Є.Given the polygamy of symptoms of rhinosinusitis, including pain in the frontal part of the head, it is sometimes difficult to differentiate the cause of the latter. Є.Rhinosinusitis is a widespread disease in most countries of the world, has an annual prevalence of 6-15% and is usually a consequence of Acute Respiratory Viral Infection. The purpose of our work was to study the clinical case of a patient with cluster headaches on the background of recurrent rhinosinusitis. The article presents a clinical case of cluster headache in a patient with recurrent rhinosinusitis, who, according to the treatment protocol, was prescribed an antibiotic, nasal rinsing with saline solutions, complex herbal preparations with decongestant, anti-inflammatory and secretolytic effects, analgesics and intranasal glucocorticoids. On the background of the prescribed treatment, inflammation of the mucous membrane of the nasal cavity and paranasal sinuses began to regress; however, the headache intensity did not change. Improvement in the patient’s general condition and clinical recovery was achieved only after consultation and treatment correction by a neurologist. This clinical case indicates that patients with rhinosinusitis, in which the nature of the pain is atypical, does not correspond to the severity of inflammation of the nasal mucosa and paranasal sinuses, and is not treatable, should consult related healthcare specialists to determine further treatment tactics.The analysis of clinical cases of patients with sinus pathology and the study of their structural features, innervation and blood supply significantly complement the teaching of human anatomical variability in age, gender and constitutional aspects.Документ Clinical thinking аs an important element of effective professional activity of the future medical worker(Полтавський державний медичний університет, 2023) Bobukh, V. For healthcare workers, the formation of professional competence is significant, a combination of a high level of theoretical knowledge, professional skills, practical skills, the creative potential of an individual and a culture of communication. A significant role in this is played by the ability of medical workers to think "clinically", to learn to express their own opinions, to analyze and think logically, and to react quickly and highly qualified in critical situations. The formation of clinical thinking begins already during training in a medical institution and is further improved during one's professional activity. In addition, various modern teaching methods play a significant role in forming clinical thinking. These include the case method, the business game method, the "brainstorming" method, as well as the use of situational tasks. All these methods aim to teach students to fulfil their professional duties with dignity and apply the acquired knowledge in practical activities. In addition, the combination of these methods contributes not only to the formation of clinical thinking but also has a direct impact on both creative and critical thinking since it is impossible to draw a clear line between these concepts, especially when it comes to medical workers. Б.One of the actual problems of modern otorhinolaryngology is a significant increase in the incidence of paranasal sinuses, in particular acute and chronic sinusitis, which, according to the literature, is 1.5-2% per year. The purpose of our research was to study and compare the frequency of isolated and combined sinusitis of the maxillary and frontal sinuses according to radiographic examination data. 100 anonymized radiographs of persons of mature age (50 men and 50 women) who were examined by referral for inflammatory lesions of the paranasal sinuses were analyzed. All images were taken in the occipito-mental projection according to Waters, which allowed visualization of the maxillary and frontal sinuses. Different variants of isolated or combined inflammatory lesions of the maxillary and frontal sinuses were found in the studied sample. Signs of one- or two-sided catarrhal or exudative sinusitis were observed in all processed Xrays. In 84% of the examined persons, a lesion of only the maxillary sinuses was diagnosed, in 16% of the examined, a combined lesion of the maxillary and frontal sinuses was detected. Not a single case of isolated frontitis was found in the studied sample. According to radiographic examination, inflammatory processes in the maxillary sinuses are observed 6 times more often than in the frontal sinuses. Isolated lesions of the maxillary sinuses are found 5 times more often than combined lesions of the maxillary and frontal sinuses.Документ Immunohistochemical characteristics of structural changes in the cortex and medulla of the adrenal glands under the influence of a food additive complex(Полтавський державний медичний університет, 2023) Donchenko, S. M.; Донченко, Світлана Владиславівна; Білаш, Сергій МихайловичThe data obtained during the experiment allow us to conclude that the influence of the complex of food addi tives sodium glutamate, sodium nitrite and Ponceau 4R is a mechanism of activation of the focus of inflammation and changes in the structural elements of the adrenal glands and hemomicrocirculatory bed. According to the data of an immunohistochemical study, it was established that in the cortex and medulla of the adrenal glands, Ki-67 positive cells are elements of the local protective barrier. The largest number of Ki 67-positive cells was determined in the walls of fenes trated capillaries that permeate the cortex and medulla of the adrenal glands. Ki-67-positive cells formed clusters of 2-10 cells at the border of the glomerular and reticular zones. The total number of Ki-67-positive cells varied ac cording to the week of the experimental study. Starting from the first week, the number of cells expressing Ki-67 increases dramatically in the adrenal cortex’s glomerular, fasciculate and reticular zones. In the fourth week of the experiment, the level of labeled cells with Ki-67 reached almost control values. However, at the 12th week, the number of cells expressing Ki-67 increases in the cortex and the medulla. On the 16th week, the indicators remain within the 12th week, with an increase in the number of Ki-67-positive cells in the fasciculate zone of the cortical substance of the adrenal glands.Документ Morphological changes in the arterioles of the haemomicrocirculatory bed of the gingival mucosa under prolonged exposure to food additives(Полтавський державний медичний університет, 2023) Oleksiienko, V. M.; Олексієнко, Владислав Віталійович; Білаш, Сергій МихайловичThis study investigates the effect of a complex of food additives, including monosodium glutamate, sodium nitrite and Ponceau 4R on the gums of rats. The study is aimed at identifying morphological changes in the walls of arterioles. The study was conducted on material from the gums of rats. Prepared sections were stained with such dyes as toluidine blue and polychrome. The main research methods were morphological, histological and static. The gums of rats in the control group were characterised by the presence of a mucous membrane without a submucosal base, being fused to its own lamina. The first layer was formed by narrow and high papillae, which seemed to grow into the epithelial layer of the mucosa. The next layer, the epithelial layer, was lined with multilayered squamous non-cornified epithelium. Arterioles, as the main vessels of our study, were defined as large-diameter, rounded structures with a thick wall and 3 layers. We studied 3 main parameters: total diameter, lumen of the canal and thickness of the arterioles walls. Particular attention was paid to changes in these values and their interpretation. At week 1, there were mainly decreases in the values relative to the control group, which increased or returned to the control group values by week 4 of the study. Further, the values increased until week 16, but these values were slightly higher than those of the control group. This indicates an insufficient effect of recovery processes in the arterioles due to the powerful effect of nutritional supplements on vascular morphology, especially with prolonged exposure. With this study, we demonstrate that the effects of a complex of food additives are much more powerful than those of individual additives. Given that all foods contain a complex of additives, this experiment will be of interest to food industry workers, as well as doctors and scientists conducting research in this area.Документ State of the autonomous nervous system and stress at outpatient appointment of dental-surgeon's patients with syncopathic conditions(Полтавський державний медичний університет, 2023) Mamai, O. In clinical practice, a dental surgeon encounters a wide range of emergencies, including epileptic seizures, cardiac arrest, fainting (or syncope), and others. At the same time, syncope is often overlooked against the background of more severe and dangerous conditions. The aim is to assess the level of perceived stress and the state of the autonomic nervous system in outpatients of a dental surgeon at the beginning of the appointment, depending on the further development of syncope. We conducted a pilot prospective study that included 33 outpatients of a dental surgeon, who were divided into 2 groups: group 1 – patients who developed syncope (n=11); group 2 – patients who did not develop syncope during the visit to the clinic (n=22). During the medical history, patients were asked to complete a questionnaire to assess perceived stress (PSS-10). Every patient received a hemodynamic assessment that included measuring their blood pressure and pulse, determining the Kerdo Index, and examining their autonomic nervous system by checking for demographism. The amount of time spent in the queue was also documented. If a patient fainted, emergency care was immediately given.There were no significant differences in gender, age, diagnosis, or wait time between patients in both groups. There were no statistically significant differences between the groups of patients in terms of perceived stress. A statistically significant higher Kerdo index was found in group 1 with a tendency to sympathicotonia. Conclusions. Thus, we have determined that it is the increased activity of the sympathetic nervous system before an outpatient visit to a dental surgeon, rather than the subjective level of perceived stress, that may be potentially associated with the likelihood of developing syncope, which requires more detailed research. Although syncopal states are generally considered benign, frequent episodes of syncope can harm the patient’s quality of life, morbidity, and compliance with the dentist. Our taste buds are responsible for recognizing flavors, determining the suitability of foods, protecting against harmful substances, and regulating food intake. A person always gets pleasure from tasty food, and sweet dishes cause positive emotions. Unfortunately, we consume food that contains many food additives and preservatives daily, although we know that it can be harmful. Tasty but not healthy food has become extremely common and available, and it is difficult for us to resist the temptation to eat it. Many nutritional supplements have been created to meet the needs of the food industry, as mass production of products is different from home cooking. These additives are necessary to preserve the product's appearance but should not harm buyers' health or mislead them. Today, there are many food additives that consumers are unaware of but which can affect human organs, particularly the adrenal glands. Some of the most common food additives we study include monosodium glutamate (E621), sodium nitrite (E250) and Ponceau 4R (E124). Consuming large amounts of dietary supplements can have the following effects: chest and abdominal pain, migraines, nausea, rapid heart rate (tachycardia), obesity, weakness, and allergic reactions. Particular attention should be paid to the effect of these supplements on the adrenal glands, as they can disrupt the function of these organs and contribute to the development of oncological diseases. Research on the morphology of the adrenal glands has been ongoing for many years. In recent decades, there has been an increase in the pathology of the adrenal glands as the negative influence of external and internal factors increases, which leads to disturbances in the structure and functioning of the glands. Adrenal glands play an essential role not only in the hypothalamic-pituitary-adrenal system but also in the endocrine system. Interest in changes in the structure of the adrenal glands does not fade among experimental medicine and scientists. Thanks to the improvement of computer diagnostic methods, the detection of both hormonally active and inactive, benign and malignant neoplasms of the adrenal glands is increasing. The most common endocrine disorders are hyperaldosteronism, adrenergic and estrogenic balance disorders. Computed tomography significantly improves the detection of tumors of various origins in the adrenal glands. H.; Бєляєва, Олена Миколаївна; Лисанець, Юлія Валеріївна; Білаш, Сергій Михайлович; Скрипнікова, Таїса Петрівна; Хміль, Тетяна Андріївна; Сологор, Ірина Миколаївна; Гаврильєва, Ксеня ГригорівнаThe article is devoted to the phenomenon of academic ratings, which have now become one of the influential levers for determining the competitiveness of higher education institutions at the international, national, and regional levels, as well as a natural result of its mass and commercialized nature. The authors observe that during the entire period of existence of academic ratings, there has been an ardent discussion about their usefulness, objectivity, quality and validity. The paper discusses the arguments of the opponents of academic ratings, which emphasize that they are often biased or based on methodologies that are not transparent (weight of compared indicators, calculation algorithms, methodological validity in data collection, etc.). The authors argue that despite the constant and painstaking work of the compilers to make the ratings as transparent as possible, for now, they often do not meet the expectations of university management and the academic community, and also provide little useful information necessary for real and potential stakeholders. The analysis of the literature on the researched issue proved that this especially applies to representatives of European universities. Opponents of the ratings explain this status quo by the fundamental difference, on the one hand, between the British and American educational systems, and on the other, between the national educational systems of Europe. Attention is focused on the fact that the orientation of the ratings mainly on the bibliometric analysis of scientific publications of academic staff is, for example, disadvantageous for French universities, since a feature of the French higher education system (in contrast to the British or American) is that scientific activity is mainly concentrated in research centers, whereas educational activities – in universities. It is noted that the analysis of quality indicators of various areas of activity of a higher education institution and the educational system of the country as a whole makes it possible to obtain unbiased information about competitiveness in the field under study. In connection with this, the “struggle for one’s place in the sun” should be perceived as a reality that determines both the current state and the prospects for the further development of higher education.
