E.; Іщейкін, Костянтин ЄвгеновичIn Ukraine, annually, 6-8 thousand occupational diseases are registered, 70% of those are bronchopulmonary pathologies (BPP) [1]. The nature, clinical course and complications of BPP depend on the composition of the industrial aerosols, their aggressiveness and genetic predispositions and immunological characteristics of the individual [2]. In living organisms, there are various systems that protect from exogenous damaging agents, such as DNA repair [3-7]. There are 4 basic DNA repair systems: base-excision repair (BER); nucleotide excision repair (NER); double-strand break repair (DSBR), which is divided into homologous recombination (HR) and non-homologous end joining (NHEJ); mismatch repair (MMR). Most of the damages to DNA (up to 70%) are removed by BER [8]. The genes encoding for BER are characterized by a high level of polymorphism, which, due to changes in the activity of reparative enzymes, can affect individual sensitivity to the actions of various genotoxic agents, including tobacco smoke and industrial aerosols. The XRCC1 gene (X-ray-repair cross-complementing group 1) is localized on the 19-th chromosome (19q13.2). A protein that it encodes, regulates the regeneration of DNA molecules that have been damaged by ionizing radiation and alkylating agents [3, 8-9]. The XRCC3 gene (X-ray-repair cross-complementing group 3) is involved in recombinant DNA repair and double-stranded DNA breaks [10]. The protein products of the NER genesare involved in the removal of the damaged nucleotides with the subsequent restoration of the structure of the DNA molecule, through the recognition and correction of basal cross-linking [11]. The XPD protein (xeroderma pigmentosum group D) functions at the beginning of the synthesis of all proteins as a subunit of the complex protein TFIIH, a complementary factor of RNA polymerase II [5, 6]. The main function of the ERCC1 gene (Excision repair cross complementing 1) is nucleotide recovery [11]. Variants of DSBR errors lead to different types of mutations and chromosome rearrangements that induce a genome instability and carcinogenesis [3, 6]. The gene XRCC7 (X-ray-repair cross-complementing group 7) is located on the 8th chromosome (8q11), encodes a protein which is a large catalytic subunit of the DNA-PC complex (DNA-PKc), which forms an active protein kinase with Ku and initiates recovery by NHEJ [8, 13, 14]. The ataxia-telangiectasia mutation (ATM) gene is localized on the 11th chromosome (11q22-23), encoding the DNA-dependent proteincanase, localized mainly in the nucleus. The carriers of mutant alleles are characterized by sensitivity to radiation, multiple defects in development, predisposition to oncology [15]. A special place among the DNA repair systems belongs to the MMR, thanks to which it is possible to preserve genetic information even when there is a high number of mutations. The MLH1 gene (mutL (E. coli) homolog 1) is located on chromosome 3, encoding a protein that regulates the replacement of improperly coupled DNA bases and is inactivated by methylation [16]. In the XX century, in the developed countries of the world, the number of people aged 55 and over increased from about 4 to 13%, topto there is a tendency for these indicators to grow [3,6]. It is this demographic situation that determines the increased attention to research in the field of aging, especially in relation to its potential age-related changes [3,6,11]. Skin aging is part of the overall biological aging process and is governed by the same laws. With age, diseases approach, the colors of life fade, physical activity and sexual desire decrease, chronic fatigue increases [3,5,6,8]. The reason for these problems is the extinction of the function of the reproductive system against the background of general aging of the body [7,8,11]. The skin is the first to react to changes in the body, and also takes on the action of exogenous (climatic changes, UV, lifestyle, etc.) and endogenous factors [1,2,5]. An important endogenous factor in skin aging, especially in women, is the state of the endocrine system [4,5,8,9]. Despite the significant preva-lence of psoriasis and a large number of studies on this problem, there is still no single view on the pathogenesis of this dermatosis. To objectively understand the pathogenesis of psoriasis, it is necessary to take into account the insufficiently studied comorbidity of this pathology. Recently, an indisputable link between psoriasis and obesity has been proven. The scientific literature widely covers the issue of identical pathogenetic mechanisms of inflammatory processes in psoriasis and obesity. Given the current data on the role of systemic inflammation underlying the development of both psoriasis and obesity, the study of molecular mechanisms of its development and taking into account the role of proinflammatory nuclear transcription factors, thiazolidinediones are the pathogenetically justified drugs of choice for treatment of these diseases. In this study, we determined the effectiveness of using 15 mg of pioglitazone once a day for 6 months in the treatment of patients with extensive psoriasis vulgaris of moderate severity and concomitant grade І-ІІ alimentary obesity by clinical and immunological examination of systemic inflammation. While studying single-nucleotide gene polymorphisms of occupational diseases, many complicated bioethical questions arise regarding the confidentiality of personal data, the choice between the profession chosen and the risk to one's own health. The recent increase in the incidence of comorbid psoriasis and obesity leads to severe, atypical, disabling, and treatment-resistant forms of dermatosis, which significantly impair the quality of life of patients, reduce the efficiency and social activity of patients, thus acquiring not only a medical but also a social significance. There is a theoretical justification for a possible role of circadian rhythm disorders, manifested by changes in eating behavior, reduced sleep duration, and individual chronotype of patients in the development of obesity, chronic low-intensity inflammation, and some chronic autoimmune diseases such as psoriasis. However, this issue remains understudied and insufficiently substantiated. Therefore, the aim of our research was to identify the correlation between the circadian rhythm in terms of changes in the chronotype of human working capacity and indicators of systemic inflammation in patients with extensive psoriasis and concomitant alimentary obesity. The results of correlation analysis showed a high negative correlation between the working capacity chronotype in patients and the level of IL-33, as well as a medium negative correlation between the working capacity chronotype in patients and the level of IL-6 and CRP. Therefore, changes in the circadian rhythm to the evening type of human working capacity chronotype may increase the level of systemic inflammation in patients with extensive vulgar psoriasis and concomitant alimentary obesity. There is still no consensus on the pathogenesis of this dermatosis. To objectively understand the pathogenesis of psoriasis, it is necessary to take into account the insufficiently studied comorbidity of this pathology. Numerous studies have shown a clear link between psoriasis and obesity. Systemic inflammation is a common link in the pathogenesis of obesity and psoriasis. In this study, we determined the effectiveness of standard treatment for patients with psoriasis and concomitant grade I-II alimentary obesity, by clinical and immunological examination of systemic inflammation. This testifies to the approximately equal effectiveness of both methods during the treatment of early forms of active syphilis. Thus, in patients who were treated with benzathine benzylpenicillin in combination with doxycycline, the negation of the complex of serological reactions to syphilis occurred faster compared to patients who were treated only with benzathine benzylpenicillin. This testifies to the favorable long-term results of the method being studied and its high effectiveness for medicine treatment of early syphilis. G.; Іщейкін, Костянтин ЄвгеновичThe purpose of the study was to establish the toxic effect of Fe2O3 nanoparticles of different sizes on the cardiovascular system after long-term exposure. Experimental rats were injected intraperitoneally 5 times a week with colloidal solutions of Fe2O3 (with particles of 19, 75 and 400 nm) in a dose of 1 mmol (0.112 mg/ml of iron) per 100 g of the rat's body weight, a total of 30 injections. To establish the cardiotoxic effect of Fe2O3 colloidal solutions, haematological, biochemical and electrocardiographic parameters were determined and compared with the data in the control group. It was established that the long-term intake of Fe2O3 solutions caused disruptions in the process of hemoglobin synthesis; changes in blood coagulation parameters with signs of hypercoagulation; damage to liver and heart cells and increased activity of AST, ALT, AP, LDG, total CK and CK-MB enzymes. The cardiotoxic effect due to the exposure to Fe2O3 colloidal solutions was manifested by disruptions of the contractile and conduction function of the heart. It was noted that the Fe2O3with 19 nm NPs had a more pronounced effect on the cardiovascular system of rats. Метою дослідження було встановлення токсичної дії наночастинок Fe2O3 різного розміру на серцево-судинну систему за умовиїх тривалого надходженняв організм щурів. Піддослідним щурам уводили в очеревину 5 разів на тиждень колоїдні розчини Fe2O3 (з частинками 19, 75 і 400 нм) удозі 1 мл (0,112 мг/мл заліза) на 100 гмаси тіла щура, всього 30 введень. Для встановлення кардіотоксичної дії колоїдних розчинів Fe2O3 визначали гематологічні, біохімічні та електрокардіографічні показники, які порівнювали з даними в контрольній групі щурів. Встановлено, що тривале надходження розчинів Fe2O3 в організм щурів викликало порушення процесу синтезу гемоглобіну; зміни показників згортання крові з ознакамигіперкоагуляції;ушкодження клітин печінки, серця та збільшення активності ферментів АСТ, АЛТ, ЛФ, ЛДГ, КК загальна і КК-МВ. Кардіотоксичний ефект за впливу колоїдних розчинів e2O3 проявлявся порушенням скорочувальної та провідної функції серця. Recently, an indis-putable link between psoriasis and obesity has been proven. Obesity has been found to increase the risk of many dis-eases, including psoriasis. The literature has broadly highlighted the question of the pathogenetic mechanisms of in-flammatory processes in psoriasis and obesity that form a vicious circle at the level of the immune system, which must be broken for the successful treatment of these diseases. In this research, we studied the clinical presentation of the disease, measured anthropometric parameters, determined the grade of obesity by BMI, analyzed the history of life and disease, and conducted clinical and biochemical blood tests. The results of the study revealed that alimentary obesity in patients with psoriasis leads to metabolic disorders, complicating the course of dermatosis, which leads to a worsening of the DLQI of patients, the inefficiency of standard methods of therapy and frequent exacerbations of psoriatic disease. In the study, experimental subgroups of laboratory rats were administered complex chemical food additives daily once orally for 1, 4, 8, 12, 16 and 20 weeks. Cognitive functions were assessed using the “Extrapolation Escape” test. The analysis of the obtained results showed that the estimated indicators at the initial stages of the experiment (up to 8 weeks) had positive dynamics, which was characterised by a reduction in the time of occurrence of reactions. After 12 weeks of introducing the complex of food additives, negative changes occurred, which were manifested by a gradual postponement of the studied indicators. The worst indicators were recorded after week 20 (significant at p<0.05. It was found that the use of a complex of chemical food additives leads to blunted reactions under conditions of acute stress, as well as impaired ability to find a way out of the acute stress situation.Документ Genetic predisposition to bronchopulmonary pathology(Українська медична стоматологічна академія, 2020) Andrushchenko, T. Є.; Іщейкін, Костянтин ЄвгеновичWe studied 215 people who work in harmful industries, 90 of which had a history of bronchopulmonary pathology of occupational etiology and 125 of them without such pathology. The following polymorphisms of DNA repair genes were identified in real-time using polymerase chain reaction: XPD (rs13181, rs799793), ERCC1 (rs11615), XRCC3 (rs861539), XRCC1 (rs25487), ATM (rs664677), XRCC7 (rs7003908) and MLH1 (rs1799977). We studied the frequency distribution of the genotypes of DNA repair genes with the subsequent integral statistical analysis of the data obtained. We applied the enzyme immunoassay to evaluate the severity of systemic inflammation. We determined the concentration of IL-33, interleukin-6, and high sensitivity C-reactive protein in the serum. PASI was used to assess the severity of psoriasis. We used BMI to determine the extent of alimentary obesity. Cryoglobulinemic syndrome (CGS) is an immune-related process caused by cryoglobulins composition in the blood in small or medium vessels. Most frequently, CGS is triggered by lymphotropic viruses, immune-related and oncological diseases.Objectives. Studying the immune-related mechanisms, molecular and genetic characteristics of patients with systemic autoimmune diseases (SAD) against the cryoglobulinemic syndrome.Methods. Among 380 patients with SAD, in 94 (57.6%) progressing chronic EBV-infection was diagnosed, and 22.1% of patients were diagnosed with progressing chronic HSV 1/2-infection based on DNA virus identifi cation through the polymerase chain reaction (PCR) in three biological media (blood, saliva, mucus membrane scraping). Results. Analysis of the cryoglobulins in such patients showed that CGS was diagnosed in 118 (31.1%) patients with the mean concentration of CG1.68±0.33 g/l at a rate of 0.48 ±0.10g/l. The patients with the systemic connective tissue diseases with CGS demonstrated statistically lower miR-146а expression which resulted in the abnormal production of pro-infl ammatory cytokines, the highest TLR9 expression on monocytes, slightly lower on lymphocytes, and the lowest on granulocytes; the increase in the relative amount of cytolytic T-lymphocytes, IL2 receptor lymphocytes, activated CD HLA DR+-lymphocytes against the reduction of NK-cells and regulatory suppressor CD4+/25+-cells was observed.The idiopathic and initiated oxidative monocyte capacity in CGS patients distinctly tended to increase, as compared to patients without CGS and normal individuals. Conclusions. Cryoglobulins may act as the so-called bridge between viral infections and the autoimmune processes. CGS was diagnosed in 31.1% of patients. Despite a substantial number of studies dedicated to the cryoglobulinemic syndrome, the peculiarities of the immune reaction of such patients need further research, since they create the risks of secondary vasculitis against SAD.Документ Lipid parameters before and after immunobiological therapy of patients with psoriasis(Полтавський державний медичний університет, 2022) Stepanenko, R. О.Psoriasis is one of the most common dermatoses and occupies one of the leading places among the current problems of modern dermatology. 148 patients were involved in the study. Comparative evaluation of the therapeutic efficacy of monotherapy with the immunobiological drug Adalimumab, as well as its combination with a domestic non-hormonal drug based on natural ingredients (Flaxseed oil, Solidol fatty, D-panthenol, Allantoin, Herd extract, Celandine acid extract, Celandine extract) in the examined patients with psoriasis vulgaris was performed according to the dynamics of regression of cutaneous clinical manifestations of dermatosis: reduction of lesion area, regression of erythema, infiltration, peeling of psoriatic skin rash, changes in PASI, PGA, BSA index. We have proposed a modified treatment regimen for patients with psoriasis vulgaris, which provides a course of systemic immunobiological therapy with adalimumab with concomitant use of domestic non-hormonal drug can increase the effectiveness of treatment and prolong the remission of dermatosis. Псоріаз є одним з найбільш поширених дерматозів і посідає одне з провідних місць серед актуальних проблем сучасної дерматології. У дослідженя було залучено 148 пацієнтів. Порівняльна оцінка терапевтичної ефективності застосування монотерапії препаратом імунобіологічної дії Адалімумаб, а також його комбінації з відчизняним негормональним препаратом на основі натуральних компонентів (Лляна олія, солідол жировий, D-пантенол, алантоноїд, екстракт череди, екстракт чистотілу, екстракт оману, сірка, саліцілова кислота) у обстежених хворих на псоріаз вульгарний проводилась згідно динаміки регресу шкірних клінічних проявів дерматозу: зменшення площі ураження, регрес еритеми, інфільтрації, лущення елементів шкірної псоріатичної висипки, змін індексу PASI, PGA, BSA. Запропонована нами модифікована схема терапії хворих на псоріаз вульгарний, яка передбачає проведення курсу системної імунобіологічної терапії препаратом адалімумаб з паралельним призначенням відчизняним негормональним препарату дозволяє підвищити ефективність лікування та подовжити термін тривалості ремісії дерматозу.
