Therefore, the goal of our work was to provide doctors with diagnostic guidelines, which are summarized in modern literature, as well as a methodology for planning a diagnostic search. A timely diagnosis of axial spondyloarthritis, regardless of its subsequent clinical evolution, allows one to switch from symptomatic to specific basic treatment as early as possible. The combination of sacroiliitis, spondylitis, and generalized enthesitis in a patient with chronic back pain is highly suggestive of axial spondyloarthritis. A condition for timely diagnosis is the correct assessment of the inflammatory nature of back pain. Pain relief on the background of physical activity, aggravation at night, improvement after morning activation, indistinct onset, and age up to 40 years are considered. With a high probability, axial spondyloarthritis is diagnosed with long-term back pain of an inflammatory nature in young people in the presence of sacroiliitis or HLA-B27 positivity in combination with specific additional signs. It is essential that this diagnosis remains legitimate even in the absence of radiological changes. Any case of chronic back pain syndrome, especially in adolescence or early adulthood, in the presence of X-ray changes in the spine requires a differential diagnosis with axial spondyloarthritis. The separation of axial spondyloarthritis into an independent clinical form among the group of chronic inflammatory spondyloarthropathies is due to the evolution of the possibilities of modern early pathogenetically directed basic treatment, which often provides a dramatic improvement in the course of the disease and the functional state of patients, reduces the risks of exacerbation, hospitalization, and disability.Документ Non-steroidal anti-inflammatory drugs in the treatment of rheumatological patients with comorbidity(Полтавський державний медичний університет, 2022-06-11) Zhdan, V. М.The results of the analysis of the importance of the main groups of chronic diseases show that rheumatic diseases have the greatest contribution to reducing efficiency, deteriorating the general condition of the patient, and increasing the number of visits to the general practitioner. Pain is the most common symptom of rheumatological patients. Non-steroidal anti-inflammatory drugs are the most popular agent of analgesic therapy in medical practice due to the combination of efficacy, safety, availability, and simplicity of use. The aim of the study was to search and analyze literature data on the use of nonsteroidal anti-inflammatory drugs in rheumatological patients with comorbidity. Non-steroidal anti-inflammatory drugs quickly and effectively overcome acute pain syndromes and play the role of not only symptom-modifying but also pathogenetic therapy in the treatment of some chronic diseases (including rheumatological), slowing the progression of the pathological process. The variety of effects of certain nonsteroidal anti-inflammatory drugs on cartilaginous tissue metabolism is directly relevant to clinical practice, as any drugs that inhibit proteoglycan synthesis and impair chondrocyte repair can potentially accelerate cartilage degradation. Responses to treatment and toxic effects vary significantly at the individual level.Variability of efficacy between patients may be due to a number of factors, such as the mechanism of action of NSAIDs (predominant blockade of prostaglandins or other inflammatory mediators), the half-life, protein binding, and pharmacodynamic features. Although there is no strong correlation between the effectiveness of NSAIDs and the halflife/ plasma concentration: lower levels of albumin in synovial fluid cause slightly fewer NSAIDs in the joints, but the concentration of NSAIDs in the joint cavity may be stable over time.Given the presence of comorbidities in rheumatology patients, it is extremely important to evaluate the possible side effects of NSAIDs. First of all, it is recommended to assess the condition of the gastrointestinal tract and cardiovascular and excretory systems. It is important to remember that allergic reactions, hematological disorders (cytopenia), neurological disorders, etc. can occur sometimes during medication management. It is an autosomal recessive inherited disorder that attracts the close attention of scientists since the disease affects many organs and systems of a patient and can develop both in children and in adults. Wilson’s disease occurs with similar frequency throughout the world. It is caused by mutations in the ATP7B gene identified on the long arm of the 13th chromosome, as well as by the heterozygous carriage. The copper metabolism disorder determined by the genetic changes plays a major role in the development of Wilson’s disease. The excess copper accumulates in the liver parenchyma, nervous tissue and in the peripheral cornea (so-called Kayser-Fleischer rings) which results in subsequent organ damage. Since there is no unified examination that could confirm or rule out Wilson’s disease, it is necessary that doctors are able to define the symptoms of this disease. The early detection of symptoms and diagnostic studies can improve the disease prognosis. The aim of our research was to study the debut of Wilson’s disease in a 39-year-old patient with comorbid pathology. We also analyzed the difficulties of managing patients with Wilson’s disease in the example of this clinical case. We considered different mechanisms of the disease development and peculiarities of the diagnosis of atypical symptoms using international recommendations and protocols. The difficulty of our clinical case was that the symptoms of the disease with a comorbid pathology were atypical at the early stage of the illness. We did not detect damage to the nervous system, as well as ophthalmological features, and changes in the amount of free copper, ceruloplasmin levels and rate of 24-hour urinary copper excretion. It was proven that detection of these biochemical markers in blood and urine is important in ruling out Wilson’s disease in a patient with cirrhosis of the liver even if the other organs that were usually affected by copper were not damaged. Prompt diagnosis of the markers of Wilson’s disease and corresponding treatment can prevent the progression of the disease and its complications in such patients. V.В результаті аналізу встановлено, що демографічні тенденції і зрушення в бік старіння населення, сприяють зростанню розповсюдженості в суспільстві дегенеративних захворювань суглобів (остеоартроз) і хребта (спондилоартроз, остеохондроз). Залишається актуальною проблема постменопаузального і сенильного остеопорозу, як причини перелому шийки стегна, хребців і нижньої третини передпліччя. Значна частина цих осіб потребує професійної паліативної допомоги в спеціалізованих відділеннях онкологічних та багатопрофільних лікарень, окремих хоспісах та в інтернатах для людей похилого віку або вдома, у сім’ї, за участі професійних виїзних бригад лікарів, медичних сестер та психологів, як це робиться в багатьох країнах Заходу. Паліативні пацієнти та члени їх родин потребують адекватного знеболювання й симптоматичного лікування, професійного догляду, морально-психологічної та духовної підтримки, а часом просто людського співчуття та уваги. Відповідно до реального стану організації паліативної медицини в Україні доцільно вважати, що хоспісна медицина займається комплексом питань, пов’язаних із допомогою паліативним пацієнтам у термінальний період життя у спеціальних медико-соціальних установах – хоспісах або відділеннях паліативної та хоспісної допомоги лікувальних закладів. Запровадження паліативної допомоги повинно бути на ранньому етапі. Допомогу слід вводити після встановлення діагнозу. Люди літнього віку з коморбідною патологією страждають від болю більше, ніж представники інших вікових груп. Посилювати біль може малорухливий спосіб життя, що спричинює скутість м’язів, погіршення рухомості суглобів, посилення остеопорозу. У людей літнього віку спостерігають поєднання м’язово-скелетного болю, закрепів, інфекцій, нейропатії периферичних нервів, емоційних порушень, великих і малих травм та ін. Протибольову терапію слід розпочинати в разі появи перших ознак болю. Анальгетики призначають через однакові проміжки часу, не чекаючи відновлення болю, з метою забезпечити постійне знеболювання. Ад’ювантні і симптоматичні препарати необхідно призначати суворо за показаннями. За пацієнтами встановлюють постійний контроль, особливо на початку лікування. Надалі також потрібне ретельне спостереження для корекції лікування і профілактики можливих ускладнень. У паліативній медицині існують медикаментозні (фармакологічні) та немедикаментозні консервативні (застосовуються переважно при слабкому або помірному болю) й агресивні (нейрохірургічні, застосовуються при сильному та нестерпному болю) методи контролю хронічного больового синдрому. Згідно з рекомендаціями ВООЗ, існують три основних рівні знеболювання при хронічному больовому синдромі у паліативних хворих. У ревматологічних хворих виникнення болю зумовлене патологічними змінами в периферичних структурах (шкіра, м’язи, сухожилля, зв’язки, синовіальна оболонка, капсула суглоба, хрящ, кістки). Під час оцінки осоливостей больового синдрому слід враховувати анамнестичні дані, коморбідні стани, результати клініко-лабораторних методів обстеження та індивідуальні особливості пацієнта. Тільки такий комплексний підхід дозволяє розробити ефективний план лікувально-реабілітаційних заходів. Найпопулярнішим інструментом аналгетичної терапії в медичній практиці завдяки поєднанню ефективності, безпеки, доступності та зручності застосування є НПЗП. При хронічному болю середнього ступеня тяжкості або тяжкому в пацієнта з невиліковною формою захворювання застосовують опіоїди. Тобто медицина в силах позбавити пацієнта болісних страждань і дати йому можливість пройти свій останній шлях з гідністю. Для паліативного пацієнта важливо сформулювати зі своїм лікарем чіткий план дій. A.The increase in the prevalence of gout leads to an increase in the frequency of concomitant diseases associated with a persistent increase in the level of uric acid (UA), the degree of decompensation associated with the frequency of arthritis exacerbations. A complex cascade of events is formed the pathogenetic mechanisms of which are closely related to the progress of each of its components. Ukrainian and international recommendations define the standards for the treatment of gouty arthritis, which provide for the mandatory achievement of the target level of UA in the blood serum. The need for a steady decrease to the target level is justified by the desire to influence the process of UA dissolution, the violation of which leads to the formation of urate crystals with their subsequent deposition in the joint cavity and soft tissues, exacerbation of arthritis and an increase in the probability of an undesirable result. Hypouricemic therapy is a mandatory component of standard drug therapy for patients with gout. The literature provides data on the severe febuxostat advantages over other hypouricemic agents. This work aimed to determine the possibility of obtaining a clinical and laboratory effect in a short time after the start of febuxostat therapy (up to 3 months) in patients with gouty nephropathy who have concomitant pathology. Achieving normouricemia in a patient with gout and comorbidities is essential for controlling the course of gout, preventing new gout attacks, and reducing the threat to life and health caused by comorbidities. Analysis of the use of febuxostat in our patients demonstrated the achievement of target values or a significant reduction in UA levels within 3 months of therapy.
