Патогенетично обґрунтовані підходи при лікуванні запальних захворювань придатків матки

Abstract

Зятіна О.М. Патогенетично обґрунтовані підходи при лікуванні запальних захворювань придатків матки. – Рукопис. Дисертація на здобуття наукового ступеня кандидата медичних наук за спеціальністю 14.01.01 – акушерство та гінекологія. – Харківський державний медичний університет МОЗ України, Харків, 2007. Дисертація присвячена диференційованому підходу при діагностиці та лікуванні гострих і хронічних запальних захворювань придатків матки. Автором проведено комплексний аналіз особливостей перебігу запальних захворювань придатків матки, вивчення клінічних, біохімічних та місцевих гемодинамічних змін у 140 хворих і у 21 практично здорової жінки, а також розробка та впровадження у клініку патогенетично обґрунтованої методики лікування, що дозволило уникнути значного медикаментозного навантаження на організм та сприяло покращенню місцевої гемодинаміки. На основі запропонованого патогенетично обґрунтованого лікування відбулись: нормалізація локального кровообігу (підвищення пульсового кровонаповнення у 1,8-3 рази, посилення венозного відтоку у 2,5-3 рази, зменшення резистентності реґіонарних артерій у 1,2-1,3 рази, усунення ознак варикозу); зменшення активності запалення (зниження вмісту серомукоїдів у 1,6-2,4 рази); при хронічному перебігу – підвищення вмісту основного ферменту антиоксидантного захисту церулоплазміну у 1,5 рази) та зменшення загального ендотоксикозу (зниження лейкоцитарного індексу інтоксикації у 2 рази), що сприяло швидшому усуненню больового синдрому, більш якісному відновленню репродуктивної функції і зменшенню ліжко днів на 3 дні.

ZYATINA O.M. The Pathogenetic grounded approaches during the medical treatment of the Inflammatory diseases of uterine appendages. – Manuscript. The Thesis describes the differentiated approach during the diagnostics of and medical treatment of the acute and chronic inflammatory diseases of uterine appendages. The author has made the complex analysis of the peculiarities of the inflammatory diseases of uterine appendages clinical course, the studying of the clinical, biochemical and local hemodynamic changes of 140 patients and 21 practically healthy women as well as the pathogenetic grounded methods of medical treatment had been worked out and introduced to the clinic that made possible to avoid the considerable medical loading for the organism and promoted the improvement of the local hemodynamics. On the basis of the proposed pathogenetic grounded medical treatment once had the normalization of the local blood circulation (increasing of the pulse blood filling in 1,8-3 times; the increasing of the venous drainage in 2,5-3 times; decreasing of the regional arteries resistance in 1,2-1,3 times; the removal of the varicosity signs); the decreasing of the inflammation activity (decreasing of the sulfurmucoid content in 1,6-2,4 times); during the chronic clinical course – the increasing of the content of the basic ferment of the antioxidant protection of the ceruloplasmin in 1,5 times) and decreasing of the general endotoxicosis (the decreasing of the leukocytes index of intoxication in 2 times) that promoted the quick removal of the painful syndrome and the decreasing of the confinement to bed for 3 days.
