Документ Age features of deciduous teeth caries parameters in 3-6 years old aged children(Aluna Publishing, 2022-02) Kaskova, Liudmyla F.; Sadovski, Marina O.The aim: Estimation of caries prevalence and intensity, determination for treatment necessity of deciduous teeth in 3-6 years old aged children from Poltava region for further effective development of programs for caries and its complications prevention. Materials and methods: Totally 335 children from 3 to 6 years old of preschool institutions of Poltava and Poltava region aged were examined with the signed accordance of their parents. All children were examined for the prevalence (%) and intensity of caries of deciduous teeth by the Decay Extracted Filling index (DEF index). Quantitative parameters were processed by standard statistic methods. The calculation of the highest caries intensity (Significant Index of Caries) was performed according to the D. Bratthol`s method. The level of dental aid (LDA) was assessed by the DEF index according to P.A. Leus recommendations. Treatment necessity of children was also determined. Results: The research in caries prevalence of deciduous teeth of revealed that the number of children with caries increases with their age. The most significant parameter growth is observed at the age of 4 to 5 years. Carious lesions intensity increases at the age from 4 to 5 and from 5 to 6 years most significantly. Conclusions: The most common indicator in all studied groups was a D-parameter, which indicates low parents awareness about deciduous teeth treatment necessity. That case confirms low level of dental aid (14%) and significant treatment need which is 50.7 %. Affection of molars always exceeds the affection of other groups of teeth. We did not find a significant difference in caries distribution among children of different genders. The obtained results encourage realization of sanitary and educational propagation on the awareness of parents on dental healthcare of their children.Документ Agents used for enamel remineralisation and reducing dentin hypersensitivity: a comprehensive review(Українська медична стоматологічна академія, 2020-04-10) Yanko, N. V.; Янко, Наталия Валентиновна; Янко, Наталія ВалентинівнаЗасоби ремінералізації не тільки посилюють резистентність емалі, але також можуть закупорювати дентинні канальці, знижуючи чутливість зубів. Стаття містить огляд різних доступних агентів, які використовуються для ремінералізації емалі та лікування гіперчутливості дентину. Традиційні фториди, що використовуються у вигляді розчинів, лаків, піни, гелів, зубних паст, продемонстрували низьку ремінералізуючу і десенситизуючу здатність в порівнянні з новими фторумісними продуктами. Глибоке фторування є ефективним методом зменшення гіперчутливості дентину з тривалим ефектом, його ремінералізуюча здатність використовується для лікування та профілактики початкового карієсу і герметизації фісур. Хоча сполуки фосфату кальцію продемонстрували перевагу перед традиційними фторидами в ремінералізації, їхня десенситизуюча здатність не була підтверджена достатньою кількістю досліджень. Використання біоактивного скла для ремінералізації та зменшення гіперчутливості дентину є досить перспективним, але щоб довести його ефективність необхідні подальші досліди. «Curodont» і синтетичний наногидроксиапатит із біоміметичних систем показали добрі результати, але для доведення їх клінічної ефективності потрібні додаткові спроби. Немає сумніву, що електрофорез сполук кальцію і фтору залишається найбільш ефективним методом ремінералізуючої терапії, який проводиться на стоматологічному прийомі, проте його потенційний ефект у зниженні гіперчутливості зубів повинен бути краще оцінений. Ефективність різних видів лазерів окремо або в поєднанні з ремінералізуючим агентом була підтверджена в основному дослідженнями in vitro, отже,потрібні клінічні випробування, які доведуть їх переваги. В нещодавних роботах дентинні герметики та десенситайзери, які обтурують дентинні канальці, перевершували традиційні фториди і синтетичні наногидроксиапатити. В той же час дані про ефективність сполук фосфату кальцію та глибокого фторування над дентинними герметиками та інгібіторами нервової чутливості є суперечливими і вказують на те, що необхідні додаткові досліди для поглиблення наших знань про недостатньо вивчену область зниження чутливості дентину. Незважаючи на нові досягнення, ідеального ремінералізуючого засобу не існує. Materials and Methods: A comparative study was conducted among 608 pupils of secondary schools in Kremenchuk, who consumed drinking water with fluoride concentrations in the lower limits of the norm, and 1214 pupils of secondary schools in Poltava, who consumed drinking water with fluoride concentrations in the optimal upper limits of the norm. Results: The rates of caries in permanent teeth in children living in a region with fluoride concentrations in drinking water in the optimal-upper normal range are several times lower than in children of the same age living in a region with fluoride concentrations in drinking water in the lower normal range, and a significant increase in the prevalence and intensity of caries is observed from 7 to 9 to 12 years of age, as teeth after eruption are most vulnerable to caries. In a region where the fluoride concentration in drinking water is in the optimal-upper range of the norm, children with early forms of fluorosis have the lowest prevalence and intensity of caries. Conclusions: Endemic features of the region directly affect the prevalence and intensity of the caries process. An increasing number of systemic diseases and conditions have been shown to be reflected diagnostically in saliva. Along with these developments are technology advancements that have overcome barriers to the widespread implementation of salivary diagnostics. These barriers include technological problems related to achieving high sensitivity, high specificity, miniaturization, high throughput, automation, portability, low cost, high functionality and speed; overcoming them has enabled researchers to detect and measure multiple disease markers. These challenges have largely been met as a result of careful studies of salivary gland physiology, development of sensitive amplification methods (ELISA, RT-PCR), and education of the scientific community in the methodology for obtaining and dealing with salivary samples. Such miniaturized saliva based diagnostic technologies as OFNASET will enable the use of minute amounts of bodily fluids to yield critical patient information that reflects health and disease status. These technologies will allow clinicians to achieve real-time and simultaneous assessment of multiple diseases. Diagnostic molecular targets in saliva are the salivary proteome and the salivary transcriptome. Salivary analysis can be done for the diagnosis of the oral, infectious, systemic, autoimmune, and hereditary diseases, malignancy, monitoring of levels of hormones, detection of drugs, bone turnover marker in saliva, forensic evidence, genetic disorders, and occupational and environmental medicine. The ability to monitor health status, disease onset and progression, and treatment out come through non-invasive means is a highly desirable goal in health care promotion and delivery. An initiative catalyzed by the National Institute of Dental and Craniofacial Research (NIDCR) has created a roadmap to achieve this goal through the use of oral fluids as the diagnostic medium to scrutinize the health and disease status. The recent advances in oral fluid biomarker diagnostics has been fuelled by novel molecular approaches (e.g. proteomics, transcriptomics and genomics) and metagenomic analyses that have broadened the discovery of microbial pathogens associated with systemic and oral diseases. Similarly, these experimental approaches have been successfully used in the diagnosis of non-infectious systemic and oral conditions (e.g., cancers, autoimmune diseases, renal disease and diabetes). Diagnostic kits for S. mutans and Lactobacillus counting and salivary buffering capacity widely use in dental practice. The p53 antibodies and salivary defensin-1 level can be detected in the saliva of patients diagnosed with oral squamous cell carcinoma. Such biomarkers as Matrix metalloproteinase-8, Matrix metalloproteinase-9, Interleukin-1β (IL-1β), Interleukin- 6 (IL-6) alone and in combination, S100 proteins, Lactoferrin, Macrophage inflammatory protein-1α, 8-hydroxy- deoxyguanosine, and periodontopatogenic bacteria were successfully implementated in periodontal disease practice due to ELISA and PCR. The future of this field will depend on further validation of disease (and stage) specific biomarkers and their incorporation into state-of-the-art, multiplex assays that are versatile, quantitative, reliable, sensitive, specific, rapid, robust, and cost effective for broad implementation in diagnostic programs. Возможность контролировать состояние здоровья, начало, прогрессирование, и лечение за- болеваний с помощью неинвазивных методов является важной задачей в продвижении и оказании меди- цинской помощи. В этой статье представлены биологические и биохимические маркеры различных заболе- ваний в слюне и научно обосновано такое её использование. Технологии, работающие со слюной, прошли путь от ИФА и ПЦР до такого миниатюрного автоматизированного портативного теста как OFNASET c высокой пропускной способностью и сравнительно низкой стоимостью. Можливість контролювати стан здоров’я, початок та прогресування, а також лікування захво- рювання через неінвазивні методи є дуже важливим завданням у просуванні і наданні медичних послуг. The study was conducted with a sample of 375 parents of children aged 4-7 years selected randomly from public schools, kindergartens, and the pediatric public dental clinic after obtaining informed consent. A questionnaire was used to assess the attitudes and practices of parents toward their child's dietary habits, oral hygiene measures, and dental information during the war. Most of the parents had satisfactory knowledge and positive attitudes regarding their children's oral health. The internally displaced children experienced more dental problems (P= .02) and consumed more food (P= .008) than the local population. The parents with university education and professional occupations were more involved in supervising their children tooth brushing (P= .0001). 57.07% of children brush their teeth once a day. 47.47% of parents supervised their child's oral hygiene, and 47.73% needed more time to implement additional oral hygiene measures. The surveyed Ukrainian parents displayed satisfactory attitudes and practices towards oral health, but they lack awareness of their children`s oral hygiene measures, which could be related to the inconveniences faced both by parents and children during wartime.Документ Changes in indices of chemical composition and mineralizing properties of oral fluid in children with intestinal dysbiosis(Українська медична стоматологічна академія, 2020) Kaskova, L. The results obtained allowed us to conclude that in the oral fluid of 2-3 years old children with intestinal dysbiosis there was a decrease in the content of calcium and inorganic phosphorus compared to similar indicators in healthy children. Thus, the rate of ionized calcium in the oral fluid was 0.23±0.01 mmol/l, which is 1.6 times less than the corresponding indice in children of the control group (0.37±0.02 mmol/l). The content of inorganic phosphorus in the oral fluid in children with intestinal dysbiosis was 4.38±0.03 mmol/l, and in somatically healthy children it was 4.66±0.03 mmol/l, which is correspondingly lower by 0.28 mmol/l. They also showed a decrease in the mineralizing potential of the oral fluid, which indicates the tendency of the erupted teeth to the enamel demineralization and increasing the prevalence of dental caries. The distribution of children by the level of mineralizing potential indicated that only 6.6% of children with intestinal dysbiosis have a high rate, which is more than 2 times worse than in the control group. Measures aimed at preventing the occurrence of dental diseases, namely dental caries, should be started at a very early age. Based on previous experience of hygiene lessons, for maximum perception of information by students, volunteers are guided by this theory, using the most correct and adapted style of learning, presentation of information, namely: visualization, audio, kinesthetic and digital types. Assessing and analyzing childrens;s reaction to the submission of material by volunteers, some children react quickly and interact with lecturers, while others lag behind. This is not due to the mental activity of students, but as a result of different types of temperament of children. Temperament does not affect a persons mental abilities, but is manifested in motility, physical activity, emotionality. This is a broader concept than character. Temperament is an innate feature of each persons character. It determines behavior, emotional state, all mental processes. In psychology, there are 4 types of childrens temperament: choleric (easily excitable, restless, emotional, energetic, prone to rapid mood swings, sometimes even aggressive children, it is difficult to predict how such a child will behave in a new environment), sanguine (sociable, restrained, balanced, smiling, cheerful, inquisitive, active child (who is almost always in a good mood), phlegmatic (restrained, diligent, thoughtful, but silent child, who is peaceful, tries to avoid conflict situations and not to violate her usual routine), melancholic, closed and gloomy, who is not confident in their abilities and needs help when you need to make the right choice). Consequently, sanitary and educational work among children of organized children's groups is the main one for the «Posmishka» volunteer squad, since, despite the modern assortment of dental products, modern medical technologies, and the availability of passive information, the prevalence of dental diseases is only increasing. The WHO estimates approximately 15% of children in their second year of life are already afflicted with dental caries. This indicator increases with age and in the period of changeable occlusion it can reach 90% depending on the territory where children reside (3,4,5,6,7). Investigation of caries indicators in children during the period of changeable occlusion is an important issue since eruption of the first permanent molars occurs, which are poorly mineralized, and have a specific relief of the masticatory surface (8). D.Нами було проведено статистичну обробку із застосуванням кореляційного аналізу даних анкетування та об’єктивного обстеження дітей та анкетування батьків з метою їх обізнаності з точки зору профілактики карієсу та способу життя. Результати показали, що на виникнення карієсу у дітей 7-12 років мають вплив соціально-гігієнічні фактори. Під час анкетування батьків виявлена дія сукупності чинників: відсутність звернення до лікаря стоматолога з метою профілактики, чистки зубів, нехтування додатковими предметами і засобами гігієни та неінформованість про них, регулярність і збалансованість харчування, недостатня поінформованість щодо причин виникнення карієсу, відсутність медичної інформації про це, звернення до лікаря лише при наявності больових відчуттів, тривожність під час відвідування лікаря-стоматолога. Анкетування дітей показало вплив сукупності факторів на виникнення карієсу, серед яких відсутність звернення до лікаря стоматолога з метою профілактики, недостатня поінформованість щодо причин виникнення карієсу, звернення до лікаря-стоматолога лише при наявності больових відчуттів, тривожність під час відвідування лікаря-стоматолога, недостатня інформованість про предмети і засоби гігієни порожнини рота. Сукупний вплив батьківської інформованості та батьківських звичок на розвиток карієсу у дітей значно вищий, ніж вплив відповідних особливостей дітей. Отже, аналіз чинників виникнення карієсу у дітей 7-12 років показав наявність тісних зв’язків між аліментарними, поведінковими звичками батьків і відповідними звичками дітей та розвитком патологічних змін у стані ротової порожнини дітей, що, в свою чергу, призводить до поширення каріозних уражень. The aim of this paper is to determine the dental health indicators in the Chernyakhov population (the end of the IVth c. AD) from Ukraine. The paper also attempts to identify the relationship between sex and these indicators in the studied population. The sample analysed consisted of the dental remains of 25 adult individuals (11 males and 14 females) and 8 children individuals excavated from the cemetery at the archaeological site of Shyshaki (Poltava region of Ukraine). A total of 760 teeth were examined for caries, tooth wear and calculus clinically, and 647 teeth of adults were observed radiographically. The studied population presents frequency of caries in individuals of 12.5%. This indicator in adult male and female dentition is 42.86% and 14.0% respectively, and 25% in children. The presented population frequency of caries teeth was 0.88%. Females presented higher caries rate than males (2.72 % females vs. 0.36 % males) (p<0.05). The most frequent were caries lesions of cementum-enamel junction (40%) and combined lesions of crown and root (40%) with no difference in terms of sex. Frequency of dental wear in adult individuals and 8-10 year-old individuals was 100%. The studied population represented TWI (tooth wear index) in adults (2.26), which increased with aging, but the difference in terms of sex is insignificant. Calculus was observed in 64.28% of the females and 63.33% of the males, showing no statistical difference (p>0.5). These findings confirm a very low rate of caries teeth in the territory of Shyshaki during the Late Roman period, which could be related to regional diet and concentration of fluorine in drinking water. The Chernyakhov population presents high dental wear and similar frequency of dental calculus when compared to the population from Roman Britain. Females presented significantly higher frequency of caries than males, whereas no significant sex differences were found regarding dental wear, calculus, and localisation of caries lesions. A month after the appointment of preventive measures, this indicator improved to satisfactory in children of group 3 and to good in group 4 (who were assigned professional hygiene twice a year, individual hygiene using calcium paste, the use of calcium supplements and those that increase the body's protective properties and amounted to 1.68±0.06 points and 1.5±0.05 points, respectively. 6 months after the first survey, oral hygiene worsened in all children relative to the second survey, but it was better than to take preventive measures. The same picture was observed when determining the hygiene index according to the Green–Vermilion. The course of caries in 7-9-year-old children occurs against the ground of decrease of pH and saliva microcrystallization indices, and it is caused by reduced dental resistance to caries which is evidenced by the results of examination of the enamel acid resistance according to the enamel resistance test. The analysis of the obtained results demonstrated existence of a reverse correlation in 7-9-year-old children with caries between the index of enamel acid resistance and mineralizing potential (r= 0.73 – strong index) and рН (r= 0.66 – reverse considerable) of the oral fluid. These children presented direct strong connection between pH index and saliva mineralizing potential (r= 0.79). The EIA is presented by 41 children skeletons, three of which were Cimmerian (the 9th - the 7th centuries BC) from the territory of contemporary Poltava region; 38 skulls from the territory of contemporary Poltava region and Crimea represented Scythian period (the 7th - the 3d centuries BC). Remains of 24 children from the Medieval populations were also examined, three of which were the ancient Hungarians from the Poltava region (the 8th - the 9th centuries AD), 6 Khazars from the Kharkiv region (the 8th - the 9th centuries), 1 child related the Old Rus culture from the Kyiv region (the 9th century), and 14 representatives of the nomadic populations in the Golden Horde period (the 13th - the beginning of the 15th century) from the Poltava and Zaporizhzhya regions. Taking in consideration the letter archaeobotanical studies we suggest that there were no major changes in the plants exploited during all the studied periods. The frequency of carious lesions in children from the Medieval populations (8.3% in individuals, 0.5% in deciduous teeth, and 0.4% in permanent teeth) is only slightly higher than those from the EIA period (2.4% in individuals and 0.2% in deciduous teeth). These indexes were not larger those of majority of European populations dated to the same historic period. Further isotopic, chemical and palaeobotanical studies of the additional sites, with sufficient sample sizes, allow us to learn so much more of the cariogenic factors in children of the past populations from the Ukraine area.Документ Gingival health in children in the different phases of acute lymphoblastic leukemia(Sciendo, 2019-10-01) Kaskova, L. While periodontal alterations in such patients are widely known, there were no studies evaluating gingival health from the time of diagnosis to the remission phase. In our study, we, therefore, analysed the frequency of periodontal diseases and the gingival indices in the different phases of ALL in children. Children aged 7-15 years were involved into the cross-sectional study. Therein, 160 children with ALL were divided into three groups: L1 – 50 children examined before the initiation phase, L2 – 50 children examined after 1 month of the chemotherapy, L3 – 60 children examined in permanent hematologic remission. The control (HC) included 150 healthy children. The L1, L2 and L3 groups had significantly worse gingival indices and frequency of gingivitis than the HC group (p < 0.0001). Frequency of gingivitis increased from before the initiation to the remission phase, but significantly only in 7-11 year-old children (p = 0.0004). Gingival indices increased after 1 month of chemotherapy (p < 0.0001), but decreased in the permanent remission phase (p < 0.0001). Dental diseases, as a rule, arise under the influence of various factors, which may include: bad oral hygiene, somatic diseases, biogeochemical features of the environment, socio-economic living conditions, quality and amount of nutrition of the population, organization of primary prevention.Material and methods. The study of dental status was conducted according to the standard method in teenagers studying in different educational establishments of Chernivtsi (school, college, university). The prevalence (%) and intensity of caries (CFR) was determined in each age group.Results and discussions. The obtained data indicates on a difference in the rate of caries intensity in teenagers of different forms of education and in different age groups. Teenagers attending school are likely to have lower indices than college students.Conclusions. Given the intensity of caries, we can assume that teenagers attend-ing college do not pay enough attention to the condition of the hard tooth tissues, which leads to an increase in the number of affected by caries teeth in each studied age period. A.Influence of the treatment and preventive measures on the physical indicators of oral liquid in children with chronic catarrhic gingivitis and diabetes mellitus The aim of our study was to find out the effect of our proposed treatment and prevention complex, which included oral administration of «Kvertulin» complex drug, «Imupret» drops, «Pikovit» multivitamin drug and «Exodent» local irrigation of the oral cavity with a solution of tooth elixir that influence on the rate of salivation and viscosity of oral fluid in children with chronic catarrhal gingivitis and diabetes mellitus.The treatment and prophylactic measures had positive effect on the homeostasis of the oral cavity, which assisted to reduce the viscosity of the oral fluid, increase the rate of salivation, as evidenced by observation of patients for 6 months. High outcomes of students who had tests with right answers to control module do not allow objectively assess their level. It is necessary to familiarize students with conclusion tests each lesson because data of classroom assessment motivate students to prepare every day and can identify teaching and learning methods that need to be changed or developed;Для того, щоб порівняти якість знань, отриманих при поточному та підсумковому оцінюванні, студенти-стоматологи були розділені на три групи згідно ознайомлення з тестами. Студенти, які мали тести з правильними відповідями до підсумкового контролю, отримали високі результати заключного контролю, що не дає можливості адекватно оцінювати рівень знань студентів. Найбільш доцільним є вирішення заключних тестів під час практичних занять за окремими темами, що мотивує студентів до щоденної підготовки та спонукає викладача до активної роботи із студентами;Для того, чтобы сравнить качество знаний, полученных при текущем и итоговом оценивании, студенты-стоматологи были разделены на три группы согласно ознакомлению с тестами. Студенты, ознакомленные с тестами с правильными ответами к итоговому контролю, получили высокие результаты заключительного контроля, что не позволяет адекватно оценить уровень знаний студентов. Наиболее целесообразным является решение заключительных тестов во время практических занятий по отдельным темам, что мотивирует студентов к ежедневной подготовке и стимулирует преподавателя к активной работе со студентами.
