Later, the pandemic of this disease took the world by storm, challenging the medical community. Its clinical manifestations vary from asymptomatic to a severe course that requires hospitalization and intensive therapy with oxygen support. The mortality rate in patients with a severe course of COVID-19 can exceed 50% The majority of fatal cases of COVID-19 were associated with thrombotic events, despite the prophylactic use of anticoagulant therapy. Numerous theoretical overviews and research articles indicate the need for genetic testing in patients with COVID-19 to determine the genetic profile of proteins involved in thrombophilia. According to the researchers, the Leiden mutation (G1691A, rs6025) of the FV gene is one of the promising candidates for testing. The aim of the work was to demonstrate the clinical features of the severe course of COVID-19 in the presence of the Leiden mutation. 58 patients with COVID19 of the intensive care unit were genotyped. The Leiden mutation in the heterozygous state was found only in one patient, who had Down syndrome. The Leiden mutation was detected with a frequency of 1.72% in the investigated group. The described clinical case clearly showed that individuals with Down syndrome, associated with hereditary thrombophilia are at risk of undesirable clinical consequences in the treatment of COVID-19. The condition of the patient with the Leiden mutation was severe when admitted to the hospital. The score according to the sequential organ failure assessment scale was 2 points. Bilateral multisegmental pneumonia was detected on the X-ray. On the second day after admission, due to the development of acute respiratory distress syndrome and multiple organ failure, the patient was transferred to the intensive care unit, where he received oxygen therapy through a facial mask. Medical treatment was carried out according to the protocol: non-steroidal anti-inflammatory drugs, antibacterial therapy, anticoagulants, sympathomimetics, and glucocorticosteroids. Despite the medical measures taken, progression of respiratory failure, renal failure, and portal hypertension was noted. On the 11th day, the patient developed asystole. Resuscitation measures were unsuccessful. Thus, the described case of a severe course of COVID-19 in a carrier of a heterozygous variant of the Leiden mutation with Down syndrome confirms the recommendations regarding the need for genetic testing for thrombophilia in high-risk groups and the appointment of personalized measures to prevent complications. Вперше про COVID-19 було повідомлено в грудні 2019 року в Ухані (провінція Хубей, Китай). Пізніше пандемія цієї хвороби захопила світ, кинувши виклик медичній спільноті. Його клінічні прояви варіюють від безсимптомного до важкого перебігу, що потребує госпіталізації та інтенсивної терапії з кисневою підтримкою. Метою роботи було продемонструвати клінічні особливості тяжкого перебігу COVID-19 за наявності лейденської мутації. Проведено генотипування 58 хворих на COVID-19 відділення інтенсивної терапії. Лейденська мутація в гетерозиготному стані виявлена лише в одного пацієнта з синдромом Дауна. У досліджуваній групі лейденська мутація виявлена з частотою 1,72%. Описаний клінічний випадок чітко показав, що особи з синдромом Дауна, асоційованим із спадковою тромбофілією, піддаються ризику небажаних клінічних наслідків при лікуванні COVID-19. Стан пацієнта з лейденською мутацією під час госпіталізації був важким. Оцінка за шкалою оцінки послідовної органної недостатності становила 2 бали. Рентгенологічно виявлена двобічна багатосегментарна пневмонія. На другу добу після госпіталізації у зв’язку з розвитком гострого респіраторного дистрес-синдрому та поліорганної недостатності хворого перевели в реанімаційне відділення, де йому проводили оксигенотерапію через лицьову маску. Лікування проводилось згідно з протоколом: нестероїдні протизапальні засоби, антибактеріальна терапія, антикоагулянти, симпатоміметики, глюкокортикостероїди. Незважаючи на вжиті лікувальні заходи, відзначено прогресування дихальної недостатності, ниркової недостатності, портальної гіпертензії. На 11 добу у хворого виникла асистолія. Реанімаційні заходи не принесли результатів. Таким чином, описаний випадок тяжкого перебігу COVID-19 у носія гетерозиготного варіанту лейденської мутації з синдромом Дауна підтверджує рекомендації щодо необхідності генетичного тестування на тромбофілію в групах високого ризику та призначення персоналізованих заходів щодо запобігти ускладненням.Документ NOS3 (rs61722009) gene variants testing in prediction of COVID-19 pneumonia severity(Elsevier, 2023) Fishchuk, L. H.; Похилько, Валерій Іванович; Чернявська, Юлія Ігорівна; Цвіренко, Світлана МиколаївнаBackground There is a hypothesis that a sufficient level of endothelial nitric oxide synthase is important for reliable protection against COVID-19. Theoretical ideas about the NOS3 gene demonstrated that it can have an effect on links of the complications pathogenesis in COVID-associated pneumonia. We determined the goal – to investigate the association of the NOS3 gene variants with the occurrence of the disease and its clinical course in patients of the intensive care unit. Methods The study group included 117 patients with a diagnosis of severe “viral COVID-19 pneumonia”. Determination of NOS3 gene variants was performed using the PCR method. The probability of differences in the quantitative results were determined using ANOVA or Kruskal-Wallis test (depends on normality of studied parameters). Results Our results indicate that the presence of the NOS3 gene 4a allele increase the risk of complicated COVID-19-associated pneumonia (χ2 = 18.84, p = 0.00001, OR = 3.53 (1.95–6.39)). It was shown, that carriers of the 4aa genotype had a significantly higher ratio of SpO2/FiO2 on the first and second days after hospitalization (p = 0.017 and p = 0.03, respectively). Patients with the 4aa genotype also had the acid-base imbalances, as showed by indicators of base deficiency and standard bicarbonate, which were beyond the reference values. Potassium and sodium concentrations on the first and second day after hospitalization were also significantly lower in patients with 4aa genotype (p = 0.009 and p = 0.048, respectively), for whom, in the same time, the concentrations of C-reactive protein and total bilirubin were significantly higher (p = 0.002 and p = 0.033, respectively). Conclusions Our results confirmed that the rs61722009 variant of the NOS3 gene is associated with an increased risk of severe СOVID-19-associated pneumonia and its adverse clinical course with potential progression of kidney and liver damage, and occurrence risk of systemic inflammatory response syndrome. These results require further research for the new metabolic strategy formation, in order to prevent the severe COVID-19 associated pneumonia and its complications.
