Застосовувалися методики визначення показників, універсальні щодо віку дітей, критерієм об’єднання яких у основну групу був соматичний діагноз. У дітей із підтвердженим діагнозом муковісцидозу визначені поширеність карієсу зубів середнього рівня, висока схильність до виникнення захворювань тканин пародонта і низький рівень гігієни порожнини рота. Найбільш суттєві достовірні відмінності стоматологічного статусу у хворих дітей та дітей групи порівняння зафіксовані для гігієнічного індексу OHI- S і пародонтального індексу КПІ; Представлены результаты исследования распространенности, интенсивности кариеса зубов, индивидуального уровня интенсивности кариеса, состояния гигиены полости рта и тканей пародонта у детей, больных муковисцидозом. Применялись методики определения показателей, универсальных в отношении возраста пациентов, объединенных в основную группу наличием соматического заболевания. У детей с муковисцидозом установлены распространенность кариеса зубов среднего уровня, высокая подверженность заболеваниям тканей пародонта и низкий уровень гигиены полости рта. Наиболее существенные достоверные различия стоматологического статуса больных детей и детей группы сравнения выявлены для гигиенического индекса OHI- S и пародонтального индекса КПИ; The emergence of diseases of maxillofacial area in childhood is caused both by the local factors and the presence of physical illness. The overall impact of these factors is subject to constant research in order to solveт the problem of early diagnostics, adequate treatment and prevention of dental diseases. Today, much attention is paid to the studying of the rare, so-called orphan, diseases, one of which is cystic fibrosis. Cystic fibrosis is a genetic disorder that affects the respiratory and digestive systems. It is based on a mutation of the gene and causes an infringement of ion exchange of the epithelial cells in exocrine glands of the body. The result of cystic fibrosis is disturbance of the structure and function of a protein that serves as a channel for the transport of chloride ions, changing the properties of the secretions and starts to develop of secondary disorders with damage to many systems and organs. Violation of exocrine glands, including salivary ones, impaired nutritional status of patients, vitamin deficiencies, osteoporosis cause the necessity of permanent use of antibacterial, enzymatic, hormonal methods which affect the oral health in the disease treatment. The aim of our study is to determine the prevalence and intensity of caries, dental individual level of caries intensity, level of oral hygiene and periodontal tissue status of the children with cystic fibrosis. We’ve used the methods for determining indicators, universal for the age of children, who have the same somatic diagnosis. To achieve this goal we’ve conducted clinical dental examination of 17 children aged 1, 5 to 16 years old with a diagnosis of cystic fibrosis. The level of prevalence and intensity of caries has been evaluated by the number of teeth, which are sealed, decayed and removed. To determine the state of oral hygiene index OHI-S (J. C. Green, J.R.Wermillion, 1964) in its component of plaque has been used. The condition of periodontal tissues has been evaluated as well on the basis of determination of complex periodontal index CPI (individual) (P. A. Leus, 1988). The results of clinical examination have found that children with the diagnosis of cystic fibrosis are determined by the prevalence of dental caries on the average level, high susceptibility to periodontal tissue disease and low level of oral hygiene. The most significant differences of dental status between pediatric patients and a comparison group of children are registered for the hygienic OHI- S index and periodontal index CPI. The findings suggest the need of in-depth study of etiological, pathogenetic factors disturbing the dental status of the children with confirmed diagnosis, possible connection between clinical manifestations of the dental pathology and genotype, disease severity or form of cystic fibrosis. S.; Kryvenko, L. S.Наведені дані щодо комплексного аналізу стоматологічного статусу 126 дітей, хворих на бронхіальну астму, алергічний риніт і атопічний дерматит. Проведений аналіз рівня інтенсивності карієсу, стану тканин пародонта і гігієнічного статусу відповідно до наявної загальносоматичної патології та трьох вікових груп. Виявлено високу поширеність запальних захворювань пародонта, незадовільний рівень гігієни серед усіх обстежених груп дітей та середній рівень інтенсивності карієсу серед дітей із бронхіальною астмою і високий рівень серед дітей з алергічним ринітом та атопічним дерматитом; Приведены данные комплексного анализа стоматологического статуса 126 детей, больных бронхиальной астмой, аллергическим ринитом и атопическим дерматитом. Проведен анализ уровня интенсивности кариеса, состояния тканей пародонта и гигиенического статуса в соответствии с имеющейся общесоматической патологией и возрастной группой. Выявлены высокая распространенность воспалительных заболеваний пародонта, неудовлетворительный уровень гигиены среди всех обследованных групп детей, средний уровень интенсивности кариеса среди детей с бронхиальной астмой и высокий уровень среди детей с аллергическим ринитом и атопическим дерматитом; The article presents data on the complex analysis of the dental status of 126 children with asthma, allergi rhinitis and atopic dermatitis. Allergic diseases are an important problem of modern medicine because sensitization of adult and child population is growing. In Ukraine, the frequency of allergic diseases has been increased in 1.5 times for the last 5 years. The aim of this study is a complex study of the dental status of children with asthma, allergic rhinitis and atopic dermatitis. Analysis of decay intensity, the state of periodontal tissues and hygiene status has been made in accordance with the existing somatic pathology and age group. The high prevalence of inflammatory periodontal diseases, an unsatisfactory level of hygiene among all surveyed groups of children and the average intensity of caries in children with asthma and a high level among children with allergic rhinitis and atopic dermatitis has been observed. Analysis of the data has shown that the decay intensity is increasing due to the age in all groups, but children with allergic rhinitis are most prone to dental hard tissues damage. Among children with asthma, periodontal disease was detected in 76.8% of cases. In most cases generalized gingivitis (62.4%) was diagnosed, the localized form was diagnosed only in 37.6% of cases. Among children with atopic dermatitis, periodontal disease was detected in 71.6% of cases. Results of clinical examination have shown that bleeding gums has been observed in 48.3% of children, dental calculus − in 23.3% of children. The most widespread form of gum inflammation is generalized gingivitis (58.2%), localized gingivitis has been diagnosed in 41.8% of cases. In children with allergic rhinitis, periodontal diseases were found most frequently among all groups of children with atopic conditions − in 78.8% of patients. Results of clinical examination have showen that bleeding gums has been observed in 48.3% of children, dental calculus − in 23.3% of children. Catarrhal gingivitis has been defined the most common form of inflammatory periodontal disease − in 74.6% of cases, hypertrophic gingivitis has been found in 24.4% of cases; the prevalence of generalized gingivitis process has prevailed in 59.6% of the surveyed children. To assess the state of oral hygiene in children with allergic diseases the comparative analysis has been made using the index evaluation. Analysis of the data has shown a poor level of hygiene in all age groups. The best level of hygiene has been observed in 5-6-year-old children with atopic dermatitis (1.88 ± 0.29), the worst level of hygiene − in children with allergic rhinitis (2.28 ± 0.34). The data have shown certain changes of the dental status of children with asthma, allergic rhinitis and atopic dermatitis. The dental examination has led to the conclusion that all examined children are affected with inflammatory diseases of periodontal tissues and have an unsatisfactory oral hygiene level. Children with asthma have the average intensity of caries and it’s high in children with allergic rhinitis and atopic dermatitis. Perspectives for future research are establishing the correlation between dental status, immune status, existing co-morbidities and developing adequate methods of prevention and treatment of dental diseases among this group of children.
