Документ Вплив ступеня артрозу колінного суглобу на вираженість синовіїту. Проаналізовано залежність вираженості синовіїту колінного суглобу від ступеня артрозу. Доведено, що пацієнти з більшим ступенем артрозу колінного суглобу мають більш виражений синовіїт колінних суглобів. Визначено схему розвитку неспецифічного синовіїту колінних суглобів. Досліджено, яким чином розвивається хронічний синовіїт. Він не буває вираженим та напруженим, так як є похідним комбінованої взаємодії зростаючого навантаження на м’язи та виражених ступенів артрозу; С помощью ультразвукового метода определены 4 стадии выраженности синовиита и 4 стадии артроза коленного сустава. Проанализирована зависимость выраженности синовиита коленного сустава от степени артроза. Доказано, что пациенты с большей степенью артроза коленного сустава имеют более выраженный синовиит коленных суставов. Определена схема развития неспецифического синовиита коленных суставов. Доказано, каким образом развивается хронический синовиит. Он не бывает выраженным и напряженным, так как является производным комбинированного взаимодействия возрастающей нагрузки на мышцы и степени выраженности артроза; Four stages of synovitis severity and four stages of gonarthrosis have been identified using ultrasound method. 461 patients have been examined during the period 08-09.2016, namely, 153 (33.2%) men and 308 (66.8%) women aged from 13 to 80 years, 218 (47.3%) – city-dwellers and 243 (52,7%) − rural residents. All the patients have been exposed to US examination of their both knee joints by the standard method for device company «BK Medical» with linear transducer with a frequency of 12.0 MHz. The front, side area and popliteal space in longitudinal and transverse projections have been scanned. Complaints about pain in the front thigh and shin have been considered. The dependence of the severity of knee synovitis from gonarthrosis degree has been analysed. It was determined that patients with higher stage of gonarthrosis have more marked knee joint synovitis. The scheme of development of nonspecific synovitis of the knee joint has been defined. With the absence of arthrosis and with its initial signs the protective properties of cartilage are not violated because synovitis develops when the load increases on account of increase in muscle spasm. Growth and expansion of the spasm up to 1/3 corresponds to most marked, intense synovitis. Further development of synovitis stops. The patient gets the treatment. With further overloading (physical and overweight) the patients have progress in osteoarthritis development within 3-5 year period. With marked stages of osteoarthritis (II-III radiographic stages by DOA) full absolution of fluid is not received on account of a permanent static power. Thus, chronic synovitis develops. It is never marked and intense as it is a derivative from combined interaction of the growing stress to muscles and severe stages of arthrosis. The study gives a new insight for understanding of the synovitis development. Synovitis of the knee joint develops in case when the load on the lower extremity exceeds the protective properties of the cartilage. The development process of synovitis has 4 stages that represent links of the same process: the first stage is muscular-tonic syndrome of semimembranous and medial gastrocnemius muscles, or nonexudative stage; the second stage – silent synovitis; the third stage – marked knee joint synovitis; the fourth stage – chronic synovitis. Thus, in addition to loads and excess weight, another cause of nonspecific knee joint synovitis development has been determined – the decrease of the cartilage protective forces (cartilage wear), that is, arthrosis development. The study has identified ultrasound stages of the knee joint synovitis that were divided into 4 groups depending on the degree of arthrosis development. Analysis of the correlation of the presence of ultrasonic synovitis signs and symptoms of gonarthrosis has determined that the highest number (33.3%) of patients without synovitis accounts for 1 group of patients without ultrasound signs of arthrosis. The greatest number of patients with the 2nd synovitis stage (silent synovitis) are represented in 4 group (72.7%) and 2 group (62.4%) – patients with dysplastic and initial signs of arthrosis, correspondingly. The third and fourth synovitis stages are the most common among 3 group patients with severe signs of arthrosis and amount to 30.5% and 24.4% in this group, correspondingly. These results demonstrate the correlation of the presence of synovitis ultrasound signs and gonarthrosis. O.Серед різноманітних проявів хронічних захворювань колінного суглобу провідне місце займає синовіїт, частота виникнення якого в останні роки невпинно зростає, складаючи від 9 до 33% патології колінного суглобу. Мета. Аналіз причин виникнення неспецифічного синовіїту колінного суглобу в основі оцінки структурних змін м’язово-сухожилкового комплексу. Методи. Вивчено ультрасонографічні дані за період 08/09 2016 р., 461 пацієнтів у віці від 13 до 80 років зі скаргами на біль в КС і підколінній ямці, обмеження рухів. Результати. Для аналізу виділено 2 групи пацієнтів. 1 група – 28 пацієнтів з потовщенням діаметру сухожилку м’язу semimembranosus, більш ніж 4 мм в місці дистальної инсерції, наявністю лінійних гіперехогенних ділянок в структурі м’язової порції на рівні середньої та нижньої третин стегна. 2 група – 433 пацієнтів з наявністю анехогенної ділянки рідини в супрапателярному завороті між листками синовіальної оболонки. Висновки. Обґрунтовано сукупність уявлень про причини і механізм розвитку синовіїту, тобто його етіопатогенез. Запропоновано нову класифікацію синовіїту: передсиновіїт, тобто ізольований м’язово-тонічний синдром напівперетинчастого та медіального литкового м’язів, ексудативний синовіїт або власне синовіїт (поява рідини в суглобі на тлі м’язово-тонічного синдрому); Среди разнообразных проявлений хронических заболеваний коленного сустава ведущее место занимает синовиит, частота встречаемости которого в последние годы неуклонно растет, составляя от 9 до 33% патологии коленного сустава. Цель. Анализ причин возникновения неспецифического синовиита коленного сустава на основе оценки структурных изменений мышечно-сухожильного комплекса. Методы. Изучены ультрасонографические данные за период 08/09 2016 г., 461 пациентов в возрасте от 13 до 80 лет с жалобами на боль в КС и подколенной ямке, ограничение движений. Результаты. Для анализа выделено 2 группы пациентов. 1 группа – 28 пациентов с утолщением диаметра сухожилия мышцы semimembranosus более чем 4 мм в месте дистальной инсерции, наличием линейных гиперэхогенных участков в структуре мышечной порции на уровне средней и нижней трети бедра. 2 группа – 433 пациентов с наличием анэхогенного участка жидкости в супрапателярном завороте между листками синовиальной оболочки. Выводы. Обоснована совокупность представлений о причине и механизме развития синовиита, т. е. его этиопатогенез. Предложена новая классификация синовиита: предсиновиит, т. е. изолированный мышечно-тонический синдром полуперепончатой и медиальной икроножной мышц, экссудативный синовиит или собственно синовиит (появление жидкости в суставе на фоне мышечно-тонического синдрома); According to the statistics of the World Health Organization joint diseases are on the third place among the most common illnesses following only vascular and digestive system disorders, and the frequency of this pathology fluctuates from 30 to 55% of all orthopedic diseases. It is not a coincidence that the beginning of the 21st century has been defined by the WHO as «the decade of fighting with joint diseases». It has been noticed that joint diseases «became younger»: young people are facing joint disorders more frequently nowadays. Synovitis occupies leading place among various manifestations of chronic diseases of the knee joint, frequency of occurrence of which has been steadily increasing for recent years, ranging from 9 to 33% of knee joint pathology. The aim of the work is to analysis ultrasound evidence of structural changes in the muscle-tendon complex of muscles of Semi group and gastrocnemius muscles in combination with US symptoms of the knee joint synovitis. Methods. There were studied the ultrasonographic data of 461 patients aged from 13 to 80 years with complaints of pain in the KJ and in the popliteal fossa, with restriction of movements during the period of 08/09 2016. All the patients had US of both knee joints by standard methods on the device by «BK Medical» company with linear sensor with the frequency of 12,0 MGz. The front, back and side areas of the knee joint were scanned in direct and lateral projection. Complaints about pain location in suprapatellar zone, on back sides of thigh and shin were taken into consideration. The following US evidence was considered: in the front projection: patellar bursa, the patella contours; the condition of adipose bodies and meniscus, joint areas of femoral bone and tibia; in the back projection: the structure of semitendinosus, semimembranous and gracilis muscles; the thickness of semimembranosus muscle in the place of distal insertion; the contours of hyaline cartilage. Two groups of patients with complaints about different pain localization, clinical trial (palpation) data and appropriate US vision of the knee joint and adjacent muscles were taken for the examination. The 1st group was patients with pain located only in the back area of a thigh, popliteal space and a shin. Clinically the patients have isolated pain by palpation of the back area of a thigh and a shin only, the tension of appropriate muscles, minor disorders of the flexor-extensor functions of the knee joint without clinical evidence of synovitis. The 2nd group was patients with complaints about pain in the back area of the knee joint, similar to the 1st group, as well as pain in the front area of a thigh and knee joint. During clinical study pain is detected by palpation of muscle portion and tendon of the 4th major thigh muscle, edema and fluctuations symptom in the area of suprapatellar pouch, which are the clinical symptoms of synovitis. Results. There were allocated 2 groups of patients for an analysis. The 1st group was 28 patients with thickening of the tendon diameter of the m. semimembranosus of more than 4 mm in the place of distal insertion, with presence of the linear hyperechogenic areas in the structure of muscular portion at the level of the middle and low third of the thigh. The 2nd group was 433 patients with the presence of unechogenic area in the fluid in the suprapattelar bend between the leaves of synovial membrane. In our opinion, the evidence of muscular tonic syndrome of semimembranous muscle and medial gastrocnemius muscle in the 1st group combined with complaints about pain in the back area and pain at palpation in 100% cases. In the 2nd group the appearance of US evidence of synovitis with different level of expression combined with the evidence of muscular tonic syndrome in the back area. Conclusions. It is substantiated a set of ideas about the causes and mechanisms of development of synovitis, i. e. its ethiopathogenesis. It is suggested a new classification of synovitis: preliminary sinovitis, i. e. spasm of the semimembranous and medial gastrocnemius muscles, tendonitis, exudative synovitis or actual synovitis itself, (musculo-tonic syndrome are present). The study resonates the combination of ideas about cause and development mechanism of synovitis, i. e. its etiopathogenesis. In the issue, the main point of the development process of nonspecific synovitis can be represented stepwise: muscular tonic syndrome (hyperechoic areas in muscle structure) – tendinitis (tendons thickening) – synovitis (fluid in a joint). Understanding the causes of synovitis appearance, which is based on etiology of the process, determines the necessity of changes in everyday treatment practice with the account on development phases of synovitis.
