In the XX century, in the developed countries of the world, the number of people aged 55 and over increased from about 4 to 13%, topto there is a tendency for these indicators to grow [3,6]. It is this demographic situation that determines the increased attention to research in the field of aging, especially in relation to its potential age-related changes [3,6,11]. Skin aging is part of the overall biological aging process and is governed by the same laws. With age, diseases approach, the colors of life fade, physical activity and sexual desire decrease, chronic fatigue increases [3,5,6,8]. The reason for these problems is the extinction of the function of the reproductive system against the background of general aging of the body [7,8,11]. The skin is the first to react to changes in the body, and also takes on the action of exogenous (climatic changes, UV, lifestyle, etc.) and endogenous factors [1,2,5]. An important endogenous factor in skin aging, especially in women, is the state of the endocrine system [4,5,8,9]. Багатофакторний патогенез акне включає фолікулярну гіперкератинізацію, мікробну колонізацію Cutibacterium acnes, вироблення себуму, складні запальні механізми, що включають як вроджений, так і набутий імунітет, нейроендокринні механізми, а також генетичні та негенетичні фактори.Фактори ризику розвитку акне включають підлітковий вік, обтяжений сімейний анамнез та жирний тип шкіри. Незважаючи на те, що існує ці-ла низка методів лікування акне, тривають зусилля, спрямовані на зменшення побічних ефектів, таких як подразнення, сухість і фоточутливість шкіри, а також на підвищення ефективності за допомогою вдосконалених формул і препаратів з новими механізмами дії. Нові препарати з новими механізмами дії та вдосконаленими формулами спрямовані на різні ланки багатофакторного пато-генезу акне. Враховуючи зростаючий ризик резистентності до антибіотиків, поточні рекомендації рекомендують обмежити прийом пероральних антибіотиків та звернути увагу на використання інших методів лікування. Ізотретиноїн є високоефективним засобом для лікування акне. Він діє шляхом зменшення розміру сальних залоз, зниження виділення шкірного сала, регулювання проліферації клітин і зменшення кератинізації, він має протизапальні та імуномодулюючі властивості. Спіронолактон відіграє важливу роль у лікуванні акне – це синтетичний антагоніст рецепторів альдостерону з антиандрогенними властивостями. Андрогени мають важливий фізіологічний вплив на шкіру.Вони регулюють різні процеси, пов’язані з оновленням шкіри, ростом волосяних фолікулів, проліферацією сальних залоз, виробництвом шкірного сала та ембріогенезом. Місцеву терапію для лікування акне можна використовувати для початкового лікування та підтримки акне, як моноте-рапію (за винятком місцевих антибіотиків) або використовувати в комбінації з іншими місцевими або пероральними засобами. Risk factors include adolescence, familial predisposition, and oily skin.Although numerous acne treatments exist, ongoing efforts aim to mitigate side effects such as skin irritation, dryness, and photosensitivity, while enhancing efficacy through refined formulations and novel drugs with innovative mechanisms of action. Emerging products target various aspects of acne's multifactorial pathogenesis, offering promising advancements in treatment options.With the escalating concern over antibiotic resistance, current guidelines advocate for the judicious use of oral antibiotics and emphasize exploring alternative treatments. Isotretinoin stands out as a highly efficacious therapy for acne: it operates by reducing sebaceous gland size, decreasing sebum secretion, regulating cell proliferation and keratinization, while also possessing anti-inflammatory and immunomodulatory properties. Spironolactone also plays an important role in acne treatment; functioning as a synthetic aldosterone receptor antagonist with anti-androgenic attributes. Androgens exert significant physiological effects on the skin, regulating processes such as skin renewal, hair follicle growth, sebaceous gland proliferation, sebum production, and embryogenesis.Topical therapies serve as viable options for both initial treatment and maintenance of acne. They can be utilized as monotherapy (excluding topical antibiotics) or in combination with other topical or oral products.
