Дисертація присвячена удосконаленню методів діагностики бронхіальної астми у дітей, на підставі визначення факторів ризику розвитку захворювання, особливостей клінічного перебігу та змін функціонального стану імунної системи, а також підвищенню ефективності лікування шляхом застосування специфічних алерговакцин та імуномодулюючих препаратів Визначено, що у дітей з бронхіальною астмою в періоді ремісії зберігається стійкість не тільки рівня сенсибілізації до певних алергенів, але й дисфункція імунної системи у вигляді цитокінового дисбалансу. Патогенетично обгрунтовано доцільність САВ у комплексі з індукторами синтезу ендогенного гамма-інтерферону.

Kyiv, 2005 Based on the conducted multifactorial analysis the main risk factors of bronchial asthma development in children (burdened allergic inheritance, especially concerning respiratory allergy, and also in female line of generation, disordered feeding during the first year of life, allergic diathesis and unfavourable living conditions) were found We have determined age regularities of bronchial asthma formation; have researched the peculiarities of sensitization spectrum formation to different allergen groups. Researching immune system status in children with bronchial asthma in the remission period we have ascertained steady disbalance in its cellular and humoral components, which closely correlates with burdened inheritance with itself. Discovered age evolution of bronchial asthma formation in children, presented by the phenomenon of unfavourable factors summation and immunological features of clinical course in the remission period, proves the necessity of treatment-and-prevention work for these patients even in this disease period. The aim of this study was to assess the clinicoimmunologic efficacy and safety of specific allergovaccination (SAV) and immunomodulating preparation, Proteflazid and Cycloferon in children with atopic bronchial asthma. The efficacy of allergovaccination was estimated after 3, 6 and 12 months with the help of clinical indexes according to the scale taking into account the day-time and night signs of asthma. The results of efficacy of the treatment were compared with the change CD 23+ cells, Ig E, interleukin-4 (DL-4) and interferon-gamma (INF- y) before and after allergovaccination. The obtained data allow us to conclude that combined treatment with SAV and Proteflazid or Cycloferon was more effective, then in die control group. In group with combined treatment, less side effects were registered. Combined immune therapy stimulated cellular and humoral immune response, normalization of cytokines (IL-4, INF-y) and Ig E production in children with bronchial asthma.
