It is known that psychological factors have some relationship with adjustment to persistent pain, that is factors leading to increased/worse or decreased/improved adjustment to back pain. We conducted a case-control study in the Communal Enterprise “1st City Clinical Hospital of the Poltava City Council” during October-January 2021. The study included 19 people with complaints of neuropathic pain in the lower back who were divided into 2 groups: group 1 included 18 people with neuropathic pain; group 2 included 21 people with nonspecific pain. Patients suffering from low back pain have different severity of affective symptoms depending on its type. Neuropathic pain may be associated with higher depression due to higher severity, which should be noted in the choosing of treatment approaches.Документ Challenges and Gaps in the Treatment of Advanced Sleep Phase Disorder: A Call for Further Research and Understanding(University of Niš, 2024) Boiko, D. A steady circadian cycle of sleep and wakefulness that is phase-shifted relative to the prior local solar time may be characterized by a short endogenous circadian period. Advanced sleep-wake phase disorder (ASPD) is characterized by a substantial advancement of the sleep-wake cycle phase, followed with sleep-related symptoms. ASPD is a challenging sleep-wake disorder to manage, with current treatment options varying in efficacy and potential side effects. Further research is needed to identify effective treatment options and to elucidate the underlying mechanisms of ASPD. More research is needed to better understand the underlying mechanisms of ASPD and the molecular and metabolic changes associated with aging that may contribute to the development of the disorder.Документ Psychoeducational training for reducing the impact of a COVID-19 pandemic on healthcare workers(University of Nis, Faculty of Medicine, 2023) Boiko, D. Healthcare workers are increasingly exposed to long-term traumatic events in the context of the COVID-19 pandemic. Methods. We conducted a study that included 100 healthcare workers and 50 healthy individuals. We used the Perceived Stress Scale, Hospital Anxiety and Depression Scale, and Global Patient Impression for Improvement in the assessment of the psychological condition of respondents. To improve the psychoemotional state of health care workers, we proposed a program of psychoeducational training that was conducted at the workplace. After it, we reassessed the level of perceived stress, anxiety, and depression. Results. During the COVID-19 pandemic, in healthcare workers, the level of stress and anxiety was higher than in non-medical specialties, and the level of depression was not significantly different. The reduction of stress and anxiety was found. The subjective assessment of the improvement of the general condition indicates its positive dynamics after the psychoeducational training in both doctors and nurses. Conclusion. We showed the effectiveness of the proposed psychoeducational methodology for improving the psycho-emotional condition of healthcare workers during the COVID-19 pandemic.Документ Якість сну та уникаюча поведінка в комбатантів з посттравматичним стресовим розладом(Полтавський державний медичний університет, 2024) Бойко, Дмитро Іванович; Мац, Оксана Василівна; Шкідченко, Олександр Ігорович; Boiko, D. Посттравматичний стресовий розлад супроводжується низкою симптомів, що призводять до поведінкових змін пацієнта, зокрема поширеними явищами вважаються порушення сну та уникаюча поведінка. Незважаючи на докази про складний двосторонній зв'язок між емоціями та сном, лише кілька досліджень вивчали асоціації між порушеннями сну та уникаючою поведінкою в цьому контексті. Мета дослідження. Оцінити вираженість уникаючої поведінки та безсоння у комбатантів з посттравматичним стресовим розладом. Матеріали та методи дослідження. Нами проведено клінічне дослідження 45 комбатантів, яких було розподілено на 2 групи: група 1 (n=24) – комбатанти з ПТСР; група 2 (n=21) – комбатанти без ПТСР. Для аналізу якості сну було використано Пітсбургський опитувальник якості сну, а саме суб’єктивну якість сну, латентність сну, тривалість сну, ефективність сну, порушення сну, використання снодійних, добову дисфункцію. Уникаючу поведінку вимірювали за допомогою багатовимірного опитувальника уникнення досвіду. Результати досліджень. Нами встановлено, що у групі 1 була гірша суб’єктивна якість сну (р=0,002), більш тривала латентність сну (р<0,001), нижча ефективність сну (р=0,019) і більш виражені порушення сну (р<0,001). Не визначено статистично достовірних відмінностей між групами за тривалістю сну, вираженістю використання снодійних та добової дисфункції. Загальна вираженість порушення якості сну була вищою у групі 1 порівняно з групою 2 (р<0,001). Більш виражені прояви поведінкового уникнення (р=0,007) та стримування та заперечення (р<0,001) у групі 1 порівняно з групою 2. Поряд з цим у групі 1 була нижчою стійкість до стресу (р<0,001). Загальний рівень уникаючої поведінки в групі 1 був вищим порівняно з групою 2 (р<0,001). Поряд з цим встановлено численні зв’язки між характеристиками якості сну та проявами уникаючої поведінки. Висновки. Встановлено, що комбатанти з ПТСР мають більш виражені прояви поведінкового уникнення, стримування та заперечення, а також нижчий рівень стійкості до стресу. Поряд з цим при ПТСР у комбатантів спостерігається погіршення суб’єктивної якості сну, зниження його ефективності та збільшення вираженості його порушень. При цьому встановлено, що уникаюча поведінка у комбатантів має численні зв’язки з характеристиками якості сну, що може бути потенційним фактором для оптимізації психотерапевтичних втручань. Introduction. Post-traumatic stress disorder (PTSD) is often characterized by various symptoms that result in behavioral changes in patients, among which sleep disturbances and avoidant behavior are commonly observed. However, despite evidence indicating a complex bilateral relationship between emotions and sleep, only few studies have explored the associations between sleep disturbances and avoidant behavior in the context of PTSD. The aim of the study is to assess the severity of avoidant behaviour and insomnia in combatants with posttraumatic stress disorder. Materials and methods. We conducted a clinical study of 45 combatants who were divided into 2 groups: group 1 (n=24) included combatants with PTSD; group 2 (n=21) consisted of combatants without PTSD. The Pittsburgh Sleep Quality Questionnaire was used to analyse sleep quality, namely subjective sleep quality, sleep latency, sleep duration, sleep efficiency, sleep disturbances, sleep medication use, and daily dysfunction. Avoidance behavior was measured using a multidimensional experience avoidance questionnaire. Results. We found that group 1 demonstrated worse subjective sleep quality (p=0.002), longer sleep latency (p<0.001), lower sleep efficiency (p=0.019) and more severe sleep disturbances (p<0.001). There were no statistically significant differences between the groups in terms of sleep duration, severity of sleeping pills use, and daily dysfunction. The overall severity of sleep quality disorders was higher in group 1 compared to group 2 (p<0.001). Avoidance behavior (p=0.007) and repression and denial (p<0.001) were more pronounced in group 1 compared to group 2. At the same time, group 1 had lower stress endurance (p<0.001). The overall level of experiental avoidance in group 1 was higher than in group 2 (p<0.001). Numerous associations between sleep quality characteristics and experiental avoidance were found as well. Conclusions. The study has revealed that combatants with PTSD have more pronounced manifestations of behavioural avoidance, repression and denial, as well as a lower level of stress endurance. At the same time, combatants with PTSD demonstrate a deterioration in the subjective quality of sleep, a decrease in its effectiveness and an increase in the severity of its disturbances. It has been found that avoidance behavior in combatants has numerous connections with the characteristics of sleep quality that may be suggested as a potential factor for improving psychotherapeutic interventions.
