Показано, что дети с II генотипом АСЕ гена имеют меньше шансов в развитии асфиксии и ее тяжелого течения. The review article presents the results of years of research of domestic and foreign scientists regarding the metabolic syndrome, which is one of the most important issues of modern medical science. This pathology combines such important components as abdominal obesity, reduced tolerance to carbohydrates, insulin resistance, dyslipidemia, hyperinsulinemia, chronic or acute arterial hypertension, hyperandrohenia in girls, reduced fibrinolytic activity, microalbuminuria, and hyperuricemia, which cause rapid development of complications leading to social exclusion, disability, and mortality. The main etiological factors of the metabolic syndrome are considered a genetic predisposition, poor diet, and lack of physical exercise. Hyperinsulinemia and insulin resistance, endocrine dysfunction of internal fat, excessive activity of sympathoadrenal and reninanhiotenzin-aldosterone systems, endothelial dysfunction, thus developing metabolic disorders, are important in the pathogenesis of the metabolic syndrome and its complications. The only criteria for the diagnosis of major components of the metabolic syndrome are obesity, hypertension, hyperglycemia, increased triglycerides, reduced levels of HDLP (high-density lipoprotein) in boys and girls. Anthropometric parameters in children and adolescents depend on age and gender, so for the diagnosis of overweight there are used protsentyl tables of BMI (body mass index) distribution in the population according to the age and gender. The evaluation of insulin resistance should consider not only the insulin level but also the level of glucose, which is important to identify the violations of glucose tolerance. Hyperinsulinemia against the backdrop of normoglycemia usually indicates the presence of insulin resistance. For the diagnosis of insulin resistance one can use the oral glucose ratio calculation of correlation between insulin and plasma glucose at the beginning and after two hours. In order to detect insulin resistance one can also use calculated indices of insulin sensitivity and insulin resistance. In children and adolescents, comprehensive treatment of the metabolic syndrome should include a complete change of lifestyle, concomitant treatment of obesity, abnormalities of carbohydrate metabolism, hypertension, and dyslipidemia. This change provides prompt normalization of regime and diet of the child, optimization of his/her physical exercise, comprehensive psychotherapy, problem-targeted training programs, and strict self-control. The main approach to the treatment of the metabolic syndrome in children is to optimize their lifestyle with increased physical activity and diet destination. The child should walk more, engage in various sports, and spend more time outdoors. At the same time, the child should be limited with a computer or TV. The diet should be highly calorific—with restriction of refined fats and «empty» carbohydrates (chocolate, sweet drinks, candy, and sugar). It is necessary to enrich the diet of the child with fruit and vegetables, as they are the main source of fiber. Meals should bring to 4-5 times a day, with breakfast must be complete. The metabolic syndrome in children and adults is easier to prevent than to cure, and to this should lead a healthy lifestyle; Резюме. В оглядовій статті наведені результати багаторічних досліджень вітчизняних та іноземних науковців стосовно метаболічного синдрому, який є одним із найважливіших питань сучасної медичної науки. Дана патологія поєднує такі важливі складові, як абдомінальне ожиріння, зниження толерантності до вуглеводів, інсулінорезистентність, дисліпідемію, гіперінсулінемію, хронічну або гостру артеріальну гіпертензію, гіперандрогенію у дівчат, знижену фібринолітичну активність, мікроальбумінурію, гіперурикемію, які спричиняють швидкий розвиток ускладнень, призводять до соціальної дезадаптації, інвалідності та смертності; Резюме. В обзорной статье представлены результаты многолетних исследований отечественных и иностранных ученых касательно метаболического синдрома, который является одним из самых важных вопросов современной медицинской науки. В статті автори, крім літературного огляду, приводять клінічний приклад дитини з діагнозом: «Гарячка невідомого генезу»; Актуальность проблемы «Лихорадки неясного генеза» обусловлена возростанием частоты встановленого предварительного диагноза педиатрами амбулаторного звена на Украине за последние 5 лет, большой вариабельностью причин болезни, сложностью специфической диагностики, что ведёт к диагностическим ошибкам, несвоевременному лечению детей, снижению доверия пациента к врачу. У статті проведено аналіз особливостей клінічного перебігу гострих респіраторних захворювань у дітей на тлі інфікування Сhlamydоphila pneumonia у порівнянні з дітьми з негативними результатами обстеженнями на хламідіоз. Показано, що інфікування Сhlamydоphila pneumonia частіше виявлялося у дітей старше 7 років. У дітей, інфікованих Сhlamydоphila pneumonia, при всіх нозологіях (пневмонії, гострі бронхіти, гострі респіраторні вірусні захворювання) характерним є поступовий початок захворювання, нормальна або субфебрильна температура тіла, болісний нападоподібний кашель, тривалий перебіг. Позалегеневими проявами інфікування Сhlamydоphila pneumonia у дітей є гепатомегалія, гіперплазія периферичних лімфатичних вузлів, міальгії, артралгії, функціональні порушення серцево-судинної системи.Для дітей, хворих на гострі респіраторні захворювання на тлі інфікування Сhlamydоphila pneumonia, характерна еозинофілія, низький рівень інтенсивності запальної реакції та імунної відповіді, функціональна депресія мононуклеарного фагоцитозу, зниження активності сукцинатдегідрогенази в лімфоцитах; Резюме. В статье проведен анализ особенностей клинического течения острых респираторных заболеваний у детей на фоне инфицирования Сhlamydоphila pneumonia по сравнению с детьми с отрицательными результатами обследованиями на хламидиоз. Показано, что инфицирование Сhlamydоphila pneumonia чаще выявлялось у детей старше 7 лет. У детей, инфицированных Сhlamydоphila pneumonia, при всех нозологиях (пневмонии, острые бронхиты, острые респираторные вирусные заболевания) характерно постепенное начало заболевания, субфебрильная температура тела, мучительный приступообразный кашель, длительное течение заболевания. Внелегочными проявлениями инфицирования Сhlamydоphila pneumonia у детей является гепатомегалия, гиперплазия периферических лимфатических узлов, миалгии, артралгии, функциональные нарушения сердечно-сосудистой системы. Для детей, больных острыми респираторными заболеваниями на фоне инфицирования Сhlamydоphila pneumonia, характерна эозинофилия, низкий уровень интенсивности воспалительной реакции и иммунного ответа, функциональная депрессия мононуклеарного фагоцитоза, снижение активности сукцинатдегидрогеназы в лимфоцитах; Abstract. Paper presents an analysis of clinical peculiarities of acute respiratory diseases in 70 children aged from 3 to 12 due to Сhlamydоphila pneumonia infection compared with clinical features of nosology in 60 children with negative analysis for Chlamydia. Such investigations have demonstrated Сhlamydоphila pneumonia infection is often diagnosed in children aged 7 years old. It has been shown for all nosologies of respiratory diseases (pneumonia, acute bronchitis and acute respiratory viruses) in children who are infected by Сhlamydоphila pneumonia characterized by gradual development of disease, normal or subfebrile body’s temperature, paroxysmal cough without sputum discharge, long-lasting course of treatment. Hyperplasia of peripheral lymphatic nodes, hepatomegaly, myalgia or arthralgia, functional disorders of the cardiovascular system as a systolic murmur, tachycardia are extrapulmonary features of Сhlamydоphila pneumonia infection in children. Children who are infected by Сhlamydоphila pneumonia are prone to unfavorable factors of preclinical features such as perinatal encephalopathy, rickets, anemia, atopic dermatitis. Children who have Сhlamydоphila pneumonia infection were prone to acute respiratory diseases and bronchitis. An analysis has demonstrated, children who have Сhlamydоphila pneumonia infection suffered from acute respiratory diseases which were accompanied by acute bronchitis with complications of bronchial obstructive syndrome and long-lasting course of treatment. Pneumonia is the most common manifestation of acute respiratory diseases in children infected by Сhlamydоphila pneumonia. Pneumonia is atypical disease which it’s difficult to diagnose due clinical features. Chlamydia infection can be manifested in such cases as gradual development of disease, the presence of bronchial and obstructive syndrome, paroxysmal cough without sputum discharge or with serous discharge, signs of respiratory deficiency of I-II stages without significant intoxication, maintenance and reinforcement of lung pattern due to regression of focal changes in lung tissue. Blood analysis in children infected by Сhlamydоphila pneumonia is characterized by moderate eosinophilia, low intensity of inflammatory reaction (low ESR (erythrocyte sedimentation reaction), low level of monocytes). The period of monocytosis was short-lived and only during the period of developed clinical picture of disease. Functional depression of phagocytosis was defined in children who are infected by Сhlamydоphila pneumonia compared with children without Chlamydia. They are also prone to low activity of main mitochondrial respiratory enzyme of succinate dehydrogenase in blood lymphocytes. В пуповинной крови у их детей значи- тельно снижен уровень гормонов щитовидной железы, что при дефиците йодного обеспечения матери может негативно влиять на адаптацию новорожденного. Обогащение рациона питания кормящих матерей,родивших маловесных детей, йодсодержащими препаратами значительно улучшает лактацию, достоверно повышая суточное количество молока и длительность грудного вскармливания на первом году жизни;В роботі показано, що у матерів, які народили передчасно маловагових недоношених дітей, в два рази частіше зустрічається патологія щитоподібної залози за даними УЗД. In umbilical blood of their children the level of thyroid hormones is significantly reduced, that in iodine deficiency of the mother may affect the adaptation of the newborn. В оглядовій статті наведені результати багаторічних досліджень вітчизняних та іноземних науковців стосовно етіологічних чинників виникнення ЦД І типу у дітей, який у ХХІ столітті є епідемією та патологією,що посідає третє місце за рівнем ранньої інвалідизації та смертності населення розвинутих країн світу. На сьо-годення такими чинниками встановлені спадковість, вірусні та перинатальні інфекції, швидкий ріст дитини, харчування, моральне та психологічне навантаження, матеріально-побутові умови, взаємовідносини в сім’ї,рівень фізичної активності, екологічний, середовищний, популяційний, геологогеоморфологічний факторитощо. Подальше вивчення причин виникнення ЦД І типу у дітей сприятиме розробці сучасних ефективних заходів попередження зростаючої захворюваності на дану патологію; Резюме. На сегодня такими факторами установлены наследственность, вирусные и перинатальные инфекции, быстрый рост ребенка, питание, моральне и психические нагрузки, материально-бытовые условия, взаимоотношения в семье, уровень физической активности, экологический, средовой, геологогеоморфологический факторы и др. Дальнейшее изучение причин возникновения СД І типа у детей будет способствовать разработке современных эффективных мероприятий предупреждения наростающей заболеваемости данной патологией; Abstract. The review article presents the results of many years research of domestic and foreign scientists regarding the etiological factors of the reasons for occurrence of type I diabetes in children, which is an epidemic and a pathology in the XXI century, ranking third in terms of early disability and mortality in developed countries. It is known that type I diabetes refers to the inherited pathologies. At present, it is proved that in most cases, type I diabetes is a genetic susceptibility, and the risk of diabetes of a patient’s close relatives is much higher. Possible genetic features of this disease are the expression of phenotypic antigens HLA DR3-DQ2 and/or DR4-DQ8 on lymphocytes of European race people. Identification of diabetes provoking loci may promote a susceptibility to type I diabetes with a fairly high percentage of certainty just at a child’s birth. In 1970, there was formulated the virus theory of type I diabetes. According to this theory, Coxsackie viruses, measles and rubella, megaloviruses, retroviruses, herpeviruses, and their varieties play the role of exogenous factors starting autoaggression due to the β-cells damage. No rational nutrition and low physical activity of children are also factors that contribute to the development of type I diabetes in children. In 1993, American doctors found that diabetes is more common in children who were at birth bottle-fed. Over the past decade, there were accumulated numerous details about the role of micro- and macro elements in the pancreas functioning. It is known that calcium is a key mineral that affects carbohydrate metabolism. Activity of endopeptydaza – an enzyme under which insulin is produced of proinsulin – depends on the presence of calcium ions in the cell. Zinc is involved in the synthesis and accumulation of insulin in β-cells; copper participates in the process of glucose oxidation; chromium promotes glucose metabolism in peripheral tissues and the synthesis of glycogen; manganese, nickel, and lithium potentiate the action of insulin. Ecological, environmental, population, geological and geomorphological factors are essential in the development of type I diabetes. Xenobiotics in the air, pesticides, nitrogen fertilizers, chlorine- and phosphorus organic substances that enter the human body through the respiratory tract, water, and food are among a large number of external factors polluting the environment, which are pathogenetically significant in the formation of type I diabetes. There exists an interrelation between indicators of disease of type I diabetes in children and the level of nitrates in the water they drink. Children with diabetes consume more foods containing nitrates, nitrites, or their nitro substitutions before diagnosis than their peers without diabetes. Throughout the world, the incidence of type I diabetes in European race population is higher than in Mongoloid and Negroid races. Most cases of type I diabetes in the northern hemisphere occur in winter (December-February), the minimum number turns in summer. Seasonal variations affect the incidence of children’s illness both family and sporadic diabetes equally. Some other factors, which associated with type I diabetes, such as the frequency increase of perinatal infections or the rapid growth of the child, were also revealed. It is assumed that improvement of hygiene level and low incidence of infectious diseases in childhood can lead to disruption of the immune system development and due to it increase of the frequency of autoimmune diseases. Further study of the reasons for type I diabetes in children today will contribute to the development of effective measures to prevent the increasing incidence of this pathology.
