Документ Azithromycin with scaling and root planing versus scaling and root planing alone in the treatment of periodontitis: A randomized controlled trial(Elsevier, 2023-07-15) Shynkevych, V. P.; Шинкевич, Вікторія Ігорівна; Коломієць, Світлана Веніамінівна; Кайдашев, Ігор ПетровичBackground The growing interest in the possibilities of macrophages modulation with therapeutic purposes promotes new approaches for periodontitis treatment. Aim The aim of this randomized controlled open clinical study was to evaluate the early clinical and immunological effects of the long-course azithromycin as an adjunct to scaling and root planing in periodontitis. Methods 50 patients (with stage I-III, grade A/B periodontitis) and 22 periodontally healthy volunteers as the reference group were recruited. Following scaling and root planing (SRP), the patients were randomly assigned to one of two treatment modalities: SRP only (n = 25) and adjunct azithromycin (Az) treatment (n = 25). The patients were monitored at baseline, and 30 ± 5 days after therapy. Clinical attachment loss (CAL), periodontal probing depth (PPD) and bleeding on probing (BoP) were evaluated. Secondary outcome measures included mean changes in single-positive CD68 + and CD163 + macrophages (Mφs) density and ratio, evaluated by immunohistochemistry, and IL1-β, IL-6, IL-10, TGF-β levels, detected by ELISA. Results At 1 month both groups showed significant improvements of CAL, PPD and BoP, without significant added benefit in terms of CAL, PPD and BoP of Az. But Az increased the density of CD68 + and CD163 + Mφs (P < 0.0001), decreased the CD68+/CD163 + ratio (P = 0.043), decreased IL-1β (P < 0.01), IL-6 (P < 0.001) levels, and increased IL-10 (P < 0.0001) and TGF-β (P < 0.001) levels compared to SRP and periodontitis at baseline. The child's age determines tooth trauma features and depends on the stage of tooth root formation and the action of the traumatic factor. And as a result of such interaction, the predictability of the course after receiving an injury, the possibility or impossibility of restoring the functions of the maxillofacial area, which requires the provision of timely qualified assistance to the child in total. Based on the systematic analysis and generalization of current scientific information, proposing effective treatment methods for trauma to temporary and permanent teeth in children will ensure positive long-term results. Conducted scientific research and generalization of the obtained data from literary sources of Ukrainian and foreign scientists based on such databases as Scopus, Web of Science, MedLine, and PubMed, the study of which does not exceed ten years, including monographs and results of clinical studies. The analysis of scientific literature shows that the choice of treatment tactics and its success largely depends on a thorough examination of the patient and the correct diagnosis of the received injury. The application of additional instrumental research methods, treatment planning, and follow-up are essential for achieving a favorable result. The prognosis of teeth after trauma depends on the type of traumatic damage, emergency care, and the time that has passed before dental manipulation. In clinical practice, a dental surgeon encounters a wide range of emergencies, including epileptic seizures, cardiac arrest, fainting (or syncope), and others. At the same time, syncope is often overlooked against the background of more severe and dangerous conditions. The aim is to assess the level of perceived stress and the state of the autonomic nervous system in outpatients of a dental surgeon at the beginning of the appointment, depending on the further development of syncope. We conducted a pilot prospective study that included 33 outpatients of a dental surgeon, who were divided into 2 groups: group 1 – patients who developed syncope (n=11); group 2 – patients who did not develop syncope during the visit to the clinic (n=22). During the medical history, patients were asked to complete a questionnaire to assess perceived stress (PSS-10). Every patient received a hemodynamic assessment that included measuring their blood pressure and pulse, determining the Kerdo Index, and examining their autonomic nervous system by checking for demographism. The amount of time spent in the queue was also documented. If a patient fainted, emergency care was immediately given.There were no significant differences in gender, age, diagnosis, or wait time between patients in both groups. There were no statistically significant differences between the groups of patients in terms of perceived stress. A statistically significant higher Kerdo index was found in group 1 with a tendency to sympathicotonia. Conclusions. Thus, we have determined that it is the increased activity of the sympathetic nervous system before an outpatient visit to a dental surgeon, rather than the subjective level of perceived stress, that may be potentially associated with the likelihood of developing syncope, which requires more detailed research. Although syncopal states are generally considered benign, frequent episodes of syncope can harm the patient’s quality of life, morbidity, and compliance with the dentist. Since 1991, internship-doctors specializing in "Therapeutic dentistry", "Dentistry of childhood", "Orthopedic stomatology", "Orthodontics" and "Surgical dentistry" study at the Department of Postgraduate Education of Doctors-Dentists of the Ukrainian Medical Stomatological Academy. The period of study on the full-time basis of the discipline "Surgical Dentistry" is 1 month. During seminars, practical classes, and lecture courses, internship-doctors study the basic issues of surgical stomatology: emergency care, anesthetics of the tissues of the maxillofacial area, typical and atypical teeth removal, inflammatory diseases, traumatic lesions of soft tissues and bones of the face, neoplasms of the maxillofacial area and more. Moreover, much attention is paid to differential diagnosis and modern methods of treatment. Undoubtedly, the basic and necessary form of training is lectures. In our opinion, the main methods that increase the effectiveness of lectures are discussions, the usage of interactive learning tools and, at the same time, the ability of the lecturer to get in contact with the audience easily paying no regard to the level of their training, who is a professional able to utilize high multimedia technologies along with pedagogical techniques and foreign languages. In addition, internship-doctors are present at the advisory activities of the professors and associate professors of the department, are involved in clinical treatments of not ordinary patients, have an independent appointment. The teachers of the department carry out individual work with each doctor, especially those who wish to link their future medical activities with surgical dentistry, and it is aimed at professional training of a future specialist. The success of the professional work of the dental surgeons depends, first of all, on their experience and knowledge of theirs specialty, the grounds of modern medical science in general, but their innate characteristics, features of character, correspondence to medical activity are not of less importance. For preparation to practical and seminar classes at the department are used self-developed study guides, monographs on surgical dentistry. It contains not only relevant educational material, but also models of test control, situational tasks, the solution of which helps in mastering the topic and indicates the ways of the practical application of the acquired knowledge. Interns receive computer-controlled training twice a month, and also use thematic programs that provide the possibility to study any sections of the course in surgical dentistry. The organization of research work of interns at the Department of Postgraduate Education in dentistry is carried out in three stages of different directions. Modern society considers a specialist not only as a person who possesses knowledge and skills in the professional field, but also as a person who is able to act effectively in complicated, non-standard situations, to make decisions independently, to self-develop and to perfect themselves, to be able to communicate people, and for this purpose All conditions are present at the department. I.Cтоматологічної допомоги потребують усі стаціонарні пацієнти із синдромом Стівенса-Джонсона та/або токсичним епідермальним некролізом (ССД/ТЕН), повідомлення про клінічні випадки яких публікуються щорічно в наукових журналах. Мета – аналіз стоматологічної допомоги за клінічними звітами про ССД/ТЕН та власного випадку ССД для окреслення шляхів її оптимізації. Для огляду проаналізовано клінічні випадки російською й українською мовами баз даних Google і Google Академія за ключовими словами. Аналіз установив одночасне призначення однакових препаратів у різних формах. Недостатня увага приділяється професійним й індивідуальним заходам щодо контролю зубної біоплівки. Використовуються екзотичні й застарілі токсичні засоби. Відсутні повідомлення про використання неадгезивних пов’язок. Таким чином, за останній час методи і засоби стоматологічної допомоги майже не змінилися. Менеджмент уражень органів ротової порожнини необхідно виконувати за допомогою частих полоскань і місцевих анестетиків для ослаблення болю, що визначає оптимальний мінімум для уникнення нових побічних впливів від невиправданої емпіричної і застарілої поліпрагмазії
