М.; Савченко, Леся ВолодимирівнаThe fourth wave of the SARS-CoV-2 virus pandemic was characterized by an incidence increase of the disease in all ages children. In most cases, the disease course in children was much easier than in adults. However, an increase in the cases number of children with coronavirus showed that some pediatric patients with SARS-CoV-2 infection developed a nonspecific hyperinflammatory syndrome. According to most observations, a multisystem inflammatory syndrome in children (MIS-C) usually occurs 2-6 weeks after coronavirus disease in previously healthy children and adolescents. It is characterized by clinical and laboratory evidence of multiorgan inflammation. The article presents a clinical case of a pediatric multisystem inflammatory syndrome associated with coronavirus infection in school-age children. The patient developed a gradual MIS-C development after asymptomatic coronavirus infection. The disease onset was characterized by nonspecific symptoms resembling the clinic of a respiratory infection. In the dynamics of the MIS-C formation, there was a lesion of the respiratory, cardiovascular, hepatobiliary systems, and skin, with the development of poly-organ failure syndrome. На час первинного специфічного обстеження у хворої визначалося ураження органів дихання, затримка фізичного розвитку та було виявлено наявність об’ємного утворення правої здухвинної ділянки із залученням в процес петель кишківника, що потребувало проведення диференціальної діагностики з новоутворенням. Отримання позитивних результатів на муковісцидоз обґрунтувало призначення специфічного лікування. В результаті лікування стан дитини покращився. Дані УЗД органів черевної порожнини в динаміці засвідчили відсутність утворення в кишківнику через 10 днів специфічного лікування. Наведений клінічний випадок свідчить про те, що настороженість лікарів стосовно муковісцидозу не повинна обмежуватися періодом новонародженості та перших років життя дитини. Поєднання патології органів дихання, шлунково-кишкового тракту, затримки фізичного розвитку є приводом до проведення діагностичних заходів у пацієнтів більш старшого віку; Резюме. В статье представлен клинический случай первичной диагностики муковисцидоза у ребенка 10 лет. На время первичного специфического обследования у больной определялось поражения органов дыхания, задержка физического развития и было выявлено наличие объемного образования правой подвздошной области с вовлечением в процесс петель кишечника, что требовало дифференциальной диагностики с новообразованием. Получение положительных результатов на муковисцидоз обосновало назначения специфического лечения в соответствии, в результате которого состояние ребенка улучшилось. Данные УЗИ органов брюшной полости в динамике показали отсутствие образования в кишечнике через 10 дней специфического лечения. Данный клинический случай свидетельствует о том, что настороженность врачей относительно муковисцидоза не должна ограничиваться периодом новорожденности и первых лет жизни ребенка. Сочетание патологии органов дыхания, желудочно-кишечного тракта, задержки физического развития является поводом к проведению диагностических мероприятий у пациентов более старшего возраста; Abstract. Cystic fibrosis is a hereditary multisystem exocrine pathology with an autosomal recessive type of inheritance, characterized by secretion of increased viscosity, which causes obstruction of the excretion ducts, with subsequent development of secondary progressive pathological changes (functional deficiency, fibrosis) of organs, especially lungs and gastrointestinal tract. According to WHO, this disease is the most widespread hereditary pathology. The article presents a clinical case of primary diagnosis of cystic fibrosis in a child of school age. The peculiarity of this study consists in the fact that this disease is most often diagnosed in newborn babies or at an early age. Conducting diagnostic tests for cystic fibrosis in children of school age is not a priority in the doctor’s practice. The present clinical observation is of interest because in a 10-year-old child at the time of the primary specific examination for cystic fibrosis, the presence of a heterogeneous space-occupying formation of the right ileal area sized 44×44×55 mm with the involvement of intestinal loops was detected, which required differential diagnostics with neoplasm. Obtaining positive results in cystic fibrosis testing justified the prescription of a specific treatment in accordance with the unified clinical protocol (Order of the Ministry of Public Health of Ukraine No. 723 as of July 15, 2016), which included a diet, pancreatic enzymes, antibiotics, mucolytics, ursodeoxycholic acid, symptomatic treatment. As a result of treatment, the child’s condition improved significantly, positive dynamics in the body weight, local manifestations of pulmonary lesion, and normalization of defecation has been observed. The ultrasound data of the abdominal cavity in the dynamics indicated the absence of a formation in the intestine after 10 days of specific treatment. In our opinion, such a formation in the intestinal loops is not specific for cystic fibrosis due to the viscous secretion of the gastrointestinal tract glands. Thus, our clinical case of primary diagnosis of mucoviscidosis in a 10-years-old child, suggests that doctors’ alertness for cystic fibrosis should not be limited to the period of infancy. The combination of pathology of the respiratory system, gastrointestinal tract, and delayed physical development gives ground for diagnostic measures in older patients.
