Дудченка складається з двох частин. Перша, основна частина – це автобіографічна розповідь про віковий життєвий шлях у досить складних політично-економічних умовах, у яких довелося жити і трудитися. У другому розділі викладені переживання і думки про ті історичні події, які спостерігав, а в переважній більшості брав безпосередню участь автор. Перебуваючи часто у складних життєвих ситуаціях, він вийшов на прямий шлях реалізації своїх здібностей у воєнній, науковій, педагогічній та суспільній діяльності, маючи повне право на критичний огляд подій нової історії. Життєвий шлях професора М. А. Дудченка є прикладом досягнення фізичних, розумових і морально-етичних цінностей від народження до глибокої старості. Книга профессора М. А. Дудченко состоит как бы из двух частей. В первой, основной части изложено автобиографическое сообщение в форме рассказа о его вековом жизненном пути в довольно сложных політико-экономических условиях, в которых пришлось жить и трудиться. Во втором разделе изложены переживания и мысли о тех исторических событиях, которые созерцал, а в большинстве своем непосредственно участвовал автор. Находясь часто в тяжелых жизненных ситуациях, он вышел на прямой путь реализации своих способностей в военной, научной, педагогической и общественной деятельности, имея полное право на критическое обозрение событий новой истории. A.Уникальность 75-летнего юбилейного выпуска военных врачей в 1943 году в Кировской академии следует отнести к многофакторности. Впервые в 1939 году академия имела такой большой набор слушателей – 600 человек. Учебные занятия этот курс начал 1 сентября, с которым совпал день начала второй мировой войны, а выдача дипломов врача выпускникам 1943 года совпал с днем начала Великой Отечественной войны – 22 июня 1941 года. Этот курс 2 года обучался в условиях мирного времени и 2 года – в период Отечественной войны. В первый год войны учеба проходила в блокадном Ленинграде и сопровождалась голодом, холодом, артобстрелами и авиабомбежками с гибелью слушателей. Курс участвовал в защите Ленинграда в окопах фронтовых окраин города, отражая наступающих фашистов, за что слушатели были награждены медалью «За оборону Ленинграда». Эвакуация академии в Самарканд проходила в небывалых условиях. В Самарканде занятия проходили без выходных и праздников по 10 часов в день. Полноценная учеба, согласно учебному плану и программе, сопровождалась 4-х летним обучением с присвоением квалификации врача и выдачей полноценного диплома, чего не происходило ни в одном медицинском ВУЗе страны. Выпускникам присвоили воинские звания капитана медицинской службы, вручили предписание с указаним фронта прибытия для прохождения дальнейшей службы. Из прибывших в воинские части 37 (10%) однокусников погибли в боях, 50% получили ранения и контузии ; The uniqueness of the 75th anniversary issue of military doctors of the Military Medical Academy C.M. A magnetic resonance imaging-estimated proton density fat fraction assessment was performed to determine LFC at screening and Week 24. Participants with an LFC of >− 10% at screening were randomized 1:1 to efinopegdutide 10 mg or semaglutide 1 mg, both administered subcutaneously once weekly for 24 weeks. Participants were stratified according to the concurrent diagnosis of type 2 diabetes mellitus (T2DM). Both drugs were titrated to the target dose over an 8-week time period. The primary efficacy endpoint was relative reduction from baseline in LFC (%) after 24 weeks of treatment. Results: Among 145 randomized participants (efinopegdutide n = 72, semaglutide n = 73), 33.1% had T2DM. At baseline, mean BMI was 34.3 kg/m2 and mean LFC was 20.3%. The least squares (LS) mean relative reduction from baseline in LFC at Week 24 was significantly (p <0.001) greater with efinopegdutide (72.7% [90% CI 66.8–78.7]) than with semaglutide (42.3% [90% CI 36.5–48.1]). Both treatment groups had an LS mean percent reduction from baseline in body weight at Week 24 (efinopegdutide 8.5% vs. semaglutide 7.1%; p = 0.085). It utilizes various methods, including structural, economic, and statistical analyses, to assess the viability of implementing non-profit insurance within Ukraine's healthcare system, drawing comparisons with practices in Western Europe, the United States, and Japan. Non-profit insurance companies, owned by their clients rather than investors, play a significant global role, accounting for about one-third of annual premiums and serving over 900 million policyholders. These entities blend features of financial and non-profit organizations: they manage statutory and insurance funds for financial operations while prioritizing member benefits over profit. Their unique approach allows for cost-effective premiums, tailored insurance conditions, democratic self-governance, and enhanced accessibility to a broad population. The study highlights the global relevance of non-profit insurance as a counterbalance to commercial insurers, fostering competition and mitigating unjustified price increases. It underscores the potential benefits for Ukraine in adopting this model, leveraging lessons from international practices, while considering national economic and social contexts. Collaborative integration of state, joint-stock, and non-profit insurers is proposed as an optimal solution to meet societal insurance needs. This requires not only the availability of high-tech equipment but also the effective use of traditional and modern interactive learning technologies. The article analyses students' attitudes to the introduction of certain interactive and traditional teaching methods and evaluates the effectiveness of their use in the training of future doctors. Based on the analysis of the student survey results, a fairly high assessment of the introduction of new technologies in the teaching process in the internal medicine clinic, in particular, the case-study method, was revealed. Since the highest prevalence of both coronary heart disease (CHD) and hypothyroidism is observed in the age group over 50 years old, the problem of combination of these nosologies in older people is increasing. Aim of the study is to investigate the structural and functional state of the left ventricular myocardium in hypothyroidism and CHD associated with chronic systemic inflammation. Materials and methods. To reach the objectives of the study, a randomized controlled clinical trial has been conducted in parallel groups of patients with hypothyroidism, CHD and combination of both. Overweight and obesity are defined as abnormal or excessive fat accumulation that impairs health. The global prevalence of obesity has been increasing significantly among all age groups. Furthermore, obesity is a comorbid factor for numerous diseases, including cardiovascular and periodontal pathologies. Objectives. The present study aimed to investigate the influence of overweight and obesity on the periodontium of young adults in relation to the functional state of the cardiovascular system. Material and methods. The oral health status of 132 males and females aged 18–22 years was examined. They were divided into 4 groups according to their body mass index (BMI): normal weight; overweight; class I obesity; and class II obesity. A periodontal chart, the records on oral hygiene and caries, and periodontal indices were provided for each participant. The main functional parameters of the cardiovascular system were measured before and after the dental examination. Results. Overweight or obesity did not affect the intensity of caries in young adults. The prevalence and severity of gingivitis were significantly higher in obese individuals. A moderate correlation was found between gingivitis and BMI in patients with class I and II obesity. Disturbances in the cardiovascular system function and in the autonomic nervous system tone were also diagnosed in obese patients. An impaired vascular response and significant functional changes in the cardiovascular system developed against the background of obesity. These changes show the development of subcompensation in young adults with obesity. Conclusions. In obese individuals, significantly worse oral hygiene was observed as compared to normalBMI patients. Moreover, the clinical manifestation and intensity of gingivitis in obese individuals were high even in those with satisfactory oral hygiene. S.; Городницька, Інеса Михайлівна; Скрипник, Ігор Миколайович; Маслова, Ганна СергіївнаThe article represents the results of our research on determining the influence of esophageal symptoms of erosive and non-erosive forms of gastroesophageal reflux disease on indicators of the physical and mental components of quality of life according to the Medical Outcomes Study Questionnaire Short-Form 36. The presence and degree of severity of symptoms of gastroesophageal reflux disease were analyzed using the Gastroesophageal Reflux scale Disease Questionnaire. We examined 18 patients with an erosive form and 18 patients with a non-erosive form of gastroesophageal reflux disease. It is shown that against the background of the erosive form of gastroesophageal reflux disease, daily manifestations of the symptoms of the disease are more often observed, namely heartburn (RR=6.00 (0.80-44.95); p>0.05), regurgitation and pain in the epigastric region of the abdomen (RR=2.50 (0.56-11.25); p>0.05), which leads to violations of mainly physical components of quality of life. According to the research findings, when investigating the causes of severe mitral regurgitation, especially in young and middle-aged people, one should primarily consider Barlow's disease, myxomatous mitral valve degeneration, as the primary cause (having excluded fibroelastic deficiency). According to the WHO, in 2016, more than 1.9 billion adults are overweight and about 650 million adults are obese. Numerous studies indicate the role of overweight, obesity and metabolic status on periodontal health, however, the mechanism of their development is not fully understood. The aim of the study was to explore the mechanism of periodontal disease development in patients with diverse body mass index and the mechanism of extra weight and obesity onset in these patients. The study involved 132 Ukrainian males and females, 18 - 22 years old. Body mass index was determined in all patients. Information on life history, illness and family history was collected. The assessment of dental status was conducted by indicating the oral and tongue hygiene indices and periodontal indices. Eating behaviour was assessed using the standardised Dutch eating behaviour questionnaire (DEBG) and the three-factor eating questionnaire (TFEQ R-18). Diet assessment was determined by standardized diet questionnaire. More than 60% of young people with the 1st and 2nd degree obesity had eating disorders, the most prevalent were disturbance of cognitive restraint and emotional eating components of eating behavior. The prevalence of periodontal disease was significantly higher in obese individuals, up to 87% in patients with the 1st and 2nd degree obesity. Eating behavior disturbances play a crucial role in the development of obesity in young adults and onset of gingivitis. The cause of non-plaque-induced gingivitis in patients with the 1st and 2nd degree obesity is a impaired general host response to the pathogenic factors of the oral cavity, caused by excess adipose tissue, which led to the development of chronic systemic mild inflammation. The purpose of the study was to identify and assess, with the help of various psychodiagnostic methods, the degree of severity of depressive disorders in patients with acute myocardial infarction depending on the treatment method as a prerequisite for optimal drug therapy. 73 patients with the aforementioned pathology were studied and were divided into two groups. Group I included 44 patients who underwent urgent coronary angiography with subsequent stenting of the infarct-related coronary artery; Group II – included 29 patients who received only drug therapy. Examination of patients was carried out according to Ukrainian standards, in particular, a survey according to the scale of Beck and Zung. Patients with acute myocardial infarction after urgent coronary angiography with stenting of the infarct-dependent artery were more likely to have mild and moderate depression according to the Beck scale compared to patients who received only conservative medical treatment. The reverse pattern is observed for moderate and severe depression. 68.2% of patients with acute myocardial infarction after interventional intervention and 62.1% on the background of conservative treatment according to the Zung scale showed depression of various degrees of severity, mostly mild.Документ Diagnostic and Treatment Algorithms of Ulcerative Colitis in Ukraine(KARGER, 2009) Skrypnyk, I.; Скрипник, Ігор МиколайовичQuestions concerning the diagnosis and treatment of ulcerative colitis (UC) in Ukraine are described, in recent years, there has been considerable progress in conservative therapy and new drugs have been developed that provide persistent remission of the inflammatory process after long-term application in many cases. The results of our own investigation on the efficiency of rebamipide in the complex treatment of UC patients are presented. Optimization of treatment with substitution of mesalazine in tablet or granule form, especially with an additional rebamipide prescription and a once-daily administration of budesonide, leads to an increased effectiveness of treatment and improvement of quality of life in UC patients. The high risk of toxic effects is the basis for studying of the main mechanisms of organs and systems Dox-induced injury development. The aim - to investigate the peculiarities of Dox-induced arginine/citrulline cycle changes in rats with simulated non-alcoholic steatohepatitis (NASH). Methods: The studies were conducted on 30 white non-linear adult rats, weighing 160-220 g. The rats were divided into 3 groups: | (n = 10) - rats (5 males, 5 females), on which NASH was modeled during 63 days, then from the 64th till 66th day they were injected with 1 ml of 0.9% sodium chloride solution intraperitoneally; | (n = 10) - rats (S males, 5 females), on which NASH was modeled during 63 days, continued with Dox injections intraperitoneally 5 mg/kg/day from the 64th till the 66th day; Ill (n = 10) - rats (5 males, 5 females), which received a standard vivarium diet during 63 days, from the 64th to 66th day - 1 ml injections of 0,9% sodium chloride solution intraperitoneally. NASH modeling was hold using a diet containing a 42.8% fats mixture and a 4% fructose aqueous solution. On the 67th day, animals were euthanized under thiopental anesthesia at a dose of 50 mg/kg. It is important to study results of lymphoma treatment in different countries and with different healthcare systems to ensure that there is contemporary, geographically relevant data on lymphoma. Association of progression of disease at 24 months (POD 24) with overall survival (OS) has been confirmed in several types of lymphomas. Methods:The analysis refers to all newly diagnosed lymphoma cases identified at the National Cancer Institute (NCI) of Kyiv and seven other Ukrainian centers from September 2019 to December 2023. The diagnosis was classified according to WHO 2016 criteria, and registration was conducted in a secure, specialized database. The study received approval from the NCI Institutional Review Board. POD 24 was calculated for patients with the more frequent histology and adequate follow-up. Survival analysis distinguished between early progressors and those without early progression, adjusting OS calculations based on the timing of progression to refine outcome assessments. Results: A total of 1074 patients with newly diagnosed lymphoma were prospectively registered in ULR. The male-to-female ratio (M/F) for the entire population was 0.94, and the median age was 49 years (range 18-100). B-cell non-Hodgkin lymphoma represents the majority, comprising 740 cases (68.9%). Hodgkin lymphoma (HL) accounts for 296 cases (27.5%), predominantly represented by the Nodular sclerosis classical subtype in 207 cases (70%). The most frequent subtypes among B-NHL was Diffuse large B-cell lymphoma (DLBCL) with 259 cases (35%). In HL patients 3-year OS was 82% and 98% for patients with or without POD24 (HR 5,7 (CI95% 2,1-15,3), p=0,00042). In patients with DLBCL POD24 was 42% and 80%, with a HR 8,8 (CI95% 2,1-15,4, p = 0.00073). In Primary mediastinal large B-cell lymphoma patients 3-year OS was 25% and 88% for patients with or without POD24 (HR 8,8 (CI95% 2-38,4), p= 0.00073). In patients with Primary CNS lymphoma POD24 was 24% and 86% with a HR 42 (CI95% 4,4-399), p<0,0001. The patients were enrolled into two groups: 31(52.5%) and 28(47.5%) pts in control and study to Filgrastim groups, respectively. 21(35.6%) acute myeloid leukemia (AML) pts and 10(17%) acute lymphoblastic leukemia (ALL) pts were involved in IA and IIA control groups; 17(28.8%) AML-pts and 11(18.6%) ALL-pts were contained in IB and IIB study groups, respectively. Results. We verified Gr 4 neutropenia in 17(80.95%) pts of group IA and 14 (82.35%) pts of group IB which received regimen 7+3 with/without etoposide. Duration time of Gr 4 neutropenia in group IB versus group IA was 1.5 times less, t(38)= 3.65, р˂0.001. FN developed significantly rarer in IB group with G-CSFs, X2(1, N=38) = 4.87, p˂0.03. Among the patients with ALL was registered Gr 4 neutropenia in 6 (60%) pts of group IIA and 6 (54.5%) pts of group IIB. Duration time of FN was 2.7 times less in IIB group with G-CSFs than in group IIA, t(21)= 6.62, р˂0.00001. But there was no difference in the number of FN episodes developed in IIB vs IIA groups, p˃0.05. Conclusions. Filgrastim effectively prevents the development of FN in patients with AML against the background of severe cytostatic neutropenia. The problem of clinical reasoning is a cornerstone of competence in current medical education. The best methods for teaching and evaluating clinical reasoning skills among medical students have not yet been elucidated. The introduction of the test assessment system requires professional knowledge combined with increasing practical and theoretical training of medical students. It accomplishes through the introduction of role-playing games, improving material, technical base and means of distance education.
