    Alternative health insurance model in economically developed countries

    The article explores the potential and dynamics of the non-profit health insurance model, emphasizing its organizational and legal structures, range of services, and factors influencing efficiency and competitiveness. It utilizes various methods, including structural, economic, and statistical analyses, to assess the viability of implementing non-profit insurance within Ukraine's healthcare system, drawing comparisons with practices in Western Europe, the United States, and Japan. Non-profit insurance companies, owned by their clients rather than investors, play a significant global role, accounting for about one-third of annual premiums and serving over 900 million policyholders. These entities blend features of financial and non-profit organizations: they manage statutory and insurance funds for financial operations while prioritizing member benefits over profit. Their unique approach allows for cost-effective premiums, tailored insurance conditions, democratic self-governance, and enhanced accessibility to a broad population. The study highlights the global relevance of non-profit insurance as a counterbalance to commercial insurers, fostering competition and mitigating unjustified price increases. It underscores the potential benefits for Ukraine in adopting this model, leveraging lessons from international practices, while considering national economic and social contexts. Collaborative integration of state, joint-stock, and non-profit insurers is proposed as an optimal solution to meet societal insurance needs. Further research is recommended to design a balanced health insurance mechanism incorporating diverse insurance models.

    Clinical and diagnostic aspects of managing patients with valvular heart disease

    The article presents the results of examination of 47 patients with valvular heart disease. According to the research findings, when investigating the causes of severe mitral regurgitation, especially in young and middle-aged people, one should primarily consider Barlow's disease, myxomatous mitral valve degeneration, as the primary cause (having excluded fibroelastic deficiency). It was found that among all cases of detected mitral valve prolapse (even minor or moderate), which is currently diagnosed quite often with the development of modern technology, stratification of patients requires additional examinations to identify people at high risk of disease progression and complications, as well as to determine the prognosis.

    Detection and assessment of disorders of the depressive spectrum in patients with acute myocardial infarction depending on the method of treatment

    The frequent combination of mental disorders of the depressive spectrum and cardiovascular diseases is one of the most urgent problems of modern medicine. The purpose of the study was to identify and assess, with the help of various psychodiagnostic methods, the degree of severity of depressive disorders in patients with acute myocardial infarction depending on the treatment method as a prerequisite for optimal drug therapy. 73 patients with the aforementioned pathology were studied and were divided into two groups. Group I included 44 patients who underwent urgent coronary angiography with subsequent stenting of the infarct-related coronary artery; Group II – included 29 patients who received only drug therapy. Examination of patients was carried out according to Ukrainian standards, in particular, a survey according to the scale of Beck and Zung. Patients with acute myocardial infarction after urgent coronary angiography with stenting of the infarct-dependent artery were more likely to have mild and moderate depression according to the Beck scale compared to patients who received only conservative medical treatment. The reverse pattern is observed for moderate and severe depression. 68.2% of patients with acute myocardial infarction after interventional intervention and 62.1% on the background of conservative treatment according to the Zung scale showed depression of various degrees of severity, mostly mild.

    Inflammation syndrome with autoimmune component in the patients with acute myocardial infarction: quantum genetic algorithm, neuronet

    The purpose of the study was optimizing the diagnosis of the inflammatory syndrome with an autoimmune component in patients with acute myocardial infarction using a quantum genetic algorithm and graph neural networks. There were 47 patients observed with acute myocardial infarction, and among them, 35 experienced complications. The diagnostic criteria of the inflammatory syndrome with an autoimmune component include elevated level of ratio of chaperone 60 autoantibodies to interleukin-10 more than 4.99 conditional units, ratio of chaperone 60 autoantibodies to C-reactive protein more than 4.2 conventional units, ratio of C-reactive protein to interleukin-10 is less than 0.2 conventional units. This syndrome was present in 33 (70.2 %) out of 47 myocardial infarction patients (p=0.01 by the Chi-Square test). It was presented on visual programming language “Dragon”. Quantum genetic algorithm, graph neural networks using for making decisions for an inflammation syndrome with autoimmune component diagnosis presented in this investigation.

The purpose of the article is to analyze various aspects and methods of independent work of students in the light of innovative educational technologies in the study of internal medicine in order to optimize the educational process.

    Аутокоїди як тригери і потенціатори циркадних змін серцевого ритму у хворих на ішемічну хворобу серця в поєднанні з гіпертонічною хворобою

    Метою дослідження було визначення циркадних змін серцевого ритму залежно від рівня маркерів запалення у хворих на ішемічну хворобу серця в поєднанні з гіпертонічною хворобою. Досліджено 35 хворих з вищезгаданою патологією. Обстеження хворих проводилось згідно стандартів України, зокрема цілодобове моніторування електрокардіограми та визначення прозапальних цитокінів. У хворих на ішемічну хворобу серця в поєднанні з гіпертонічною хворобою з підвищеним рівнем інтерлейкіну-8 встановлено зниження хронотропної активності серця. Відмічається збільшення рівня інтерлейкіну-6 у хворих на ішемічну хворобу серця в поєднанні з гіпертонічною хворобою з підвищеним рівнем інтерлейкіну-8. Підвищення рівня інтерлейкіну-1β та зниження рівня С-реактивного білка характеризується значним переважанням у хворих симпатичного відділу нервової системи вночі. Виявлено збільшення стандартного відхилення R-R-інтервалів у хворих на ішемічну хворобу серця в поєднанні з гіпертонічною хворобою з підвищеним рівнем ІЛ-6.

    Організація науково-дослідної роботи студентів при вивченні внутрішньої медицини як передумова у формуванні професійної особистості лікаря

    Проведення науково-дослідної роботи студентів на кафедрі внутрішньої медицини № 1 підвищує ефективність навчання та сприяє формуванню професійної особистості лікаря. Основною структурною одиницею організації науково-дослідної роботи майбутнього лікаря ̶ студентська наукова група, де головною формою є участь студентів у наукових дослідженнях, що виконуються на кафедрі. Атмосфера студентської наукової групи дозволяє майбутнім медикам відчути себе справжнім фахівцем своєї професії та науковцями, а проведення засідань групи дає їм можливість зрозуміти тонкощі роботи лікаря. На кафедрі є наукова лабораторія з різною інструментальною апаратурою для обстеження пацієнтів, де майбутні лікарі мають вільний доступ до практики. Науковою роботою охоплені студенти 4-6 курсів, які неодноразово посідали призові місця на Республіканських та Міжнародних наукових студентських конференціях, Всеукраїнських науково-практичних конференціях. Курація стаціонарного пацієнта з оформленням історії хвороби є варіантом навчально-дослідної роботи при вивченні внутрішньої медицини, що дозволяє закріпити отримані теоретичні знання та практичні навички. Кращі історії хвороби за змістом і за оформленням, які оцінені викладачем на «відмінно» представляються на конкурс «На кращу історію хвороби». Нагородження студентів монографіями від співробітників кафедри та грамотами активує їх до більш серйозного відношення у виконанні даної роботи. Проводяться щотижневі клінічні конференції для студентів 6-го курсу, де заслуховують доповіді, які доповнюються клінічним розбором історій хвороб з демонстрацією пацієнта. Таким чином, науково-дослідна діяльність на кафедрі внутрішньої медицини № 1 в рамках студентської наукової групи та навчально-дослідна робота кожного студента, яка проводиться на практичних заняттях, формує позитивне ставлення студентів до своєї майбутньої професії, сприяє підвищенню якості підготовки майбутніх фахівців, які зможуть застосувати знання, вміння і навички в практичній діяльності.
