This is a difficult social, scientific, and practical problem, which determines the relevance of this research. The aim of the study was to carry out a comparative analysis of caries and complicated caries indices of children in Poltava and the suburb. Materials and methods. A dental examination was carried out on 905 children 8-10 years old who studied in schools of Poltava and the suburbs. We determined the following indicators in children: the intensity of caries according to decay-missing-filled (DMF) indices in permanent, temporary, and deciduous teeth, the intensity and prevalence of complicated caries, and the need for treating. Research results and their discussion. Our epidemiological research work evidence that there is no statistically significant difference in the intensity of caries according to the DMF index between children in Poltava and in the suburb (p≥0.05). They were statistically significant in the comparison between the indicators of the gymnasium in the village Rozsoshentsy and schools in villages Shcherbany and V. Trostyanets and educational establishments of Poltava (р≤0.05). At the same time, the intensity and prevalence of complicated caries is statistically higher (p≤0.05) in children from suburbs, than in children from Poltava. The difference was statistically significant when comparing all groups, except indicators between the gymnasium in the village Rozsoshentsy and school № 10 in Poltava (р≤0.05). Conclusions. We can explain it by the low level of dental awareness of the Ukrainian population, and the closure of dental offices in schools. It leads to the impossibility of constant monitoring of dental health in children. Рівень стоматологічної захворюваності серед дітей не знижується і є значущою соціальною про- блемою, що вимагає ефективних шляхів розв’язання. Карієс зубів виникає і неухильно прогресує з раннього дитячого віку, досить швидко призводить до руйнування тимчасових і постійних зубів у ді- тей. Попри високий рівень розвитку стоматологічних технологій, карієс і його ускладнення займають чільне місце в структурі стоматологічної патології в дітей. Це стало гострою соціальною і науково- практичною проблемою, яка і визначає актуальність даного дослідження. Мета дослідження. Провести порівняльний аналіз показників карієсу й ускладненого карієсу в школярів м. Полтави і передмістя. Матеріали і методи дослідження. У рамках планової санації проведено стоматологічне обсте- ження 905 дітей віком 8-10 років, які навчалися в школах м. Полтави і передмістя. У всіх школярів ви- значали: інтенсивність карієсу за індексами кп, КПВ+кп, КПВ, інтенсивність і поширеність ускладненого карієсу, потребу в санації. Результати дослідження. Проведені епідеміологічні дослідження свідчать про те, що показники інтенсивності карієсу за індексами КПВ+кп не відрізняються між собою в дітей, які навчаються в шко- лах м. Полтави і передмістя, і були статистично достовірні в порівнянні з показниками Розсошенської гімназії із с.Щербані – В. Тростянецькою школою і навчальними закладами м. Полтави (р≤0,05). Інтенсивність і поширеність ускладненого карієсу вірогідно вища в школярів навчальних закладів передмістя, у порівнянні з учнями шкіл, що знаходяться безпосередньо в м. Полтаві. Статистично вірогідна різниця була достовірна при порівнянні всіх груп школярів, окрім показників між Розсошенсь- кою гімназією і школою №10 м. Полтави (р≤0,05). Висновок. Отримані результати можна пояснити низьким рівнем стоматологічної обізнаності насе- лення України, закриттям стоматологічних кабінетів у школах країни. Due to hypoglycemia, metabolism in the periodontal tissues is disturbed, which in turn leads to the progression of inflammatory and degenerative processes in the oral cavity.Purpose of the study. To determine the structure of dental morbidity in children with insulin-dependent diabetes mellitus. System of distance education is a virtual classroom with the possibility to train interns and doctors from different regions of Ukraine at the same time. There are many educational platforms for distance learning nowadays, such as Moodle (Australia), iSpring Learn LMS (Russia), Collaborator (Ukraine), eTutorium LMS (Ukraine), Opigno (Belgium), Atutor (Canada). Moodle is a free platform that allows users to create individual courses. It supports more than 100 languages. iSpring Learn LMS is a simple and user-friendly system that is a paid alternative to Moodle. Collaborator is a platform that works effectively on all modern devices and browsers and is virtually independent of the software of the user's device. eTutorium LMS is a virtual distance learning system that allows to create an online course of any complexity quickly. Opigno is a modern free distance education system based on Drupal (a popular content management system). Atutor, like Moodle, is an open web-based e-learning system. Conclusion. Distance learning systems differ not only functionally, but also in the way they solve problems. The intensity of caries in children with diabetes was higher (2.7 ± 1.5) than in children without con-comitant pathology (1.9 ± 1.6). The hygiene index according to Fedorov-Volodkina in the group of so-matically healthy children is 1.49 ± 0.06, which corresponds to good oral hy-giene. In the group of patients with type I diabetes mellitus, the above index is 2.08 ± 0.08, which corresponds to unsatisfactory oral hygiene. The results of assessment according to the simplified index of hygiene OHI-S (Green-Vermillion) are written below: 0.69 ± 0.05 in the group of so-matically healthy children, which corresponds to a good level of hygiene. The hygiene index OHI-S (Green-Vermillion) in the group of patients with type I diabetes is 1.74 ± 0.08, it can be interpreted as satisfactory, however, it should be noted that the range of values corresponding to the above level of hygiene is within from 0.7 to 1.8, that is almost on the border with the bad. The periodontal index in children of the first group indicates the ab-sence of gum disease. On days 5 and 12, the animals were removed from the experiment. Determination of carbohydrate components of the structural elements of the mucous membrane of the attached part of the gums was performed by lectin histochemistry. On the fifth day of observation, the study of the specificity ofreceptor binding to the fucose-specific lectin of the bark of the golden rain on the structural components of the epithelial plate ofthe attached part of the gums of rats revealed that the expression of receptors on the horny scales and basement membrane decreasedfrom moderate to weak. The reaction of receptors to the lectin of the bark of the golden rain on the basal membranes of the vessels of the hemomicrocirculatory channel increased from moderate to strong. The study aimed to determine the dental status and study the content of pro-inflammatory IL-18 in oral fluid in primary school children with insulin-dependent diabetes mellitus and to develop an algorithm for predicting IL-18 levels in oral fluid of children with chronic catarrhal gingivitis in type I diabetes mellitus background by establishing a relationship between the level of IL-18 and the assessment index of periodontal tissues according to the Sillness scale, Loe. The study examined 82 children aged six to fourteen years, including 56 with type I diabetes and 26 without concomitant somatic diseases. The IL-18 amount in the oral fluid to characterize the inflammation in the oral cavity organs was determined. The IL-18 value increases with the periodontal index and bleeding gums during probing. It is consistent with the data of scientific studies on IL-18 amount in the oral fluid of adults without diabetes. Our study results also show that an increase in the IL-18 value in oral fluid is closely related to type I diabetes in children. Moreover, in our opinion, IL-18 can be considered a potential biomarker of inflammation in periodontal disease in children with type I diabetes mellitus, the clinical manifestations of which are not yet visible. We found that IL-18 levels are elevated even in the absence of clinical gingivitis manifestations. Materials and methods: 82 children were examined, they were divided into groups by presence of gingivitis and diabetes mellitus. NO-synthase (NOS) activity was determined in oral fluid by the difference in nitrite concentration before and after incubation. The arginase activity was determined in oral fluid by the difference in the concentration of L-ornithine before and after incubation. Results: Use our treatment scheme in children with chronic catarrhal gingivitis and type 1 diabetes mellitus lead to a change in the polarization of oral macrophages towards the predominance of M2 polarization in 1 month. The polarization of macrophages changed to the predominance of M1 polarization activity in 1 year. Conclusions: We have elaborated a scheme for the treatment of chronic catarrhal gingivitis in children with type 1 diabetes mellitus. The presence of type 1 diabetes mellitus in children is a major risk factor for periodontal disease. The aim of research work was to determine the activity of NO-synthase and arginase in saliva in children age with insulin-dependent diabetes mellitus. Materials and methods. We examined 82 children including 56 children with type 1 diabetes mellitus and 26 children without somatic diseases. NO-synthase (NOS) activity was determined by the difference in nitrite concentration before and after incubation of tissue homogenate. Determining arginase activity was based on analysis the difference in the concentration of L-ornithine before and after incubation in phosphate buffer solution. Statistical processing of the results was performed with the help of Real Statistics extension for Microsoft Excel. The significance of the differences between groups was estimated by Khruskal-Wallis ANOVA test with following paired comparison by Mann-Whitney test. Research results and their discussion. The violation of the indicators’ balance between groups of healthy children with healthy gums and children with diabetes mellitus and healthy gums showed us a decrease in regenerative capacity in the mucous membrane in persons with type 1 diabetes mellitus. Increased ARG activity in group of children with type 1 diabetes mellitus and chronic catarrhal gingivitis may lead to rivalry between NOS and ARG for L-arginine. Increased ARG activity in group of somatically healthy children with chronic catarrhal gingivitis and children with type 1 diabetes mellitus and chronic catarrhal gingivitis compared with group of healthy children with healthy gums indicated an adaptive response aimed at repairing gum damage. Based on this, increased NO production from NOS is a consequence of insulin deficiency (systemic factor). Conclusions. Materials and methods: The study involved 26 children aged 9 to 16 years, patients with chronic generalized catarrhal gingivitis mild to moderate severity (CGCG) and 18 children with intact gums were comparison group. We determined the hygienic indices Fedorov, has been received, Silness-Loe, PMA, bleeding index for Myuleman and intensity of caries index CFD + cf, CFD. Histological and immunohistochemical studies were performed on serial sections kriostatnyh who made biopsy of gingival papillae. Microbiological study gingival part of crown plaque was performed by multiplexed PCR in real time. Results: Value hygienic indices in children with CGCG higher than in healthy, indicating the difficulty of care in the presence of periodontal inflammation. As a result of immunohistochemical studies revealed that HLA-DR + cells under conditions of active disease migrate to mucosal lamina propria epithelium. Number of CD3 + cells in the epithelium CGCG was significantly higher than the number in the intact epithelium and was the most numerous of population. In the biopsy of affected children significantly reduced the number of CD4 + cells. When CGCG quantitative total bacterial mass, Lactobacillus spp., Enterobacteriaceae, Gardnerella vaginalis / Prevotella bivia / Porphyromonas spp. in the sample CROWN dental plaque was significantly higher than rates under physiological conditions, and may serve as diagnostic criteria of dysbiosis. Conclusions: So, CGCG is a disease in the etiology of which is one of the leading roles played by microbial factor, namely, the value of the quantitative ratios of certain types of microorganisms of dental plaque compared to the total bacterial mass of plaque. A.; Максименко, Аліна Іванівна; Шешукова, Ольга Вікторівна; Кузь, Ірина Олексіївна; Писаренко, Олена АнатоліївнаДля сучасного етапу розвитку дитячої стоматологічної допомоги у світі характерна підвищена увага до проблем забезпечення кваліфікованою медичною лікувально-профілактичною допомогою дітей із обмеженими фізичними можливостями. В даний час найбільш поширеним неврологічним захворюванням, що діагностується у дітей в ранньому віці, є дитячий церебральний параліч (ДЦП). Полісиндромний характер даної патології визначає різноманіття клінічних проявів, в тому числі високий ризик виникнення стоматологічної патології. При ДЦП ушкоджуються кіркові, підкіркові, ство- лові структури головного мозку. Порушуються функції організму: функції м'язів, рухів, прийому їжі тощо, вплив яких на формування зубощелепних аномалій загальновідомий, як і поєднання морфологічних порушень розвитку зубощелепної системи з міофункціональними порушеннями, які роблять зубощелепно-лицевий комплекс нестійким до фізіологічних навантажень. Наприклад, формування рухового стереотипу, становлення у здорової дитини мовних функцій мають закономірності. Якщо у здорової дитини лепет вже достатньо сформований і вдосконалюється функція дихання, то у дітей з ДЦП у цей період відзначаються порушення тонусу м'язів артикуляції, обмеження довільних рухів язика і губ, оральні синкінезії, утруднення процесу годування. З цього випливає, що моторні порушення при ДЦП стосуються не лише кінцівок і тулуба, а й щелепно-лицьової ділянки. При цьому у дітей із ДЦП уповільнено формування рухових функцій, порушено координацію тонких, диференційо- ваних рухів, що призводить до неможливості правильного догляду за порожниною рота у таких дітей. Мета дослідження. Вивчення особливостей клінічних проявів дитячого церебрального паралічу в порожнині рота у дітей. Методи дослідження. Аналіз та узагальнення літературних даних, систематизація інформації про виявлені зміни стану порожнини рота у дітей з ДЦП. Наукова новизна. Вивчення та систематизація причинно-наслідкових зв′язків розвитку основних стоматологічних захворювань та їх залежність від вираженості неврологічної симптоматики дозволить розробити комплекс лікувально-профілактичних заходів, що призведе до поліпшення якості життя дітей з ДЦП. Висновки. У дітей, які страждають на ДЦП, відзначається стійка тенденція до підвищення частоти та інтенсивності каріозних уражень, помірно виражені запальні явища в пародонті. Найбільш важка зубощелепна патологія найчастіше спостерігається у дітей, які мають мовленнєві порушення. Діти з ДЦП мають підвищений ризик розвитку стоматологічних хвороб, що зумовлюється характером основного й супутніх захворювань, і тому потребують особливої уваги з боку лікаря-стоматолога щодо своєчасного проведення лікувально-профілактичних заходів із врахуванням перебігу неврологічного захворювання.Документ Oral hygiene and periodontal indices in children with 1 type diabetes mellitus during gingivitis treatment(2022) Sheshukova, O. I.; Шешукова, Ольга Вікторівна; Кузь, Ірина Олексіївна; Максименко, Аліна ІванівнаThe oral manifestations of diabetes mellitus are observed in the vast majority of patients, and some dentists indicate 100 % damage to the tissues of the oral cavity. The purpose of the study was to assess the state of periodontal disease in children with type 1 diabetes mellitus under the influence of our treatment scheme of chronic catarrhal gingivitis. The study involved 56 patients with type 1 diabetes mellitus and 26 children without concomitant diseases. We determined oral hygiene index (OHI) according to Fedorov-Volodkina and OHI–S (Green, Vermillion), PMA index, gingival index according to Loe, Silness, and the bleeding index according to Muhleman H.R. We have found a statistically significant difference in a month since the treatment completes, and in 3 month, 6 month, and a year follow-up period. Oral hygiene indices before the treatment demonstrated unsatisfactory level of oral hygiene, but they considerably improved since the treatment had started. Periodontal indices evidenced a moderate degree of the inflammatory process. These values decreased significantly and pointed out mild inflammation in 1 month after the treatment. In 3 months, 6 months and a year follow up period the above indices slightly increased, but, nevertheless, were twice as lower than before the treatment. The use of the proposed treatment scheme had a positive effect on the clinical course of periodontal diseases and contributed to the improvement both in the immediate and long-term follow up.Документ Signs of diseases of the gastrointestinal tract in the oral cavity of children(Полтавський державний медичний університет, 2021) Maksymenko, A. I.; Максименко, Аліна ІванівнаУ наш час питання поєднаних уражень порожнини рота і внутрішніх органів займають чільне місце серед проблем стоматології, оскільки дозволяють відобразити сутність генезу багатьох захворювань, що виявляються в порожнині рота. Мета. Проаналізувати відомості щодо взаємозв’язку хвороб порожнини рота із захворюваннями різних відділів шлунково-кишкового тракту. На думку багатьох вчених, при захворюваннях шлунково-кишкового тракту можуть спостерігатися зміни в порожнині рота: катаральний гінгівіт, стоматит, глосити, оскільки в цих випадках виникає дефіцит вітамінів групи В. А такі хвороби порожнини рота як хронічний рецидивуючий афтозний стоматит, плескатий лишай, хронічний рецидивуючий герпетичний стоматит, хронічний пародонтит, мають тяжчий перебіг за наявності патології травного тракту. Висновки. Отже, анатомо-фізіологічні особливості дітей, які хворіють на хронічні хвороби ШКТ, диктують необхідність вивчення клінічних проявів стоматологічних хвороб у них. Перспективи подальших досліджень. Materials and methods: 82 children were examined, they were divided into groups by presence of gingivitis and diabetes mellitus. The level of interleukin-18 in oral fluid was determined by immunoassay. Results: In patients with chronic catarrhal gingivitis and type I diabetes mellitus the level of interleukin-18 in oral fluid is the highest (70.91±7.48 pg / ml); the level of interleukin-18 in children with diabetes mellitus and healthy gums is high enough too, it is 14.87±1.11 pg / ml. Interleukin-18 is 3.41±0.25 pg / ml in healthy children with healthy gums. It is 5.74±0.27 pg / ml in somatically healthy children with chronic catarrhal gingivitis. Conclusions: We indicated that an increase in the value of interleukin-18 in oral fluid is associated with the presence of diabetes mellitus in children. Unfortunately, the mechanism that regulates the maintenance of a certain composition of saliva is still unclear. The main attention of clinical specialists is attracted by new laboratory methods of saliva analysis in order to obtain various diagnostic information. In the next few years, achievements are predicted in the field of diagnosis of diseases of the maxillofacial area, taking into account the properties of saliva. Oral fluid consists of mixed saliva, that is, a mixture of secretions of three pairs of large and many small glands of the oral cavity and organic impurities. Mineral and organic components determine the properties and functions of oral fluid. Therefore, the detection of quantitative and qualitative changes in oral fluid, especially in children, allows us to determine their role in the formation of pathological processes. In addition to the fact that the composition of the oral fluid can change in the presence of systemic diseases (especially those that affect the function of the salivary glands), the possibility of simple, safe and non-invasive collection of the researched material is important. Existing scientific works confirm that diabetes mellitus (DM) leads to dysfunction of the secretory capacity of the salivary glands, which later develops into an increased susceptibility of the body to pathogens of the oral cavity. Type 1 diabetes mellitus is also known to affect the homeostasis of the oral cavity. The study of saliva according to many clinical and biochemical indicators has advantages in comparison with methods of laboratory blood diagnosis. Unlike blood, saliva is an easily accessible biological fluid, its protein composition is largely identical to blood serum, and it is physiologically related to homeostasis. Saliva collection is carried out without invasive interventions, and it can be widely used in hygienic, toxicological, and immunological studies. Moreover, some indicators of saliva are sensitive indicators of serious systemic diseases and conditions of the body. Even short-term and minor chemical and metabolic disturbances in the body, which accompany general somatic pathological conditions, can change the rheological properties of saliva. Early detection of the disease plays an important role in the success of its treatment. If the disease is diagnosed earlier, the chances of its successful treatment will increase. If the disease cannot be cured, early diagnosis will still allow better control of the course of the disease. The aim of the study was to find out the influence of oral fluid on the development of periodontal diseases in children with type 1 diabetes mellitus. Materials and methods. The analysis included generalization and systematization of literature data on this topic. Scientific novelty. Generalization and analysis of literature data according to the role of oral fluid in the development of periodontal diseases in children with diabetes mellitus. Study of cause-and-effect relationships of endocrine and dental pathology. In the future, this will allow to improve treatment and prevention measures in pediatric dentistry and will lead to an improvement in the quality of life of such patients. Conclusions. Adequate knowledge about saliva and its role in maintaining oral health is very important. Some indicators of saliva are sensitive indicators of serious systemic diseases and conditions of the body. Early detection of the disease plays an important role in the success of its treatment.
