Architectonics of bone tissue of the alveolar process of the mandibular in a patient with deformative arthrosis of the temporomandibular joint, chronic periodontitis on the background of polyarthrosis (clinical case)

Abstract

This article considers the problem of diagnosing the density of bone tissue of the alveolar process of the mandible, its architectonics in a patient with deforming arthrosis of the temporomandibular joint, chronic periodontitis on the background of polyarthrosis. The density of the alveolar bone of the mandible was determined in the areas of the corners of the alveolar bone of the TMJ heads and frontal teeth. The results were evaluated in units of the Hounsfield scale (UH) using the classitication of bone tissue types according to C. E. Misc. They indicate that in the area of healthy periodontal tissue the bone type is D2. In chronic generalized periodontitis of the II degree of severity. in the area of teeth 4.3, 4.2, 4.1, 3.1, 3.2, 3.3 bone type – D3. Bone density in the area of incisors – D2 bone type. According to CT – 3D density of the alveolar process in the areas of the corners of the lower jaw – right bone type – D2, left – bone type D2. Density indicators in the areas of the articular heads: right and left correspond to D 4 type of bone tissue, which is characteristic of osteoporosis. The frontal area of the alveolar process near teeth 31, 32, 33 is bone type D3, located under the periodontal pockets. The specified diagnostic algorithm was carried out by us for the necessary individual planning of appropriate stages of treatment for the patient.

У цій статті розглядається проблема діагностики щільності кісткової тканини альвеолярного відростка нижньої щелепи, її архітектоніки у пацієнта з деформуючим артрозом СНЩС, хронічним пародонтитом на тлі поліартрозу. Щільність кісткової тканини альвеолярного відростка нижньої щелепи визначали в ділянках кутів альвеолярного відростка голівок СНЩС, фронтальних зубів. Отримані результати оцінювали за одиницями шкали Хаунсвільда (Hounsfield), (UH) з використанням класифікації типів кісткової тканини за C. Е. Misсh. Вони свідчать, що в ділянці здорового пародонту тип кістки – D2. При хронічному генералізованому пародонтиті ІІ ступеня тяжкості. в ділянці зубів 4.3, 4.2, 4.1, 3.1, 3.2, 3.3 тип кістки – D3. Щільність кісткової тканини в ділянці різців – D2 тип кістки. За даними КТ – 3Д щільність альвеолярного відростка в ділянках кутів нижньої щелепи – правого тип кістки – D2, лівого – тип кістки D2. Показники щільності в ділянках суглобових голівок: правої та лівої відповідають D4 типу кісткової тканини, що характерно для остеопорозу. Фронтальна ділянка альвеолярного відростка поблизу зубів 31, 32, 33 – тип кістки D3, локація під пародонтальними кишенями. Зазначений діагностичний алгоритм проводився нами для необхідного індивідуального планування доцільних етапів лікування пацієнтки.
