості в порожнині рота (на 40,9%) і дискомфорту при застосуванні зубних протезів (на 29,2%), отже, покращується адаптація до знімних конструкцій зубних протезів; По результатам проведенного анкетирования пациентов с ишемической болезнью сердца до и после применения лечебно-профилактического комплекса мы пришли к выводу, что улучшаются общие ощущения пациентов, ослабевают ощущение сухости в полости рта (на 40,9%) и дискомфорта при применении зубных протезов (на 29 2%), следовательно, улучшается адаптация к съемным конструкциям зубных протезов; The aim - to improve the process of adaptation to removable designs of dentures in patients with coronary heart disease. To achieve this goal, we examined 81 patients aged 42 to 67 years. I (basic) group consisted of 53 patients who underwent structures removable dentures (partial removable dentures (PRD) and who had a history of major diseases - coronary heart disease, stable tensional angina FC II - III FC. II group - 31 patients who received the combinED treatment together with a cardiologist (antiplatelet therapy and dental treatment) and who were administered health care complex. The control group was involved 28 people aged 42 to 60 who belonged to the second group of health by the WHO classification. Patients of the main group after treatment of the underlying disease and the appointment of conservative treatment in 7-20 days time held dental treatment (partial removable dentures with appointment of health-care complex). The proposed health care complex had such algorithm of use: softener oral "Corsodyl" - 3-5 times a day after meals; clean teeth and dentures 2 times a day (morning and bedtime) toothpaste "Parodontax" and the bath of solution Ringer 3-5 times a day, with dryness in the mouth For the functional assessment made dentures were designed by us and proposed to use two methods of assessing patients to adapt designs removable dentures: - Objective evaluation - "Method of determining the degree of adaptation to the designs of removable dentures," Ukraine patent for utility model №101852 of 12.10.15; - Subjective evaluation - "Method of accelerated determination adaptation of patients to removable dentures designs using screening test" certificate of registration of copyright Ukraine №59280 15.04.2015. The first questionnaire includes only clinical data fills a dentist and does not reflect the subjective feelings of the patient. The second profile is completely based on the coverage of their own feelings patient and filled only patient. According to these questionnaires, we determined the degree of adaptation of patients to removable designs of dentures and compared its components. Questioning was conducted immediately after the prosthesis after 30 days and after 80-100 days of treatment. The results of the survey of patients with coronary heart disease before and after the use of health-care complex, we have concluded that improving the general feeling of patients diminish dryness of the mouth (40.9%) and discomfort when using dentures (29, 2%), thus improving adaptation to removable designs of dentures.Документ Спосіб виготовлення хірургічного шаблону для встановлення дентальних імплантатів(Вищий державний навчальний заклад України «Українська медична стоматологічна академія», 2016) Павліш, Ігор Вікторович; Павлиш, Игорь Викторович; Pavlish, I.; Дворник, Валентин Миколайович; Дворник, Валентин Николаевич; Dvornik, V.; Рябушко, Наталія Олексіївна; Рябушко, Наталия Алексеевна; Ryabushko, N.В статті представлений розроблений нами спосіб виготовлення хірургічного шаблона для дентальної імплантації з врахуванням даних комп’ютерної томографії. Спосіб базується на принципі перенесення даних про положення імплантату в анатомічних структурах після віртуального планування імплантації з допомогою скан-шаблона і пристрою для позиціонування моделей. При цьому шаблон виготовляють на гіпсовій моделі, яку встановлюють у позиціонуючий пристрій. З цією метою нами були розроблені оригінальні конструкції скан-шаблона та позиціонуючого пристрою, який фіксується в паралелометрі. Даний спосіб, поряд з високою точністю, має низку переваг перед існуючими комерційними системами, які мають високу вартість: відносна простота позиціонуючого пристрою, якій фіксується у стандартному стоматологічному паралелометрі, можливість використання любої програми придатної для перегляду і аналізу комп’ютерних томограм, а в особливих ситуаціях можливо виготовити шаблон без застосування комп’ютера. Дані переваги розширюють можливості застосування хірургічних шаблонів у повсякденній практиці; В статье представлен разработанный нами способ изготовления хирургического шаблона для дентальной имплантации, который учитывает данные компьютерной томографии. В основе способа лежит принцип перенесения данных о положении имплантата в анатомических структурах после виртуального планирования имплантации с помощью скан-шаблона и устройства для позиционирования модели. С этой целью нами были разработаны оригинальные конструкции скан-шаблона и позиционирующего устройства, которое закрепляется стоматологическом параллелометре. Данный способ, наряду с высокой точностью, имеет ряд преимуществ перед существующими коммерческими дорогостоящими системами изготовления хирургических шаблонов: относительная простота конструкции позиционирующего устройства, которое предусматривает использование стандартного стоматологического параллелометра, возможность использования любых компьютерных программ, предназначенных для просмотра и анализа данных компьютерной томографии, а в отдельных случаях и без них. Использование нашего способа позволит изготавливать точные хирургические шаблоны с минимальными затратами в любой зуботехнической лаборатории и обеспечит более широкое их применение; Basis of endoosseal implantation is insertion of an implant infrastructure in-to the alveolar bone and use of it for fixing of denture. It is difficult to over-estimate importance of an adequate implant location. In dental implantology the guided planning allows us to attain high precision of implant insertion, taking into account anatomic terms next to minimum invasion interference. A surgical template is the individually made device for the correct keeping of drill during the operation of implantation and, as a rule, it is a plate, which is supported by teeth, gum of jaw or both with the shells located in it through which preparing of bone is conducted. There are different methods of surgical guide making. Some of them require the exact account of anatomic features of area of implantation which can be realized by the use of information of computer tomography and CAD-CAM technologies (Nobel Guide: Nobel Biocare; SKYplanX: Bredent, SimPlant: Materialise Dental; Implant 3D: Med 3D та ін.), or by measuring of thickness of mucus membrane (mucosa mapping) taking into account information of panoramic skiagraphy. We put a purpose to develop the method of surgical guide taking into account information of computer tomography, but without the use of the special software and expansive equipment of the commercial systems of planning of dental implantation. Materials and methods. Analyzing modern technologies of surgical guide making, which are based on CT data, we selected three basic methods which differ on principle of transference of computer planning information from a virtual environment to the real surgical template. We take for basis method of transference of information of an implant position in anatomic structures is received after the virtual planning of implantation using scan- pattern and model positioning device with the use of the specialized software. Thus a template is made on a stone model which is set in a positioning device (SkyplanX, Bredent (Germany) and Implant 3d, Med 3d (USA)). The original constructions of scan- pattern and positioning device for this purpose were developed and build. Computer tomography was conducted on a high-speed cone-beam tomography Pax–zenith 3d, VATECH, South Korea. For a vision and analyzing of CT-research the regular software Ez3d 2009 was used, VATECH. As a result the next stages of surgical guide manufacturing was developed: I. Making of scan- templet. This stage includes impression taking, diagnostic model casting, positioning of X-ray contrast markers and templet forming. II. Leadthrough of computed tomography with scan- pattern. III. Installation of guiding shells in-to the template. This stage includes the next steps: drawing of marking elements (determining of drilling location), determining and fixation of model ground position using of positioning device, determining of drilling angulation based on CT data measurements using of positioning device, drilling of model, preparing of template and fixation of guiding shells. A method of surgical guide manufacturing, we have developed, has several advantages, including: the use of standard dental parallelometer and a relatively simple device for model positioning, which is installed in a standard coupling parallelometer. The method of transferring of CT scan data to the model using a original designed template does not require of specialized for planning of dental implantation. For our techniques you can use any software suitable for viewing and analyzing, and in special cases may make a guide without a computer using a standard CT scans printout.
