Materials and Methods: Pre-treatment by 3D Cone-beam computed tomography radiographs for 20 patients of two age groups with class 1 malocclusion was undergone for 11 angular and 4 liner measurements from right and left sides, anteroposterior, vertical and frontal view by hand except the hard landmarks determination. Chi-test was used to compare the result of both sides (P < 0.05). Results: No difference was found between left and right analysis for linear measurements as for the angular cephalometric measurements. Conclusion: Measurements of skeletal analysis by 3D CBCT cephalograms from right side were found to be similar to left side. So, we can use the left analysis for orthodontics diagnosis as right side. The Sella in orthodontics is most an important skeletal constant unilateral landmark in all cephalometric analysis of the neurocranial and craniofacial complex which located at the center of sella turcica. It's used to measurement the positions of maxilla and mandible in relation to the cranium and to themselves which help in investigation, diagnosis and treatment plan. Aims : The purpose of this study was to evaluation of Sella Turcica dimension(area and perimeter) in skeletal class III malocclusion (as study group) in comparative with skeletal class I normal occlusion (as control group) among adult Ukrainian peoples by using 3D Cone-beam computed tomography (CBCT). Materials and Methods: Pre-treatment 3D Cone-beam computed tomography radiographs for 62 subjects of adult patients (18-27 years old ) , 31 subjects (10 male, 21 female) with class I normal occlusion as control group and 31 subjects (12 male ,19 female) with class III skeletal malocclusion according to ANB (The angle formed at A point, Nasion point and B point of skull) and FMA (The angle formed between the mandibular plane and Frankfort Horizontal plane) angle. The digital tracing of sella Turcica was done by the area and perimeter measures icon tool for all subjects in window of the advanced 3d-imaginag software Ez3d2009. The independent samples T test was used to assessed the relationship between skeletal type and sella Turcica dimension, also was used to determine if skeletal type showed the significant differences. Standard devotions, standard errors, mean values and normality of data were generated for all parameters. Result and discussion: When comparing the area of sella Turcica for two group, there were significant differences between the study group and the control group (P=0.0116) while the sella perimeter were non-significant differences (P=0.0662). Furthermore, when our result was compared with those in other global data, disparities dimensions among different populations were observed. Conclusion: When sella size was compared with skeletal type, a significant difference was found in diameter size between Class I and Class III subjects. Larger diameter values were present in skeletal Class III subjects, while smaller diameter sizes were apparent in Class I subjects, so the size of Sella Turcica and pituitary gland play important role to determine the skeletal developmental, classification and occlusion of jaws. The results of the present study of sella area and perimeter may be used as reference standards for Ukrainian subjects in relation to sella Turcica size.Документ Functional condition of masseters muscles of patients with class II subdivision(Aluna Publishing, 2017) Kuroedova, V. Masseter. We have not found datum about functional status of the masticators in patients with class II subdivision. The aim: The purpose of our study was to investigate the functional characteristics of m. Masseter, m. temporalis in adult patients with class II subdivision malocclusion. Materials and methods: There have been carried out the surface electromyographic study of m. masseter, m. temporalis in 17 adult patients with class II subdivision. It was realized quantitative analysis of 271 electromyogram, it was determined the average bioelectric activity index activity, symmetry and torsion index. Results: It was observed predominance of the bioelectrical activity of m. temporales on m. masseter for all persons with class II subdivision. Bioelectrical activity for m. masseter was bigger on side of distal ratio and for m. tennporales on side of neutral ratio. In class II subdivision right, the mandible was deviated to the left side and in class II subdivision left is deviated to the right side. Thus, rotational moment generated during compression of the jaws, causes deviation of the lower jaw to the side, with a neutral molar ratio. During voluntary chewing bioelectrical activity of m. masseter and m. temporalis was higher in the right side. The aim of the paper is to carry out a comparative analysis of the mineral density of the bone tissue of the lower jaw (mandible) and the mineral density of the second cervical vertebra (C2) in patients with malocclusion. Materials and methods: 37 computer tomograms of orthodontic patients were studied, which were divided into three age groups according to the periods of the formation of the dentoalveolar system. Measurement of bone density of (second cervical vertebra) C2 was performed in the sagittal projection along the middle of the height of the vertebra. In the mandible, measurements were made on axial sections in the vestibular and oral direction in the region of the alveolar process between the central incisors, between the canine and the first premolar at the mid-root level, in the region of the first molars below the bifurcation level, in the retromolar region and the region of the articular process. Results: The greatest similarity in densitometric parameters of bone density is established between the second cervical vertebrae and the density of the joint head. The most dense site on the lower jaw is the alveolar process between the central incisors, which increases with age from 1274.71 ± 34.7 in group I to 1400.6 ± 7 5 , 5 6 in the III group, these indicators are almost 2-2.5 times higher than the density of C2. Conclusions: Mandible presents irregular density of bone based on optic denstitometry in different areas. Maximal indices of bone density are established in the area of alveolar processes where the jaw is exposed to maximal occlusal loading.Документ Optical density of upper jaw in patients with malocclusion(2017) Kuroedova, V. Computer tomography plays an important role to examine and investigate the density of bony tissue resulting from total increase of osteopenic diseases and diseases of periodontal tissue. The aim of the paper is to compare indices of mineral density of bony tissue of the upper jaw and mineral density of the second cervical vertebra in patients with malocclusion. Materials and Methods: 37 orthodontic patients were involved in the investigation. They were divided into three age groups depending on the period of formation of dentofacial system. Density measurement of bone of the second cervical vertebra was done and also density measurement of upper jaw in the area of alveolar process between central incisors, canines and the first premolar on the level of the middle of roots, in the area of the first molars under the level of bifurcation and in cusp was performed. Results: Optical density of bone of the second cervical vertebra with age increases from 501±61,06 to 587,6±48,81. The densest area on the upper jaw is alveolar process between central incisors, which increases with age from 1045,14±59,81 to 1318±69,28. The least indices of optical density were determined in area of the cusp of the upper jaw: the first group presented 174,21±38,94, and the third one included 338,87±26,91. Conclusions: Densitometry of bony tissue with computer tomography is diagnostically informative and available method for investigation and it can be used for diagnostics of bony tissue condition and for evaluation of orthodontic treatment.Документ Symmetry of elements of temporomandibular joint (TMJ)(2017) Kuroedova, V. Evaluation of TMJ and detailed characteristics of its elements with additional methods of examination in children and adults is necessary for clinical definition of proposed disturbances of the structure and functions of the joint. The aim of the investigation is to study morphological symmetry ofTMJ in patients with dentofacial abnormalities and with dentofacial abnormalities complicated by secondary edentulism. Materials and Methods: 57 patients were involved in the examination. Based on gender principle patients'distribution was almost equal: there were 30 women and 27 men. Cone-beam computerized tomography (CBCT) Galileos (SIRONA DENTAL, Germany) was used to all patients. Results: It was done analysis of parameters (height and length) of right and left heads (condyles) of temporomandibular joint in both groups. Asymmetry of parameters of heads' length in saggital area in patients of the second group was defined. It was proved statistically (left 10,38±0,76, right 8,16±0,78). Conclusions: Increase of asymmetry of length of heads of TMJ in saggital area with age was determined. It can be explained by complication of dentofacial abnormalities and the presence of secondary edentulism. Depending on bite type length of condyle, especially at prognathism (in saggital) area peculiar clinical problems with TMJ can be present due to asymmetry of condyles. K.This article is concemed with the evaluation oftemporomandibular joint position, symmetrical heads position, timely diagnostics ofdentofacial abnormalities. The aim of the investigation is to study position and evaluate symmetry of condyles of temporomandibular joint of dentofacial abnormalities based on Angle s classification of malocclusion (the first class includes normal jaw relations; the second one contains distal position of mandible, the third class includes medial position of mandible). Materials and methods. 70 patients were involved in the investigations that have dentofacial abnormalities. cone beam computed tomography was also used. Based on Angle's classification, all patients were divided in three groups: 1st group included 34 patients (the first class), 2nd group included 30 patients (the second class), 3rd group included 6patients (the third class), 34 male patients and 36 female patients were involved in the investigation aged from 8 to 29 years old. The reveal of heads of temporomandibular joint position was done by H Gelb in medial sagittal plane ofjoint. Results. Based on results of our investigation, it has been established that patients having bite abnormalities and correct position of temporomandibular joint heads on either side of position 4/7, only 8 patients are involved in this group (11.43 %). Based on Angle's classification (the 1st class) symmetrical position of heads of that joint in 4/7 was indicated 17.65 %, the second class includes 6.67 %, the third one contains no one case was established. Despite bite abnormality, number of patients with correct position of condyles in segment 4/7 from 20 % to 5 % is decreased. Besides pathology is caused by increase of asymmetrical correlations of condyles of the joint in the segments with 6.67 % to 50 % (based on Gelb). Conclusions. Despite class of bite pathology symmetrical position of heads in position 4/7 is occurred only in 11.43 % cases, and it indicates the correlation between dentofacial abnormalities and temporomandibular joint pathology, correspondingly complex approach to treatment of orthodontic patients is simply correct. Number of patients with optimal position of heads of the joint in segment 4/7 with 17.65 % based on 1st Angle's classification, to 6.67 % based on 2ndAngle 's classification, absence of patients of the 3rd class is decreased. The aim of the investigation is to compare the analysis of temporomandibular joint symmetry, the position of mandible and rotation edges of temporomandibular joint heads in dentofacial abnormalities based on the first and the second type of classification by Angle. The investigation was carried out among 27 patients with dentofacial abnormalities using cone beam computed tomography. The patients were divided into two groups based on types of malocclusion - the first type of occlusion by Angle and the second one by Angle’s classification. While studying the distance from the middle of articular heads on the right and on the left to midline, passing through pterygoid processes of sphenoid bone it was determined that in the second class of Angle’s classification of dentofacial abnormalities, articular heads are situated posteriorly to cranial basis on 2.45-3.47 mm (р<0.005). The difference between the rotation edges of temporomandibular joint heads is more than 5° of the 1st class according to classification and it occurred in 23.08% of patients; the second class occurred in 64.29%. The range of maximal difference of the rotation edge of the 1st class contains 5.2°, and the 2nd class of Angle’s classification includes 15.2°, that determines the severity of dentofacial abnormality of the second classification by Angle. In the second type of Angle’s classification articular heads of temporomandibular joint are situated posteriorly that confirms the pathogenesis of distal bite. Proposed methods of measurement can be used for differential diagnosis of skeletal forms of second type dentofacial abnormalities according to Angle’s classification. With increase of dentofacial abnormality severity, rotation of temporomandibular joint heads also increases. Денситометрия костной ткани с применением цифровой КТ является диагностически информативным методом исследования и может применяться для диагностики состояния костной ткани челюстей и оценки качества ортодонтического лечения.; Due to the rapid increase of osteopenic diseases and diseases of periodontal tissues, X-ray examination of malocclusion anomalies at the diagnostic stage is a major step at orthodontic treatment planning. One of the modern, moust informative and accessible method of objective research of patients is a computer tomography, which is increasingly used in orthodontic practice and bone density studys. The aim of the paper is to conduct a comparative analysis of the values of mineral density of bone tissue in different sections of the upper and lower jaws and the mineral density of the second cervical vertebra (C2) in orthodontic patients. Object and methods. In total, 15 CT orthodontic patients aged 21-40 years old (mean age 26.6) were studied, which corresponds to the period of active functioning of the tooth-jaw system. The basis for comparison was taken the density of the bone C2, which was carried out in the sagittal projection mid-height of the vertebra. On both jaws, measurements were made in the area of the alveolar appendix between the central incisors, from the cervix to the first premolar at the midpoint of the roots, in the region of the first molars below the bifurcation level. In addition, at the upper jaw, measurements were made at the same points in the bulge region, on the lower jaw in the retro-molar region and in the articular head. Results. The most dense areas on the upper and lower jaws is the alveolar process between the central incisors – 1318±69,28 and 1400,6±75,56, respectively. Between the canine and the first premolar on the upper and lower jaws, the following data were established: 930,97±29,44 and 1204,17±54,8. The statistical difference was established (p1-2<0.05). These values are on average 50% greater than the bone density in zone C2.At the area of first molars bifurcation, the density of bone tissue on the lower jaw (882,67±53,9) is exceeds similar indicators for the upper (531,47±36,82) with a statistical difference. The optical density in the area of the tuberosus of the upper jaw is lower than the density of C2 and the bone density of the alveolar process. On the lower jaw, the most similar densitometric indicators of bone density are established between C2 and the density of the joint head. In the retromolar region, high indices compared with C2 (p<0,01) are cause of the dense compact plates of the outer and inner oblique lines in the region of the angle of the lower jaw. Conclusions. Densitometry of bony tissue with computer tomography is diagnostically informative and available method for researches. Распространенность зубочелюстных аномалий среди взрослых составила 82%. За последние 113 лет наблюдается увеличение количества аномалий І класса и уменьшение аномалий III класса, что может быть связано со значительным снижением жевательной нагрузки современного человека. Наименьших изменений претерпела распространенность нарушений прикуса ІІ класса, что отражает некую независимость дистальной окклюзии от эволюционных процессов. Впервые определена и проанализирована частота встречаемости односторонних форм ІІ класса по E. Angle, частка яких серед інших зубощелепних аномалій склала 13%.; There has been performed the estimation of I, II, III classes of malocclusion prevalence in adults. These data has been assessed in historical perspective. Prevalence of malocclusion among adults was 82%. Over the last 113 years there has been an increase in the number of class I anomalies and the reduction of class III anomalies that may be associated with a significant reduction in masticatory loading of modern man. The smallest change undergone prevalence of class II malocclusion, which reflects a certain independence of the distal occlusion from evolutionary processes. V.; Makarova, A. N.; Rud, Y. V.; Kuroiedova, V. D.Сучасний ортодонт є дуже затребуваним фахівцем, в зв'язку з великою кількістю пацієнтів, що звертаються за ортодонтичною допомогою. Аналіз пацієнтів за віком, діагнозом і статтю допомагає ортодонту більш конкретно зрозуміти, з якою патологією і які пацієнти найчастіше звертаються на прийом. Такий аналіз дає можливість сформувати напрямок розвитку діагностики, лікування і профілактики зубощелепних аномалій. На кафедрі післядипломної освіти лікарів - ортодонтів провели аналіз 2359 амбулаторних карток: 1296 пацієнтів, які лікувалися в 2019 році, і 1063 карток - в 1999 році. За 20-річний період звернення пацієнтів зросло в 9,9 разів, не зважаючи на стрімкий розвиток приватної стоматології. Результати показують, що на сьогоднішній день більшість ортодонтичних пацієнтів – це дорослі. Тому пріоритетним стало лікування незнімною технікою. При цьому частіше за допомогою звертаються жінки, що обумовлено зростанням соціального відношення жінок до своєї зовнішності і є підтвердженням соціально-психологічного значення ортодонтії. У віці 14-18 років збільшується кількість осіб чоловічої статі, які звертаються за допомогою до ортодонта, що може бути пов'язано з проходженням медичної допризовної військової комісії. На основании амплитудных пока-зателей биопотенциалов m. masseter и m. temporales опре-делены индексы Ac, SIM, Tor. Обнаружено, что функциональной характеристикой асим-метричного ІІ класса является преобладание нерациональ-ного темпорального типа жевания и смещение нижней че-люсти в сторону с нейтральным соотношением моляров при стандартных электромиографических пробах; У статті викладені результати електроміографічного до-слідження жувальних м'язів у осіб з асиметричним ІІ кла-сом по E.H. Angle. На підставі амплітудних показників біо-потенціалів m. masseter і m. temporales визначені індекси Ac, SIM, Tor. Виявлено, що функціональною характеристикою асимет-ричного ІІ класу є переважання нераціонального темпора-льного типу жування і зсув нижньої щелепи в сторону з нейтральним співвідношенням молярів при стандартних електроміографічних пробах. There is evidence that class II subdivision is connected with bone asymmetry of upper and lower jaw and skull base. Data of functional status of masticatory muscles in patients with class II subdivision was not found. The aim of the study was to investigate the functional character-istics of the masticatory muscles in adults with class II subdivi-sion. Electromyography study of m. masseter and m. temporales was held in 17 adults with class II subdivision: group A (9 patients) – with subdivision right; group B (8 patients) – with subdivision left. Such functional tests were used: maximum volitional lock-jaw in central occlusion (1), free chewing (2) given chewing on the right side of (3) given chewing on the left side of (4). Indices Ac, SIM, Tor were defined. Results. In tests (1,3,4) for all patients m. temporales dominated over m. masseter, which was confirmed by the negative value of the index Ac. Rotary moment produced during compression of the jaws, free and given chewing deposed mandible to the side with neutral molar relationship. Мета дослідження - перевірити доцільність використання індексу Шимбачі в обстеженні ортодон- тичних пацієнтів. Було проведено морфометричне дослідження контрольно-діагностичних моделей щелеп 300 ортодонтичних пацієнтів різного віку. За результатами дослідження, всі види ЗЩА характеризуються зниженням міжальвеолярної висоти відносно індексу Шимбачі, навіть ті, що супроводжуються зменшенням глибини різцевого перекриття. Отримані дані наводять на такі роздуми: або «ідеальна» висота прикусу, розрахована за індексом Шимбачі, не така й ідеальна, а її розрахунки дещо завищені й потребують уточнення, зокрема дослідження скелетних параметрів за даними рентгенографії черепа, або, справді, сучасні ортодонтичні пацієнти мають тотальне зменшення міжальвеолярної висоти.Документ Характеристика односторонних аномалий ІІ класса по Э. Выявлено, что односторонний II класс ЗЧА не является зубоальвеолярной аномалией, а связан с асимметричным смещением НЧ относительно верхней в сторону с дистальным соотношением моляров;The aim of the study was to evaluate the asymmetry of dentition, mandible and bite features in adult patients with unilateral class II malocclusion. To develop clinical thinking of doctors, the department uses a number of modern educational technologies aimed at both high theoretical training and perfection of practical skills. In the course of theoretical training with cadets we provide: lectures, seminars, medical history challenges using multimedia presentation, abstracts preparation for scientific conferences. The theoretical training of a modern cadet, presupposes an active self-employed work, the organization of which is helped by the methodical guidelines that we developed, teaching aids, etc. At transfer to the European system of teaching, the department is expands access for teachers and cadets to data banks of new scientific directions, new educational literature, new computer classes, phantoms and dummys.Practical classes for cadets provide involvement in the clinical and work in a dental laboratory. During practical training, cadets master skills needed for working with orthodontic patients of different ages, the latest diagnostic methods (cephalometric, CBCT), modern removable and non-removable orthodontic techniques. In the dental laboratory master the stages of manufacturing removable orthodontic apparatus, control-diagnostic models, bite patterns and etc. In the training process of the “Specialization” cycle, there are 3 intermediate exams, that allows cadets to more thoroughly approach orthodontic study and successfully prepare for the final exam. The Department of doctor-orthodontist’s postgraduate education uses final examination as a last monitoring method to check cadet-doctors acquired knowledge. At this exam we assess the level of theoretical knowledge and practical skills done by “Orthodontics” curriculum and the Ministry of Health program. This examination provides extensive knowledge control, that includs independent computer testing and practical exams (based on real clinical cases). That’s why various modern methods of training and future doctors’s knowledge monitoring, increases motivation of mastering own profession and effectiveness of provided materials, which leads to increasions of future doctors’s professional skills.
