Проведений аналіз рівня інтенсивності карієсу, стану тканин пародонта і гігієнічного статусу відповідно до наявної загальносоматичної патології та трьох вікових груп. Виявлено високу поширеність запальних захворювань пародонта, незадовільний рівень гігієни серед усіх обстежених груп дітей та середній рівень інтенсивності карієсу серед дітей із бронхіальною астмою і високий рівень серед дітей з алергічним ринітом та атопічним дерматитом; Приведены данные комплексного анализа стоматологического статуса 126 детей, больных бронхиальной астмой, аллергическим ринитом и атопическим дерматитом. Проведен анализ уровня интенсивности кариеса, состояния тканей пародонта и гигиенического статуса в соответствии с имеющейся общесоматической патологией и возрастной группой. Выявлены высокая распространенность воспалительных заболеваний пародонта, неудовлетворительный уровень гигиены среди всех обследованных групп детей, средний уровень интенсивности кариеса среди детей с бронхиальной астмой и высокий уровень среди детей с аллергическим ринитом и атопическим дерматитом; The article presents data on the complex analysis of the dental status of 126 children with asthma, allergi rhinitis and atopic dermatitis. Allergic diseases are an important problem of modern medicine because sensitization of adult and child population is growing. In Ukraine, the frequency of allergic diseases has been increased in 1.5 times for the last 5 years. The aim of this study is a complex study of the dental status of children with asthma, allergic rhinitis and atopic dermatitis. Analysis of decay intensity, the state of periodontal tissues and hygiene status has been made in accordance with the existing somatic pathology and age group. The high prevalence of inflammatory periodontal diseases, an unsatisfactory level of hygiene among all surveyed groups of children and the average intensity of caries in children with asthma and a high level among children with allergic rhinitis and atopic dermatitis has been observed. Analysis of the data has shown that the decay intensity is increasing due to the age in all groups, but children with allergic rhinitis are most prone to dental hard tissues damage. Among children with asthma, periodontal disease was detected in 76.8% of cases. In most cases generalized gingivitis (62.4%) was diagnosed, the localized form was diagnosed only in 37.6% of cases. Among children with atopic dermatitis, periodontal disease was detected in 71.6% of cases. Results of clinical examination have shown that bleeding gums has been observed in 48.3% of children, dental calculus − in 23.3% of children. The most widespread form of gum inflammation is generalized gingivitis (58.2%), localized gingivitis has been diagnosed in 41.8% of cases. In children with allergic rhinitis, periodontal diseases were found most frequently among all groups of children with atopic conditions − in 78.8% of patients. Results of clinical examination have showen that bleeding gums has been observed in 48.3% of children, dental calculus − in 23.3% of children. Catarrhal gingivitis has been defined the most common form of inflammatory periodontal disease − in 74.6% of cases, hypertrophic gingivitis has been found in 24.4% of cases; the prevalence of generalized gingivitis process has prevailed in 59.6% of the surveyed children. To assess the state of oral hygiene in children with allergic diseases the comparative analysis has been made using the index evaluation. Analysis of the data has shown a poor level of hygiene in all age groups. The best level of hygiene has been observed in 5-6-year-old children with atopic dermatitis (1.88 ± 0.29), the worst level of hygiene − in children with allergic rhinitis (2.28 ± 0.34). The data have shown certain changes of the dental status of children with asthma, allergic rhinitis and atopic dermatitis. The dental examination has led to the conclusion that all examined children are affected with inflammatory diseases of periodontal tissues and have an unsatisfactory oral hygiene level. Children with asthma have the average intensity of caries and it’s high in children with allergic rhinitis and atopic dermatitis. Perspectives for future research are establishing the correlation between dental status, immune status, existing co-morbidities and developing adequate methods of prevention and treatment of dental diseases among this group of children.Документ Порівняльна оцінка ураженості карієсом постійних зубів у дітей із використанням індексів НІК, ICDAS і якості їхнього стоматологічного здоров'я за критеріями EGOHID(Українська медична стоматологічна академія, 2016) Смоляр, Ніна Іванівна; Чухрай, Наталія Львівна; Smoliar, N. Аналіз інтенсивності карієсу постійних зубів за індексом ICDASII 4-6 показав, що середнє його значення у віці 7 років складає 1,13±0,06 зуба, тоді як за індексом ICDASII 1-6 – 1,72±0,06 зуба. З віком значення обох індексів зростають. Середнє значення ICDASII 4-6 збільшується до 12 років у 3,08 раза, а до 15 років – у 4,45 раза. Натомість середнє значення ICDASII 1-6 до 12 років зростає в 3,41 раза, а до 15 років – у 5,69 раза. Установлено, що в 7-річних дітей середнє значення SIC склало 2,56±0,14 зуба при КПВ=0,93±0,13 зуба; в 15 років - 8,59±0,43 зуба при КПВ=5,40±0,31 зуба. Суб’єктивні індикатори доповнюють дослідження стоматологічного статусу і надають інформацію про фактори ризику виникнення стоматологічних захворювань та їхнього впливу на якість життя дітей; Проведена оценка пораженности кариесом постоянных зубов у детей с использованием индексов КПУ, SIC, ICDAS II и качества их стоматологического здоровья согласно критериям EGOHID. Анализ интенсивности кариеса постоянных зубов по индексу ICDAS II 4-6 показал, что среднее его значение в возрасте 7 лет составляет 1,13±0,06 зуба, тогда как по индексу ICDAS II 1-6 – 1,72±0,06 зуба. С возрастом значения обоих индексов увеличиваются. Среднее значение ICDAS II 4-6 увеличивается до 12 лет в 3,08 раза, а до 15 лет – в 5,69 раза. Установлено, что в группе 7-летних детей среднее значение SIC составило 2,56±0,14 зуба при КПУ=0,93±0,13 зуба; в 15 лет - 8,59±0,43 зуба при КПУ=5,40±0,31 зуба. Субъективные индикаторы дополняют исследования стоматологического статуса и предоставляют информацию о факторах риска возникновения стоматологических заболеваний и их влияния на качество жизни детей; An assessment of dental caries severity of the permanent teeth in children has been carried out via studying DMF, SIC, ICDAS II indices and quality of their dental health by EGOHID criteria. Analysis of intensity of permanent tooth caries by the ICDASII 4-6 index has shown that its average value in the age group of 7-year-olds is 1.13±0.06 teeth, while by the ICDASII 1-6 index it is 1.72±0.06 teeth. It has been estimated that with age the values of both indices grow. Average value of ICDASII 4-6 is increased in 3.08 times till 12, and in 5.69 times till 15. The significant increasing in the number of affected teeth with code 1 (the first visible changes on the surface of enamel after prolonged drying) and code 2 (an obvious visual changes enamel) with age has been assessed statistically. Thus, the number of carious lesions with the code 1 from 7 to 15 years old has been increased from 0.26 ± 0.09 teeth to 1.10 ± 0.17 teeth, and the number of carious lesions with the code 2 has been increased from 0.33 ± 0.09 teeth to 2.21 ± 0.30 teeth. The medium stage of dental caries, characterized by localized visible destruction of tooth enamel, is signed by the code 3 in the case when there is a visible localized destruction of enamel with no signs of dentine injuring (the code 3 by ICDAS II index) or translucence darkening under the enamel dentine (code 4). The number of cavities with the code 3 per child tends to increase with age, but their number with the code 4 (ICDAS II) has increased slightly. The same tendency goes with deep carious lesions (code 5 and 6 code of ICDAS II index). It has been set that in 7-year-old children the average index value of SIC is 2.56±0.14 teeth at DMF=0.93±0.13 of teeth, in 15-year-old age is 8.59±0.43 teeth at DMF=5.40±0.31 of teeth. Analysis of children’s responses to the question "How do you assess the state of your teeth?" (Indicator D1) has revealed that with age self-estimation is changing. The percentage of children who evaluates the condition of their teeth as excellent has been increased from 12 to 15 years old from 4.00 ± 1.96% to 9.00 ± 2.86%, while the number of persons who is hesitant has been reduced from 15.00 ± 3.57% to 6.00 ± 2.37%. Processing and analyzing the results of the responses on indicator D3 have shown that from 12 to 15 years old the number of persons that are “satisfied” of appearance of their teeth has been reduced from 59.00±4.92% to 55.0±4.97 %, and the number of persons who cannot answer this question has been reduced from 26.00±4.39% to 18.00±3.84%, while the proportion of “dissatisfied” children has been increased from 15.00±3.57% to 25.00±4.33%. The responses of children to the question “How often do you feel toothache for the last 12 months” (Indicator D2) and “Did you leave the lessons or miss classes because of toothache?” (Indicator D5) have been analyzed. It has been established that with age (from 12 to 15 years) the proportion of children who often feels toothache (from 4.00±1.96% to 5.00±2.18%) and those who felt it significantly increases, but rarely (from 38.00±4.72% to 45.0±4.97%). Among the 12-year-old surveyed children while answering the question “Did you leave the lessons or miss classes because of toothache?” (Indicator D5) 10.00±3.00% answered positively, the amount of these children has decreased to 6.00±2.47% till 15. Subjective indicators are complemented with researches of stomatological status and give information about the risk factors of dental diseases’ origin and their influence on children’s life quality.
