There is a theoretical rationale for the possible role of circadian rhythm disturbances, manifested by a change in eating behavior, a decrease in sleep duration, and the individual chronotype of patients in developing obesity and some autoimmune diseases. However, this issue remains insufficiently studied and proven. The purpose of our study was to identify the correlation between the circadian rhythm and indicators of changes in the chronotype of a person's working capacity and indicators of the severity of the course of psoriatic disease, the degree of obesity, and the degree of the negative impact of the disease on the quality of life of patients. The results of the correlation analysis showed a very high negative correlation between the chronotype of patients' working capacity and BMI indicators and a high negative correlation between the indicators of the PASI index, ACTIVITY, and the chronotype of patients' working capacity.Документ Research on the relationship between the severity of the course of psoriasis and metabolic syndrome and the level of indicators of systemic inflammation(Полтавський державний медичний університет, 2023) Yemchenko, Y. V.; Ємченко, Яна Олександрівна; Кайдашев, Ігор Петрович; Іщейкін, Костянтин Євгенович; Безега, Олена ВікторівнаIn patients with moderate psoriasis with concomitant metabolic syndrome, the indicators of general clinical and biochemical blood tests and carbohydrate and lipid metabolism were determined, and indicators of systemic inflammation were assessed by the level of susceptible C-reactive protein, ceruloplasmin, interleukin-6 and tumor necrosis factor-α. A laboratory study found that in patients with moderate psoriasis with concomitant metabolic syndrome, an important role is played by the systemic inflammatory process, which is accompanied by the development of insulin resistance, impaired lipid and nitrogen metabolism, and regulation of vascular tone. Despite the widespread prevalence of psoriasis and a large number of studies on this problem, there is still no single view of the pathogenesis of this dermatosis. For an objective understanding of the pathogenesis of psoriasis, it is necessary to consider the insufficiently studied comorbidity of this pathology. Recently, an undeniable link between psoriasis and obesity has been proven. Taking into account the current data on the role of systemic inflammation underlying the development of both psoriasis and obesity, the study of molecular mechanisms of its development, and taking into account the role of proinflammatory nuclear transcription factors, thiazolidinediones are the pathogenetically determined drug of choice for the treatment of these diseases. In this study, we determined the efficacy of using 45 mg of pioglitazone once daily for six months in the complex treatment of patients with moderate vulgar psoriasis with concomitant alimentary obesity of I-II degree by clinical and immunological studies of systemic inflammation. Analyzing the study results, it was found that long-term use of 45 mg of pioglitazone was effective, led to a decrease in systemic inflammation, and contributed to a milder course of psoriasis in case of recurrent relapse of the disease.
