Документ The effect of α-lipoic acid on the functional state of peripheral nerve fibers in polyneuropathy of the lower extremities in patients with chronic hemoblastosis(Aluna Publishing, 2020) Piddubna, O. A.; Піддубна, Олександра Олегівна; Литвиненко, Наталія Володимирівна; Кривчун, Анжеліна Михайлівна; Таряник, Катерина АнатоліївнаThe aim: To evaluate the effectiveness of the appointment of α- lipoic acid for correcting electroneuromyographyс indicators polyneuropathy of the lower extremities in patients with hemoblastosis. Materials and methods: The study included 77 patients from hematology department of Poltava Regional Hospital, who were divided into 3 groups according to the underlying disease: Group 1 - patients with chronic myeloid leukemia (CML) (n = 26 ) groups and 2 - patients with chronic lymphocytic leukemia (CLL) (n = 27 ), group 3 - patients with multiple myeloma (MM) ( n = 24 ). For the examined patients were taken electroneuromyographyc researches of distal lower extremities by measuring the following parameters: the amplitude of the M-response from short extensors nerves of the fingers ( MEDB) and abductor of the big toe ( MAH) and the rate of excitation the spread of short extensors nerve of the fingers ( VEDB), thumb abductor ( VAH), superficial shallow ( VSF) and calf ( VC) nerves on both sides (sin/dex ) before and after administration of α- lipoic acid. Results: ENMG examination of the peripheral nerves of the lower extremities in patients with CML and CLL revealed a slight decrease in M-responses at normal values of the rate of conduction of excitation through sensory and motor fibers. In patients with MM, multiple lesions of peripheral nerve fibers of the lower extremities were detected, which is represented by a decrease in the M-response and the rate of excitation through sensory fibers and indicates deeper lesions, namely the axial cylinder, which requires longer treatment. The difference in the dynamics of the studied indicators indicates a different degree of PNP. Administration of α-lipoic acid increased the M-response rate and the rate of excitation of motor fibers in all groups of patients. The rate of excitation through sensory fibers was improved in patients with CLL and MM. Following the use of α-lipoic acid in the complex treatment of patients with MM ENMG, the signs of peripheral nerve fiber damage in the lower extremities were worse due to the severe initial condition, but reached a medium level. The positive effect of α-lipoic acid can be caused by a decrease in oxidative stress and endothelial dysfunction in nerve vessels, and, as a consequence, an improvement in the trophism and functional state of nerve fibers. Conclusions: ENMG signs of defeat of PNP of the lower extremities are more expressed at MM. The appointment of α-lipoic acid in the complex treatment of PNP in patients with chronic hemoblastosis improves the functional state of the peripheral nerve fibers of the lower extremities and may be a component of pathogenetic therapy.Документ Корекція немоторних флуктуацій у хворих із хворобою паркінсона на фоні лікування препаратами леводопи(Вищий державний навчальний заклад України «Українська медична стоматологічна академія», 2013) Таряник, Катерина Анатоліївна; Литвиненко, Наталія Володимирівна; Таряник, Екатерина Анатольевна; Литвиненко, Наталья Владимировна; Taryanyk, K. A.; Lytvynenko, N. V.При тривалому використанні препаратів леводопи виникають ускладнення у вигляді моторних та немоторних флуктуацій. Досить часто немоторні прояви являються першими передвісниками початку хвороби та домінують у клінічній картині захворювання. Метою нашої роботи було вивчити вплив агоністів дофамінових рецепторів та амантадинів на перебіг немоторних флуктуацій у хворих із хворобою Паркінсона (ХП) на фоні леводопатерапії. Обстежено 30 хворих із акінетико- ригідною формою хвороби Паркінсона, які знаходились на обстеженні та лікуванні у неврологічному відділенні Полтавської обласної клінічної лікарні ім. М.В. Скліфосовського та центрі паркінсонізму на базі кафедри нервових хвороб із нейрохірургією та медичною генетикою ВДНЗУ «Українська медична стоматологічна академія». Хворих розподілили на групи: 1-а група (10 чоловік) – тривалість за- хворювання 6,5± 1,2 роки - отримували лікування сульфатом амантадину 500 мл внутрішньовенно краплинно на протязі 5 днів, а потім – перорально по 100мг 2 рази на добу після їжі; 2-а група (10 чоловік), тривалість захворювання 5,2± 1,4 роки – праміпексол пролонгованої дії в дозі 1,5 мг на до- бу, 3-я група (10 чоловік), тривалість захворювання 4,9± 1,4 роки – отримувала сульфат амантадину перорально у дозі 200 мг на добу. На фоні проведеного лікування відмічено покращення когнитивних функцій, зменшення вираженості вегетативних флуктуацій та сенсорних порушень у пац єнтів всіх трьох груп, але здебільшого у пацієнтів першої групи. У всіх пацієнтів відмічено поліпшення показників якості життя за шкалою PDQ-39. Наявність немоторних флуктуацій корелює з важкістю та тривалістю перебігу захворювання. З метою корекції немоторних флуктуацій, а також профілактики побічних ефектів леводопатерапії, необхідно додавати до лікування агоністи дофамінових рецепторів та амантадину сульфат; При длительном использовании препаратов леводопы возникают осложнения в виде моторных и немоторных флуктуаций. Целью нашей работы было изучить влияние агонистов дофаминовых рецепторов и амантадин на ход немоторных флуктуаций у больных с болезнью Паркинсона (БП) на фоне леводопатерапии. Обследовано 30 больных с акинетико - ригидной форме болезни Паркинсона, которые находились на обследовании и лечении в неврологическом отделении Полтавской областной клинической больницы им . М.В. Склифосовского и центре паркинсонизма на базе кафедры нервных болезней с нейрохирургии и медицинской генетики ВДНЗУ «Украинская медицинская стоматологическая академия». Больных разделили на группы: первая группа (10 человек) - длительность заболевания 6,5 ± 1,2 года - получали лечение сульфатом амантадина 500 мл внутривенно капельно в течение 5 дней, а затем - внутрь по 100 мг 2 раза в сутки после еды ; вторая группа (10 человек), длительность заболевания 5,2 ± 1,4 года - прамипексол пролонгированного действия в дозе 1,5 мг в сут- ки, третья группа (10 человек), длительность заболевания 4,9 ± 1, 4 года - получала сульфат амантадина перорально в дозе 200 мг в сутки. На фоне проводимого лечения отмечено улучшение когнитивных функций, уменьшение выраженности вегетативных флуктуаций и сенсорных нарушений у пациентов всех трех групп, но в основном у пациентов первой группы. У всех пациентов отмечено улучшение показателей качества жизни по шкале PDQ -39. Можно сделать вывод о том, что наличие немоторных флуктуаций коррелирует с тяжестью и длительностью течения заболевания. С целью коррекции немоторных флуктуаций, а также профилактики побочных эффектов леводопатерапии необходимо добавлять к лечению агонисты дофаминовых рецепторов и амантадина сульфат; Prolonged levodopa therapy may result in some side effects presented by motor and non-motor fluctuations. Parkinson's disease, in addition to motor disorders, is characterized by a wide range of non-motor manifestations that appear in each patient regardless of the age and the stage of the disease. Frequent nonmotor signs are the first harbingers of early disease and dominate the clinical picture. The aim of our study was to examine the effect of dopamine receptor agonists and amantadine in the course of non-motor fluctuations in patients with Parkinson's disease (PD) during the levodopa therapy. The study involved 30 patients with akinetic-rigidity form of Parkinson's disease who were on the examination and treatment of the neurological department of the Poltava Regional Clinical Hospital M.V Sklifosofskiy and Parkinson Center at the Department of neurology with neurosurgery and medical genetics "Ukrainian Medical Stomatological Academy". The average age of patients was 58,4 ± 6,01 years. The average disease duration - 6,8 ± 1,5 years. Stage of disease was assessed on a scale Henn and Yahr . To assess cognitive disorders was used brief rating scale mental status (Mini Mental State Examination - MMSE), Beck Scale . Assessment of vegetative state patients was conducted by questionnaire to assess autonomic functions . Quality of life of patients was determined by the PDQ- 39 scale. All patients received long-term levadopa therapy 4 times a day (daily dose of levodopa 250- 375mh ). The degree of severity of CP on a scale Henn and Yahr was 3,5 ± 0,5. Patients were divided into groups: the first group (10 people) - disease duration 6,5 ± 1,2 years - treated with amantadine sulphate 500 ml intravenously for 5 days, and then - 100 mg orally 2 times a day after meals, the second group (10 persons), disease duration 5,2 ± 1,4 years - pramipexole extended release dose of 1.5 mg per day, the third group (10 persons), disease duration 4,9 ± 1, 4 years - received amantadine sulfate orally at a dose of 200 mg per day. During the therapy we marked improvement in cognitive function, the reduction in the severity of autonomic fluctuations and sensory disorders in patients of all three groups, but mostly in patients of the first group. All patients noted the improvement in quality of life by scale PDQ-39. Thus, we can conclude that the presence of non-motor fluctuations is correlated with the severity and duration of Parkinson’s disease. The first group of patients with the disease lasting for 6 years demonstrates marked disturbances of autonomic, mental, sensory areas, but these symptoms are well subjected to correction by amantadine sulphate. More pronounced effect is observed in the group of patients primary treated with intravenous forms of amantadine sulphate. Partial improvement has been registered on the 5th day and continues a month since the start of the therapy. During the treatment we noticed marked improvement of cognitive function, the reduction in the severity of autonomic fluctuations and sensory disorders in the patients of all three groups, but mostly in patients of the first group. All patients noted the improvement in quality of life scale PDQ-39. To correct nonmotor fluctuations and to prevent side effects produced by levodopa therapy it is advasible to introduce dopamine receptor agonists and amantadine sulphate into the therapy.
