Документ Analysis of electromyographic indexes of temporal and masticatory muscles in patients with distal occlusion complicated by dental crowding(Aluna Publishing, 2018) Dmitrenko, M. I.; Дмитренко, Марина ІванівнаIntroduction. Rate of distal occlusion (up to 40%) and dental crowding (up to 65%) is the largest in the structure of orthodontic pathology. Aim of the study. The aim of the study was to investigate bioelectrical activity of temporal and mastcatory muscles and to set parameters of electromyographic indexes in patients with distal occlusion and dental crowding in permanent occlusion.Materials and methods Bioelectrical activity of temporal and masticatory muscles in 11 patients aged 16-24 years was determined before orthodontic treatment. Distal occlusion complicated by severe degree of dental crowding was revealed in this group of patients. These patients comprised the treatment group. The control group consisted of 10 subjects ( mean age 21,3 ± 1,25 years) with physiological occlusion.Results. It was found that functional state of temporal and masticatory muscles in patients with distal occlusion complicated by dental crowding changes during permanent dentition. It was determined that in clenching state, according to ACTIV index, temporal muscles are more active, due to the lack of activity of masticatory muscles (ACTIV = -20,73% ± 8,01%). Moreover it is worthy of notice, that functional muscle symmetry (index SIM TА/MМ) at rest in presence of distal occlusion is lower than in subjects with physiological occlusion (p <0,05). Conclusions. According to index АCTIV TА/MМ it was established the dominance of temporal muscle activity due to lack of activity of masticatory muscles in patients with distal occlusion complicated by dental crowding and it was determined lower overall muscle symmetry index (index SIM TА/MМ ) than in individuals with physiological occlusion (p <0,05).Документ Changes in biochemical parameters of oral fluid in patients during the orthodontic treatment with a bracket system under the action of a developed mucosal gel with probiotic(2018) Voronkova, A. Based upon this the objective of the current study was to determine the effect of the developed mucosal gel with probiotics on the biochemical parameters of the oral fluid of patients during the orthodontic treatment with a bracket system. The aim: Aim of our study is to determine the effect of the developed mucosal gel with probiotics on the biochemical parameters of the oral fluid of patients during the orthodontic treatment with a bracket system. Materials and methods: 45 patients at the age of 18-24, with 15 people in each group (control, main and comparison group) were examined. The main group was presented by patients who, in order to prevent dysbiosis of the oral cavity during orthodontic treatment, were prescribed local use of the developed mucosal gel with probiotic. The statistical processing of the results of the study was carried out using methods of variation statistics using the EXCEL program (the standard package of Microsoft Office). Results: According to the results of biochemical studies, it was found that the use of orthodontic treatment of mucosal gel with probiotic in patients with crowded teeth contributes to the strengthening of antioxidant protection, an increase in nonspecific resistance, decrease in inflammation and normalization of microbiocenosis of the oral cavity. The study included 55 people at the beginning of the retention stage after active orthodontic treatment. Patients of the study group were carried out functional therapy using proposed appliance during 3 month. The control group used the standard retention method. Clinical examination and EMG of masticatory muscles was performed to all patients at the beginning of the retention and in tree months; Results and conclusion. In 3 month of functional correction EMG-activity of the masticatory muscles significantly decreased and became synchronous and symmetrical in patients of the study group, that proved the effectiveness of the method of functional therapy at the beginning of retention stage after active orthodontic treatment; Целью исследования было изучение изменений активности ЭМГ жевательных мышц после функциональной терапии на стадии ретенции после стадии активного ортодонтического лечения; Обьекты и методы. В исследование включены 55 человек в начале стадии ретенции после активного ортодонтического лечения. Пациентам основной группы исследования провели функциональную терапию ортодонтическим аппаратом предложенной конструкции в течение 3 месяцев. В контрольной группе использован стандартный метод ретенции. Клиническое обследование и ЭМГ жевательных мышц были выполнены всем пациентам в начале стадии ретенции и через 3 месяца; Результаты и выводы. Disorders in functioning of hypothalamic-pituitary system can cause violations in reproductive system formation, decreasing of mineral bone mass, somatic diseases, including malocclusions. The aim of our study was to investigate the differences in EMG-activity of the masseter and anterior temporalis muscles in girls with disorders of reproductive system and without. The study group included 32 girls of puberty age with reproductive health disorders. The average age of girls in the study group was 14,66±0,3 years. All subjects were evaluated by the same examiner, according to the Research Diagnostic Criteria for TMD RDC/TMD. Recording of sEMG of masticatory muscles was performed according to the recommendations Sforza et al. and Tartaglia. Significant difference is investigated in EMG-activity of the masseter and anterior temporalis muscles between girls with disorders reproductive system and without. 93,8% adolescents of the study group had TMD group Ia and Ia plus IIa. Aim: to study electromyographic indices of orbicularis oris in patients with dental crowding in permanent occlusion. Materials and methods: Thirty four patients with malocclusion and a severe degree of severity of dental crowding (15 males, 19 females, aged 16-29 years) who underwent orthodontic examination. The treatment group was divided into three: Group Ia comprised 11 subjects with mandibular crowding (mean age 19,27 ± 1,08 years); group Ib, 10 patients with maxillary dental crowding (mean age 20,10 ± 1,60 years) and group Ic, 13 subjects with both maxillary and mandibular crowding (mean age 20,15 ± 1,45 years). The control group consisted of 10 patients with malocclusions but without dental crowding (mean age 20,70 ± 1,32 years). The findings were compared with similar indices in subjects with normal occlusion (mean age 21,3 ± 1,25 years). The index of orbicularis oris activity (ACTIV,%) was determined for each patient. A Student's t-test was used to analyze statistical difference between different groups. Results: Patients having crowding of maxillary teeth showed greater activity of muscles of the upper lip during maximum voluntary clenching (АCTІV=-0,99±7,44%). Activity of the muscles of the lower lip in patients with crowding of mandibular teeth (АСTІV=20,52±4,22%) and crowding of maxillary and mandibular teeth (АСTІV=17,93±4,33%) is prevailing. Односторонній перехресний прикус пов’язаний з асиметричною діяльністю жувальних м’язів. Електроміографія жувальних м’язів проведена 20 пацієнтам з одностороннім перехресним прикусом зі зміщенням нижньої щелепи в праву (9 пацієнтів) і ліву (11 пацієнтів) сторони у період постійного прикусу. Результатом електроміографії було графічне зображення та цифрова характеристика біоелектричної активності скроневих і жувальних м’язів. Виявлено зв’язок між одностороннім перехресним прикусом, циклом жування та відхиленням електроміографійної активності. Результати дослідження доводять участь мімічних м’язів та м’язів шиї у стисканні зубів, рухах нижньої щелепи та ковтанні, що вказує на функціональну єдність нейромускулярної складової стоматогнатичної системи. Surface electromyography (EMG) is a well-used modality and is used in dentistry to access the status of the muscles of mastication. The aim of the research was to evaluate the EMG-characteristic of masticatory muscles in patients with TMD and Angle Class II malocclusion. Materials and methods: The study comprised 23 patients with Angle Class II malocclusion and TMD. The average age of the subjects was 26,5 ± 2,3 years. Malocclusion was evaluated according to Angle classification, TMD – according to the Research Diagnostic Criteria (RDC/TMD). Registration of EMG-activity of masseter and anterior temporalis muscles was performed during maximum voluntary clenching, clenching on the right and left sides. Results: EMG-activity of masticatory muscles are characterized by: 1) increased values of EMG-activity of temporal and masseter muscles – peak and average amplitude in tests of unilateral clenching and maximum clenching (values are greater than 2000 µV); 2) in unilateral clenching of jaws (on the left or right) the increased EMG-activity was detected on the balancing side; 3) in unilateral clenching of jaws (on the left or right) the increased muscle activity on the working side with disproportional (asymmetric) load distribution between masseter and temporal muscles has been found. Затримка прорізування цього зуба може викликати морфологічні, функціональні та естетичні порушення. До них відносяться зменшення вертикальних розмірів нижньої частини обличчя, екструзію зубів антагоністів, порушення прикусу, формування фолікулярної кісти, перикоронального запалення, і резорбцію коренів сусідніх зубів, так і функціональні проблеми з боку зубо-щелепної ділянки та ін. У зв’язку з цим, питання етіології, патогенезу, ранньої діагностики та подальшої тактики лікування ретенції нижніх перших постійних молярів є дуже актуальним для сучасного ортодонта. Матеріали і методи дослідження. У статті проведений огляд та аналіз літератури щодо етіології, патогенезу, ранньої діагностики ретенції нижніх перших постійних молярів. Ре-зультати. Аналіз літератури вказав на незначну кількості досліджень щодо порушення прорізування перших моля-рів нижньої щелепи. В більшості випадків вони представлені клінічними випадками. За даними світової літератури поширеність затримки прорізування першого моляра нижньої щелепи становила від 0,01% до 0,04%. Дані літератури вказують на те, що ретенція нижнього першого моляру – це поліфакторне захворювання. До етіологічних чинників автори відносять загальні та місцеві фактори. Місцеві фактори, що викликають затримку прорізування зубів, включають порушення шляху прорізування зубів, аномалію форми зуба, гіперплазію, зміщення зачатку через кісту і пухлину, втрату простору для прорізування, фіброз ясен, ідіопатичні умови. Системні фактори включають ендокринні розлади, такі як гіпотиреоз, гіпертиреоз, гіпопаратиреоз, рахіт, краніофаціальну гіпертрофію. В літературному огляді представлені морфологічні та рентгенологічні діагностичні критерії порушення прорізування перших постійних нижніх молярів. Так, корейські вчені запропонували визначати глибину ретенції, кут нахилу та простір для їх прорізування на панорамних рентгенограмах. Objective. To collect and represent evidences of MARPE treatment outcomes in orthodontics based on the data available within systematic reviews. Materials and Methods. Search was provided via PubMed Central, Web of Science and Scopus databases, while also within Cochrane Library. Only systematic reviews written in English were considered for analysis. Primary outcomes such as success rate (percentage of cases, where required maxillary width was achieved) and maxillary expansion rate in means of midpalatal split,dental intermolar width, skeletal and dentoalveolar expansion, palatal suture opening, palatalwidth measured in millimeters or percentage were tabulated and analyzed. Secondary outcomes were presented in descriptive manner Results. Overall 12 systematic reviews were enrolled into study group of targeted publications. Among these 12 systematic reviews primary outcomes were extracted from 8 of them, while rest 4 systematic reviews were used for analysis of secondary outcomes. Succes rate of MARPE varied in the range 82.8-100%, while intermolar width increase after MARPE treatment – in the range of 4.79-6.55 mm. Conclusion. Considering data available within systematic reviews MARPE approach seems to be reliable orthodontic treatment option, which help to gain significant increase for skeletal and dental intermolar width among patients with maxillary transverse deficiency. Such treatment strategy provides better results than conventional rapid maxillary expansion and lesser periodontal negative consequences compare to surgical-assisted rapid palatal expansion, even though quality of data supporting these statements are insufficient and debatable.Документ Features of children’s prosthetics with plural adentia complicated ectodermal dysplasia (clinical case)(Полтавський державний медичний університет, 2022) Smaglyuk, L. In such patients, the shape of the face is sharply disturbed before dentures due to a significant shortening of its lower part, the protrusion of thickened lips, sagging of the bottom of the mouth as a result of low location of the tongue. Toothless children due to poor nutrition lag behind in growth, suffer from their aesthetic shortcomings, need dental prosthetics from 3 years. In the absence of teeth, the absence of growth of alveolar processes in height is defined. Due to the formation of defects of teeth and dentitions are formed differently oriented on the planes and the severity of the anomaly of the dental system or its deformation. Teeth, especially the front group, are of great importance in the formation of sound and the formation of purity of speech, the formation of facial aesthetics. No less important is the factor of psychological trauma and the formation of the child’s character. Given the complexity of the defect of the dentition of the upper and lower jaws, complete removable dentures were used to create conditions for recovery of masticatory function, swallowing, speech, activation and development of the maxillofacial muscles, recovery of facial height. In the process of making prostheses, it is important to remember the peculiarities of growth and development at the appropriate age. Structures should not impair oral hygiene and have good fixation and stabilization, especially in the case of edentulous jaws. Knowledge of the principles of orthopedic care, and types of dentures used in multiple defects of the dentition will allow dentists to timely prosthetics of patients with defects of the dentition and prevent complex deformities of the dental apparatus. Materials and methods: A group of 21 people aged from 10 to 16 years was formed to conduct the research. Clinical situation according to diagnostic criteria was identical in all the patients. The group was divided into two groups - control and experimental, which in their turn were fragmented into two subgroups. Immunohistochemical studies of mucosal biopsies were performed in accordance with the recommendations for selection. Results: Study of ratio of CD68+/CD163+ cells revealed imbalance in individuals with vestibularly impacted teeth due to higher infiltration density of CD163+ (p<0,05), compared to CD68+ of control group. In individuals with palatally impacted teeth, ratio of CD68+/CD163+ increased 3,6 times, as well as compared with control group, but due increased infiltration density of CD68+. Conclusions: In the epithelium of oral mucosa located over impacted teeth, both on vestibular and palatal surface, number of CD 68+ and CD163+ cells had no significant differences compared to control group. In biopsies of the lamina propria of mucosa over vestibularly impacted teeth, the ratio M1/M2=0,91±0,11 (p<0,05) decreases, with predominance of macrophages CD163+ subpopulation activity, and over palatally impacted teeth balance of M1/ M2 macrophages elevated (M1/M2= 2,10 ± 0,32, p<0,05), due to increased infiltration density of CD68+.Документ Interdisciplinary approach to diagnostics of malocclusions (review)(Aluna Publishing, 2019) Smaglyuk, L. Review: The human body is a biological system consisting of interconnected and subordinate elements. Any abnormalities in the operation of this system may lead to functional impairment in a single organ. This is fully applicable to dento-facial anomalies and deformations; their development is closely related to other diseases. Temporomandibular disorders (TMD), as a collective term that covers a number of clinical problems related to the masticatory muscles, occlusion, and structural components of the TMJ. Diagnostic evaluation of the muscles activity is an integral part in the planning and set of measures for rehabilitation of dental patients. Electromyography is one of the methods of objective, minimally invasive muscle examination instrumental and highly informative method of functional diagnostics of muscles, namely recording of biopotentials of muscle fibers and activity of their motor units. The aim. The aim of our study was to investigate features related to EMG-activity of masticatory muscles in subjects with TMD. Materials and methods. The study comprised 22 patients with clinical symptoms of TMD. The average age of the subjects was 29.3 ± 4.4 years. Malocclusion was evaluated according to Angle classification, TMD – according to the Research Diagnostic Criteria (RDC/TMD). Registration of EMG-activity of masseter and anterior temporalis muscles was performed during maximum voluntary clenching, clenching on the right and left sides. Results. Normal activity of the masticatory muscles is characterized by symmetrical and EMG-activity of the masticatory muscles on the left and right sides. EMG-activity of masticatory muscles in subjects with TMD are characterized by: 1) increased values of EMG-activity of temporal and masseter muscles; 2) disproportional and asymmetric muscle work of the masticatory muscles on the right and left sides. Conclusion. An analysis of 37 literature sources, which contains information about the methods of examination of patients with dento-maxillar anomalies. The functions of the tongue (swallowing and speaking) are very complex and coordinated movements that require the activation of many anatomical structures.Therefore, the disorders of the tongue during swallowing and speech are still considered by scientists as the most difficult in terms of correction and achieving of the long-term and stable results of orthodontic treatment. The study of modern trends in the diagnosis of the state of language functions in terms of understanding the functioning of the dental-maxillary area as a masticatory organ, the work and condition of the components of which depend on many systems of the human body, is relevant.Thus, the emergence of new and modifications of existing research methods undoubtedly expand the capabilities of modern orthodontists. However, this does not reduce the importance of the clinical stage of examination of patients with disorders of the maxillofacial area, including the functions of the tongue. Impacted teeth is complex anomaly of teeth eruption that requires a balanced approach not only in the differential diagnosis of its forms, but choice of rational methods of treatment. The aim of the study. Optimization of the tactics of orthodontic-surgical treatment of patients with impacted teeth based on the development and implementation of computed tomographic indices (KT) and photometric indices (FM) of opening of surgical access (OSA) to crowns of impacted teeth Objects and Methods of research. The results of treatment of 48 patients with delay of permanent teeth eruption have been analyzed. For an objective assessment of treatment results , a group of 24 (aged from 9 to 19 years old ) was formed. All 24 patients had typical clinical situation. Results. Orthodontic correction of patients envisaged, first of all, the elimination of obstacles in the way of teeth eruption, if necessary to provide space in dental arch and simultaneous treatment of associated bite malocclusions. Surgical exposure of impacted tooth crown was carried out and at the same time a triangular shaped guiding channel was formed, base of channel was at the impacted tooth and its angle finished into the dental arch. Precise dimensions and depth of the channel were preliminary planed on computed tomography slices with 3D reconstruction. Mean values of CT width (7,13±0,54mm), and length (6,42±0,78mm) of OSA and CT index (130,79±8,19%) of OSA to impacted teeth crowns were determined. Conclusion. To improve the quality of diagnosis and optimization of methodological approaches to treatment of patients with teeth impaction, we have proposed CT and FM OSA indices to the crowns of impacted teeth. Materials and Methods: 22 patients aged 13-16 years with cross bite with displacement of the lower jaw were examined. The first group consisted of 15 people with a right-sided displacement of the lower jaw, the second – 7 patients with a left-sided one. The condition of the musculoskeletal system was assessed by the position of the head, shoulders, shoulder blades, back (curvature of the spine), legs, chest shape, and abdomen. To determine the state of stability of the body in space, posturological and kinesiological tests were performed. The location of TMJ elements was evaluated on orthopantomograms. Statistical processing of the material was carried out with the help of the “Excel” license package. Results: 63.64% of patients with a cross bite have disorders of the musculoskeletal system: scoliotic posture – 40.91% and scoliosis – 22.73%. TMJ dysfunction was detected in all examined patients. It was established that the anatomical and topographic features of the joint elements depend on the side of the lower jaw displacement. The aim of this study was to specify the peculiarities of teeth size in adolescents who were diagnosed with Angle’s Class I malocclusion and display different somatotypes. Materials and methods: The study included 63 male and 66 female subjects diagnosed with Class I malocclusion by E. Angle classification (1906). Results: It has been determined that the 33rd tooth in hypersthenic female individuals is of a greater mesiodistal size than in normosthenic and asthenic body types (p <0,05).Left maxillary incisor in hypersthenic individuals is of a larger size than in asthenic and normosthenic (p The normosthenic male individuals have been found out to have significantly larger size of all canines than that in the females (p The asthenic male adolescents compared with females of the same somatotype there has been revealed the difference in the size of the left mandibular canine e (pe (p <0.05). The hypersthenic male adolescents demonstrate an increase in the size of the lateral maxillary incisors and the first right premolar (p <0,05) compared with those in female individuals of the same somatotype. The approach to treatment of patients with impacted teeth should be comprehensive and to combine surgical treatment, aimed at creating conditions for eruption of impacted teeth and orthodontic movement them into the dental arch. The article presents methods of surgical techniques planning in patients with impacted teeth and demonstrates two clinical cases of treatment with differentiated access to crowns. At their vestibular position, surgical exposure should be carried out with a wide access, and at a palatal one, the access should be limited to sparing. The developed diagnostic and treatment algorithm made it possible to achieve acceleration of bite restoration terms and increase in the effectiveness of stabilization measures in 85% of cases.
