V.Вивчення хірургічної анатомії вегетативних вузлів голови являється складовою частиною дослідження нерво¬вих джерел голови та їх розповсюдження на певні ділянки. Вегетативні парасимпатичні вузли голови, які розташовані за ходом гілок трійчастого нерва взагалі розглядаються як складні периферичні нервові центри, котрі пов 'язані з центральною нервовою системою. В клінічних умовах зустрічаються запалення вегетативних вузлів голови, які носять назву гангліонітів. Варіанти топографії вегетативних вузлів голови у значній мірі залежать від особливостей зв'язків з іншими вегетативними вузлами голови. Це пов'язано з тим, що вегетативні ганглії, нерви та сплетення розташовані на великій площі і функціонально пов'язані між собою та з верхнім шийним симпатичним вузлом. У людей похилого і старечого віку дуже часто зустрічаються захворювання трійчастого нерва та вегетативних вузлів голови. Але надання суттєвої допомоги не може бути ефективним без детального вивчення їх взаєморозташування. Изучение хирургической анатомии вегетативных узлов головы является составной частью исследования не¬рвных источников головы и их распространения на определенные участки. Вегетативные парасимпатические узлы голо-вы, которые расположены по ходу ветвей тройничного нерва вообще рассматриваются как сложные периферические нервные центры, связанные с центральной нервной системой. В клинических условиях встречаются воспаления вегета¬тивных узлов головы, которые носят название ганглионит. Варианты топографии вегетативных узлов головы в значитель¬ной степени зависят от особенностей связей с другими вегетативными узлами головы. Это связано с тем, что вегетатив¬ные ганглии, нервы и сплетения расположены на большой площади и функционально связаны между собой и с верхним шейным симпатичным узлом. У лиц пожилого и старческого возраста очень часто встречаются заболевания тройничного нерва и вегетативных узлов головы. Но предоставление существенной помощи не может быть эффективным без деталь¬ного изучения их взаиморасположения.The study of surgical anatomy of vegetative nodes of the head is an integral part of the study of the nerve sources of the head and their spread to certain areas. Vegetative parasympathetic nodules of the head, which are located along the course of branches of the trigeminal nerve, are generally considered as complex peripheral nerve centers associated with the central nervous system. In clinical conditions inflammation of vegetative nodes of the head, which are called ganglionitis. Variants of topography of vegetative knots of the head to a large extent depend on the features of the links with other vegetative nodes of the head. This is due to the fact that autonomic ganglia, nerves and plexus are located on a large area and functionally interconnected with the upper cervical sympathetic knot. In people of elderly age and aged people, diseases of the trigeminal nerve and vegetative nodes of the head are very common. M.В результаті проведених нами досліджень слід вказати, що існує два крайніх положення вушного вегетативного вузла: заднє верхнє і переднє нижнє. При цьому для осіб з брахіцефалічною формою мозкового відділу голови та хемепрозопічною формою обличчя характерне заднє верхнє положення вузла відносно овального отвору і нижньощелепного нерва. Переднє нижнє положеня відносно овального отвору і нижньощелепного нерва характерне для осіб з доліхоцефалічною формою голови та лептопрозопічною формою обличчя. Найбільш частим варіантом зовнішньої будови вузла є округла форма. Значно рідше зустрічається веретеноподібна та трикутна форми, а сіткоподібна – рідко. При цьому крайніми формами будови вушного вузла є компактна та дифузна форми будови. Що має велике значення для визначення топогафо-анатомічних особливостей розташування вегетативного вушного нервового вузла, також діагносики та лікування ганглионітів. В результате проведенных нами исследований следует указать, что существует два крайних положения ушного вегетативного узла: заднее верхнее и переднее нижнее. При этом для лиц с брахицефа-лической формой мозгового отдела головы и хамепрозопической формой лица характерно заднее верхнее положение узла относительно овального отверстия и нижнечелюстного нерва. Переднее нижнее положение относительно овального отверстия и нижнечелюстного нерва характерно для лиц с долихоцефалической формой мозгового отдела головы и лептопрозопической формой лицевого отдела. Наиболее частым вариантом внешнего строения узла является круглая форма. Значительно реже встречается веретенообразная и треугольная формы, а сетевидная – очень редко. При этом крайними формами строения ушного узла являются компактная и диффузная формы cтроения. Что имеет большое значение для определения топографоанатомических особенностей расположения вегетативного ушного нервного узла, а также диагностики и лечения ганглионитов.The autonomic nervous nodes of head and neck: pterygopalatine, ear, submandibular and hypoglossal nodes play an important role in the innervation of the mucous membranes of the nasal and mouth cavities and of salivary glands.In the clinical conditions you can find inflammations of head and neck vegetative nodes, which have nosology names of ganglions. The possible origins of ear nerve node pathologies are largely dependent on the characteristics of its connections with other head and neck vegetative nodes.It’s common enough in clinical practice that elderly individuals have the disease of branches of branches of the trigeminal nerve.The diversity and complexity of clinical manifestations usually complicate the diagnosis, which requires the examination of patients at neurologists’, dentists’, ENT specialists’ and other specialists for further diagnosis and treatment of the treatment of the pathological process.The difficulty of the symptoms largely depends on the existing relations between the autonomic ganglions, including the ear ganglions as well.Determine the location of the ear node, on the basis of the skull form and the variability of its shape, considering various skull shapes. For the research were used the biopharmaceuticals of deaths people of 56-77 years. The number of investigated biopharmaceuticals of ear nodes is 72 units. The following methods were used: anthropometric method, macro- and microscopy method, demi-thin serial dissections, statistical methods of data processing.With the method of macro-microscopic dissections of Vladimir Vorobyov and semi-thin serial dissections, we determined the shape and size of the ear unit.The correlation between the shape and size of the ear unit was found out. Thus, the following shapes were observed on the biopharmaceuticals: fusiform, triangular, oval, rounded and complex shape of the ear. Among these shapes the fusiform was identified for 24 cases, triangular for 12 cases, rounded for 34 cases and complex for only 2 cases. Despite the diversity of the ear node shapes in the examined cases, all of them were flattened in the sagittal plane.There are two positions of the ear vegetative node: rear upper and lower front. For the persons with cerebral brachycephalic form of skull characterizes with rear upper position from the oval foramen and the mandibular nerve. The front lower provisions regarding from the oval foramen and the mandibular nerve is typical for people with dolichocephalic and leptoprosopic shapes of face.In most of the cases, the external structure of the node is rounded. Less common are spindle and triangular shapes. Reticle form is extremely rare. Extremely rare forms of the ear node structure are compact and diffuse ones.The identified features of forms, the size and the location of the ear node are of practical importance in the clinical practice of the following specialists: neurologists, dentists and otorhinolaryngologists in determining of the topographic anatomical features, of the location and treatment of the nodes in the maxillofacial area, as well as the variability of the nerve relations with other nerves and nodes.We will continue to study the dependencies on the features of forms, the size and the location of the ear node. Yu.За допомогою загальноклінічних та спеціальних методів діагностики проведено дослідження захворюваності на гангліоніти у щелепно – лицевій ділянці, визначена ефективність проведеного лікування гангліонтів з урахуванням особливостей розташування вузлів у залежності від типу будови черепа. В результаті проведених досліджень виявлено, що раціональне лікування гангліонітів залежить від правильного проведення блокади, із урахуванням локалізації вегетативних вузлів, що пов’язано з типом будови черепа. Застосування цієї методики дозволяє досягти кращих результатів в лікуванні запалень вегетативних вузлів голови; С помощью общеклинических и специальных методов диагностики проведено исследование заболеваемости ганглионитами в челюстно-лицевой области. Определена эффективность проведенного лечения ганглионитов с учетом особенностей расположения узлов в зависимости от формы черепа. В результате проведенных исследований выявлено, что рациональное лечение ганглионитов зависит от правильно проведеной блокады, с учетом локализации вегетативных узлов в зависимости от типа строения черепа. Применение этой методики позволяет достичь лучших результатов в лечении заболеваний вегетативных узлов головы; Autonomic ganglions of the head, i.e., sphenopalatine, otic, submandibular and sublingual, are crucial in providing of innervations of mucous membrane of nasal and oral cavities, salivary glands. In the clinical conditions inflammations of autonomic ganglions of the head, called ganglionitis, occur. Clinical course of inflammation greatly depends on specific features of topography of this or that ganglion and availability of its interactions with other autonomic ganglions of the head. Disorders of the autonomic ganglions of the head account for 75% of cases among existing pathological syndromes of the face. The clinical picture of autonomic ganglionitis is different. It is reasoned by the fact that autonomic ganglions, nerves and plexuses are located on the large area and functionally interconnected with each other and superior cervical sympathetic ganglion. In ganglionitis of autonomic ganglions of the head the pain is often of the paroxysmal nature: attacks were observed during hours and sometimes even weeks. Specifically, the pain wave goes beyond the borders of areas of anatomical innervations of trigeminal nerve branches. The affection of autonomic ganglion of the head is often manifested by the edemas of facial soft tissues, mucous membrane of tongue and oral mucosa. The study revealed specific availability of typical secretory impairments: in affection of sphenopalatine ganglion lacrimation, drooling and rhinorrhea was observed and in affection of otic, submandibular and sublingual ganglions hypersalivation was typical. The blockade was carried out according to the ganglionitis location, type of skull structure of the patient and probable topographic anatomical ganglion. In treatment of sphenopalatine ganglion the most effective is the modified way of Vaisblat’s infratemporal-sphenopalatine anaesthesia in sphenopalatine fossa. Modification consists in extra tilt of needle up or down by 10°, stipulating by the type of scull of a patient and location of sphenopalatine ganglion in sphenopalatine fossa. According to Rybalko T.I., in people with brachycephalic type of scull the ganglion is located closer to posterior wall of the fossa and, consequently, it is more precise to introduce the needle into the superior parts. In individuals with dolichocephalic shape of the head the ganglion is located closer to anterior wall; therefore it is more precise to introduce the needle into the inferior parts of sphenopalatine fossa. During the introduction of the needle the main focus was the lateral lamina of pterygoid process, and point of injection was the median tragoorbital line. As for the otic autonomic ganglion, it has two locations relative to oval lumen and submabdibular nerve. In individuals with dolichocephalic shape of the head and chemoprosophilic face form, otic ganglion is of posterosuperior location; in dolichocephalic people with leptoprosophilic face form it is of anteroinferior location. Therefore, to introduce the needle with maximal precision to the otic autonomic ganglion, the modification of Vaisblat’s central anaesthesia of mandibular nerve in infratemporal fossa should be applied. The modification consists in extra tilt of needle up or down by 10° according to the type of scull of a patient and ganglion location. In affection of sublingual and submandibular autonomic ganglions the injection was made at the level of last molar under mucosa of sublingual area into the deepest part of mandibulolingual groove, as the ganglion is located on the lateral surface of sublingual salivary gland above the lingual nerve. In treatment of ganglionitis the efficacy was higher when the type of scull shape was considered. Consequently, the effective, rational treatment of ganglionitis depends on correct performance of blockade, taking into account the localization of autonomic ganglions according to the scull shape. Application of such technique benefits to achieving the better success in treatment of impairments of autonomic ganglions of the head.
