Однако даже такие высокоточные конструкции дают определенный процент неудач и осложнений, которые подробно описаны в данной статье; Nowadays the method of teeth restoration with veneers is a leader of the defects of the front teeth removal due to its constructions which are non-invasive and have a good aesthetic. However, such high-precision restorations have a failure rate and complications that are detailed below in the article. The outcomes of porcelain veneers have been studied during a long period. A recent meta-analysis found the summary cumulative survival was 95% at 5 years, 93% at 10 years and 83% at 20 years. The change of the colour, non-satisfaction with restoration shade, postoperative sensitivity, secondary caries, bad marginal adaptation, marginal discoloration, fractures were common complications for teeth restored with porcelain composite veneers. Tooth preparation causes the formation of smear layer, which is removed or modified by the adhesive system. In order to achieve a stable bonded interface to dental tissues, adhesive systems have to include three crucial application steps, i.e. etching, priming and bonding. Etching with an acidic solution, such as 35–37% orthophosphoric acid, demineralizes both enamel and dentin surfaces producing roughness and increasing their surface of free energy. With priming, wettability of the tooth surface is increased and hydrophilic monomers and solvents in the primer composition provide substitution for the water on the substrate with the resin monomers. In the bonding step, after penetration and polymerization of monomers onto the etched enamel surface or exposed collagen network in dentin, micromechanical interlocking occurs. Self-etching refers to the acid etching and priming of dentin at the same time with acidic monomers without rinsing resulting in a modified smear layer to form the hybrid layer between the dentin and resin, whereas etch-and-rinse approach refers to acid etching of the enamel/dentin surfaces and after this treatment the surfaces are rinsed in order to eliminate the smear layer and then primer, bond and resin application to form the hybrid layer. Biodegradation of the non-encapsulated collagen fibrils are the main cause of degradation of the hybrid layer and thus of adhesion between the tooth and the restoration. Biodegradation occurs in the hybrid layer and involves consecutive processes. The first stage of biodegradation begins when dentin is acid-etched for removal of the smear layer, exposing the underlying collagen fibril matrix for hybrid layer formation. The second stage involves extraction of the resins that had infiltrated the dentin matrix through water-filled voids within the hybrid layer. The third stage involves enzymatic attack of the exposed collagen fibrils, leading to depletion of collagen fibrils. It has been demonstrated that exposure of collagen matrix by acid etching activates matrix metalloproteinases (MMPs), that are known to cause collagenolysis in the presence of water. To ensure the retention is achieved, good marginal fit, tightness of indirect restorations also largely depends on luting cement. Materials for fixation must be indifferent to the tissues, do not cause the irritation of dentin and pulp, provide strong bond strength results and adhesion, to keep a constant volume and does not deform during curing, get a coefficient of thermal expansion close to the coefficient of expansion of tissues. Marginal adaptation which determined by the shrinkage of the material - it should be minimal, and it also depends on film thickness of luting cement. Today, porcelain laminate veneers are mainly used to optimize tooth form and position, close diastema, replace discoloured or unaesthetic composite resin restorations, restore teeth with incisal abrasions or tooth erosion, and mask or reduce tooth discoloration. They are a valid alternative to complete-coverage restorations since they avoid aggressive dental preparation, thus maintaining tooth structure. However, even such high-precision restorations have a failure rate and complications. Veneer removal is generally performed with a rotary instrument. Using this method, the veneer removal is complete, but is relatively time consuming and this technique is not ideal as the underlying tooth structure may be damaged. Moreover, the irritating noise, uncomfortable vibration and pain during veneer removal with high-speed handpieces are unpleasant for the patients. These disadvantages led to the search for new techniques for hard dental tissue removal, such as lasers, which would make treatment less painful to patients, with no need of local anaesthetics. With the recent introduction of lasers in dentistry, there may be beneficial application of lasers in removing veneers with lasers. Using an Er:YAG laser allows debonding porcelain veneers from teeth without aggressive destruction or removal of underlying tooth structure and without destroying the veneers. Purpose. The aim of our experimental study was to investigate the safety of indirect ceramic restorations removal by comparing the dynamics of thermometric indicators. Materials and methods. Experimental studies conducted at Shupyk National Medical Academy of Postgraduate Education. According to our study following groups were created: group 1 (control) – veneer removal using rotary instruments (24 veneers): IА–ceramic veneers, fabricated from VITABLOCS MarkII, IB – ceramic veneers, fabricated from IPS e.max CAD; group II – veneer removal using a solid-state laser (Er: YAG) (32 veneers): IIА – ceramic veneers, fabri cated from VITABLOCS MarkII, IIB - ceramic veneers, fabricated from IPS e.max CAD; group III – veneer removal using a solid-state laser (Er, Cr: YSGG) (34 veneers): IIIА – ceramic veneers, fabricated from VITABLOCS MarkII, IIIB – ceramic veneers, fabricated from IPS e.max CAD. The intrapulpal temperature measurements were performed with thermocouple. Results of the study: According to the results of our study using an Er:YAG and Er, Cr: YSGG lasers allows debonding porcelain veneers from teeth without aggressive destruction or removal of underlying tooth structure and in most cases without destroying the veneers, make veneer debonding less painful to patients. Both laser groups generated significantly less heat compared to bur group (p<0,05). None of the laser groups increased pulpul axial wall heat temperature above the critical 5,5˚С (3,4˚С for Er:YAG and 2,3 for Er,Cr:YSGG lasers). The bur group generated higher heat (more than 5,5˚С). According to the results of our experimental study on thermal safety Er:YAG and Er, Cr:YSGG lasers allow to remove indirect ceramic restorations (veneers) below the critical threshold described of 5,5˚С (Zach and Cohen).Документ Клінічна оцінка ускладнень при протезуванні непрямими реставраціями фронтальної групи зубів(Українська медична стоматологічна академія, 2017) Чамата, В. Однако даже при применении виниров с использованием высокоточных технологий и надежных фиксирующих цементов, к сожалению, со временем наблюдается большое количество механических, биологических и эстетических неудач; The esthetic appeal, durability, and biocompatibility of porcelain laminate veneers (PLVs) have made them an established option for restoring anterior teeth for almost three decades. Today, PLVs are mainly used to optimize tooth form and position, close diastemata, replace discolored or unesthetic composite resin restorations, restore teeth with incisal abrasions or tooth erosion, and mask or reduce tooth discoloration. They are a valid alternative to complete-coverage restorations since they avoid aggressive dental preparation, thus maintaining tooth structure. However, even such high-precision restorations have a failure rate and complications that are detailed in this article. The least common problems associated with porcelain laminate veneers are marginal discoloration and loss of color stability. A possible cause of marginal discoloration and the loss of color stability of the restoration is marginal leakage or a breakdown of the bond either between the cement and the tooth or between the cement and the veneer. This discoloration starts as a dark line but eventually works its way under the restoration, with a resultant diffused discoloration that spreads from the involved margin. Another rare occurrence is the cohesive failure of either the tooth or the porcelain. In the first instance, the fracture of the underlying tooth is usually the result of poor judgment in selection of the tooth to be veneered. Vital anterior teeth with large existing restorations on the mesial and distal surfaces and nonvital anterior teeth might be better served with full-coverage porcelain restorations bonded to the additional surface area of the crown preparation on dentin. A more common problem is the cohesive failure of the porcelain itself, which may occur during cementation as a result of a poor-fitting restoration, a resin that is too thick (viscous), or a resin that has gone through some initial setting. The latter can result if the resin is left too long in ambient light or unit light. Cohesive failure also may occur after cementation as the result of poorly planned occlusion or traumatic injury. The aim of this clinical study was to evaluate the clinical quality, success rate of porcelain laminate veneers. Anterior teeth in the maxillae and mandibles of 65 patients were restored with 356 porcelain veneer restorations. Veneer failures and reasons for failure were recorded. Additionally, patients were asked about their satisfaction with the veneer restoration and if they would undergo treatment again. Only anterior veneers were included in this study. Clinical examination was performed during patients’ regularly scheduled maintenance appointments. Esthetic match, porcelain surface, marginal discoloration, and integrity were evaluated following modified California Dental Association/Ryge criteria. The main clinical outcomes identified in this research were: fracture/chipping, debonding, caries recurrence, endodontic problems after cementation, severe marginal discoloration, color match, marginal integrity, hypersensitivity, misplaced veneers. Results: The main reason for failure was fracture of the ceramic (5,1%), hypersensitivity (1,1%), noncolor match (4,2%), debonding (2%), caries recurrence (3,9%), endodontic problems after cementation (1,1%), severe marginal discoloration (2%), bad contact point (1,1%), defects surface texture (4%) , marginal integrity (13%), , misplaced veneers (2%) . At such vulnerable locations, severe marginal discoloration (19%), loss of restoration gloss (1,1%), deterioration of fixing cement (2%) and caries recurrence (10%) were frequently observed. Conclusion: It was concluded that porcelain veneers represent a reliable, effective procedure for conservative treatment of unaesthetic anterior teeth. Occlusion, preparation design, presence of composite fillings, and the adhesive used to bond veneers to tooth substrate are coverable that contribute to the clinical outcome of these restorations in the long-term.Документ Оптимізація процесу дебондингу непрямих реставрацій фронтальної групи зубів(Українська медична стоматологічна академія, 2017) Чамата, В.
